It was evidenced the thematic axis: Faller Elderly supported by four thematic categories: Changes caused by Falls, I am a faller, I take care of me and Prevention of the Fall. The repercussions of the fall were evidenced in the impairment of the health condition, self-care and functional capacity. We observed the naturalization of the phenomenon and the passivity with the harmful consequences of the event. Final Considerations: The fall is valued the more negative its repercussion, such as the need for hospitalization and surgery. Managing the vulnerability of the elderly, especially in primary care, evaluating their comorbidities and their internal and external environment, will minimize unfavorable consequences and the social and financial cost of hospitalizations.
RESUMOO envelhecimento populacional tem impactado nas situações de vulnerabilidade a que os idosos estão expostos. No âmbito dos agravos externos, encontra-se a queda como evento acidental e multifatorial, de relevante preocupação tanto nas áreas da saúde, quanto social, pois proporcional ao aumento da população idosa encontram-se as situações dos cenários de vulnerabilidade. Objetivo: desvelar as situações de vulnerabilidade relatadas por idosos e cuidadores em um hospital público em uma capital no sul do Brasil. Metodologia: pesquisa descritiva, qualitativa, com 16 idosos. Utilizou-se análise de conteúdo de Bardin. Resultados: evidenciou-se o eixo temático queda, com quatro categorias: como foi a queda; primeiro atendimento e hospitalização; causalidade da queda; significados e sentimentos provocados pela queda. A vulnerabilidade individual desvelou-se pelas comorbidades do idoso. A vulnerabilidade institucional revelou-se na hospitalização e nos significados da queda, alertando para o medo de cair, o sentimento de culpa, a incapacidade, a perda de autonomia, a dor e o desconforto. Conclusão: observa-se a relevância das estratégias preventivas nas dimensões da vulnerabilidade, ampliando o olhar na atenção ao idoso. Tais estratégias poderão levar a um mapeamento do itinerário da queda de cada idoso hospitalizado, diagnosticando suas necessidades para assim prestar assistência qualificada, por meio de uma prática clínica eficiente. Objective: To evaluate and compare the quality of primary care provided for children with type 1 diabetes mellitus, among the types of public health care services, based on the experience of their main caregivers. Methods: Cross-sectional study, grounded in health assessment, using interviews with 55 caregivers of children with type 1 diabetes mellitus, based on the Primary Care Assessment Tool-Child Version. Results: Most of the attributes of primary health care presented unsatisfactory scores, including the general and essential scores. A greater link with the specialized service (pediatric endocrinology clinics) was found. Although the general and essential scores did not attain satisfactory values, the specialized services achieved better results than the primary health care services, showing, from the perception of the consumers, relevant differences between services. Specialized services were perceived as regular sources of care, and better providers of primary health care practices. This point may be related to the greater contact of the participants with the professionals from the specialized services, and the readiness of these services to care for children with type 1 diabetes mellitus, suggesting fragilities in the primary health care services. Conclusion: The health care of children with type 1 diabetes mellitus, in the studied municipality, shows little presence and extension of primary care attributes, with fragmented and disconnected actions, leading to losses in the integration and expansion of the care network. ResumoObjetivo: Avaliar e comparar a qualidade da atenção primária ofertada às crianças com diabetes mellitus tipo 1 entre os tipos de serviços públicos de atenção à saúde na experiência dos seus principais cuidadores. Métodos: Estudo transversal, fundamentado em avaliação em saúde, a partir de entrevistas com 55 cuidadores de crianças com diabetes mellitus tipo 1, com base no instrumento de avaliação Primary Care Assessment Tool-versão criança. Resultados: Grande parte dos atributos da atenção primária à saúde apresentou escores considerados insatisfatórios, inclusive os escores Geral e Essencial. Foi encontrado maior vínculo com o serviço especializado (ambulatórios de endocrinologia pediátrica). Apesar dos escores Geral e Essencial não terem alcançado valores satisfatórios, os serviços especializados apresentaram melhores resultados que os serviços de atenção primária à saúde, mostrando, na percepção dos usuários, diferenças relevantes entre os serviços. Os serviços especializados foram percebidos como fontes regulares de atenção e melhores fornecedores de práticas de atenção primária à saúde. Tal apontamento pode estar relacionado ao maior contato dos participantes com os profissionais dos serviços especializados e prontidão desses serviços na atenção às crianças com diabetes mellitus tipo 1, sugerindo fragilidades nos serviços de atenção primária à saúde. Objective: to investigate the trajectory and follow-up of the health of children with diabetes mellitus type 1 in regard to the attention given by the different public health services and the relationships between the services, the child and their families, from the perspective of mothers and caregivers. Method: qualitative study with inductive thematic analysis, based on the theoretical references of vulnerability and health care networks, from in-depth interviews with 56 mothers or caregivers, in two Brazilian public health services. Results: reports marked by sufferings, challenges and notes show the centrality in the disease, fragile access, little resolution, insecurity in urgent and emergency situations and superficial bond with insufficient support to the family, and fragmentation. The specialized care is characterized by trust, the bond and the efforts to prevent the use of other health services. Conclusion: families and children are exposed to vulnerable circumstances in the longitudinal follow-up, with consequent distancing of network care. Thus, they indicate the need to expand the integration of care, intersectoral actions, social participation and coordination of care networks, in search of greater access and inclusion. Therefore, there are implications with political and financial efforts to advance access to comprehensive care and reduce vulnerabilities.
