Objective: To evaluate and compare the quality of primary care provided for children with type 1 diabetes mellitus, among the types of public health care services, based on the experience of their main caregivers. Methods: Cross-sectional study, grounded in health assessment, using interviews with 55 caregivers of children with type 1 diabetes mellitus, based on the Primary Care Assessment Tool-Child Version. Results: Most of the attributes of primary health care presented unsatisfactory scores, including the general and essential scores. A greater link with the specialized service (pediatric endocrinology clinics) was found. Although the general and essential scores did not attain satisfactory values, the specialized services achieved better results than the primary health care services, showing, from the perception of the consumers, relevant differences between services. Specialized services were perceived as regular sources of care, and better providers of primary health care practices. This point may be related to the greater contact of the participants with the professionals from the specialized services, and the readiness of these services to care for children with type 1 diabetes mellitus, suggesting fragilities in the primary health care services. Conclusion: The health care of children with type 1 diabetes mellitus, in the studied municipality, shows little presence and extension of primary care attributes, with fragmented and disconnected actions, leading to losses in the integration and expansion of the care network. ResumoObjetivo: Avaliar e comparar a qualidade da atenção primária ofertada às crianças com diabetes mellitus tipo 1 entre os tipos de serviços públicos de atenção à saúde na experiência dos seus principais cuidadores. Métodos: Estudo transversal, fundamentado em avaliação em saúde, a partir de entrevistas com 55 cuidadores de crianças com diabetes mellitus tipo 1, com base no instrumento de avaliação Primary Care Assessment Tool-versão criança. Resultados: Grande parte dos atributos da atenção primária à saúde apresentou escores considerados insatisfatórios, inclusive os escores Geral e Essencial. Foi encontrado maior vínculo com o serviço especializado (ambulatórios de endocrinologia pediátrica). Apesar dos escores Geral e Essencial não terem alcançado valores satisfatórios, os serviços especializados apresentaram melhores resultados que os serviços de atenção primária à saúde, mostrando, na percepção dos usuários, diferenças relevantes entre os serviços. Os serviços especializados foram percebidos como fontes regulares de atenção e melhores fornecedores de práticas de atenção primária à saúde. Tal apontamento pode estar relacionado ao maior contato dos participantes com os profissionais dos serviços especializados e prontidão desses serviços na atenção às crianças com diabetes mellitus tipo 1, sugerindo fragilidades nos serviços de atenção primária à saúde. Objective: to investigate factors associated with the use and quality of Primary Health Care, as well as the occurrence of pneumonia and diarrhea in children under one year. Method: cross-sectional study conducted with 317 caregivers of 56 Family Health teams from Juazeiro do Norte, Ceará, from August 2014 to October 2015, using the child version of