Resumo Esta revisão integrativa da literatura teve como objetivo verificar se o Método Canguru, conforme instituído no Brasil, desde a primeira etapa até o acompanhamento ambulatorial, tem influência sobre o aleitamento materno. Foram incluídas pesquisas realizadas no Brasil, publicadas em periódicos nacionais e internacionais, nas principais bases de dados, em português, inglês ou espanhol, nos anos de 2000 a 2017, disponíveis na íntegra e com a temática relacionada com o objetivo deste estudo. Foram encontrados 1328 artigos sendo excluídos artigos não realizados no Brasil, artigos de revisão da literatura e de temáticas não relacionadas com o Método Canguru, sendo então selecionados 21 estudos. As pesquisas encontradas apontaram para uma influência positiva do Método Canguru sobre o aleitamento materno e estabelecimento de vínculo entre mãe-filho. No entanto, a terceira etapa ou acompanhamento ambulatorial, não se mostrou eficaz na manutenção do aleitamento materno. Faz-se necessário maior participação da atenção básica nos cuidados domiciliares prestados ao recém-nascido pré-termo, com vistas à promoção do aleitamento materno exclusivo até os seis meses de idade e complementado até os dois anos.
Objective: To evaluate the impact of new penicillin susceptibility breakpoints on resistance rates of pneumococcal strains collected from children with pneumonia.Methods: Pneumococcal strains collected from patients admitted with pneumonia were isolated at the clinical analysis lab of Hospital de Clínicas de Uberlândia, Uberlândia, Brazil, and sent to Instituto Adolfo Lutz, São Paulo, Brazil, for further identification, serotyping and determination of antimicrobial susceptibility.Results: From April 1999 to December 2008, 330 strains of pneumococcus were sent to Instituto Adolfo Lutz; of these, 195 (59%) were collected from patients with pneumonia. One hundred strains collected from patients ≤ 12 years old were analyzed. The patients' age ranged from 1 to 12.6 years old (with mean age of 2.4 and median of 1.7 years). Forty-seven patients were male. The strains were isolated from blood (42%) and pleural fluid (58%). There were 35 oxacillin-resistant strains: according to the criteria defined by the Clinical and Laboratory Standards Institute (CLSI) in 2007 [minimum inhibitory concentration (MIC) ≤ 0.06 µg/mL for susceptibility (S), 0.12 to 1 µg/mL for intermediate resistance (IR), and ≥ 2 µg/mL for full resistance (FR)], 22 strains had IR and 11 strains had FR. According to the current breakpoints defined by the CLSI in 2008 (≤ 2 µg/mL for S, 4 µg/mL for IR and ≥ 8 µg/mL for FR), only one strain had IR to penicillin. There was resistance to co-trimoxazole (80%), tetracycline (21%), erythromycin (13%), clindamycin (13%), and ceftriaxone (one strain simultaneously resistant to penicillin). Conclusions:When the new breakpoints for in vitro susceptibility were applied, penicillin resistance rates dropped 97%, from 33 to 1%. ≤ 0,06 µg/mL para sensibilidade (S), 0,12 a 1 µg/mL para resistência intermediária (RI) e ≥ 2 µg/mL para resistência plena (RP)], 22 cepas apresentaram RI e 11, RP para penicilina.
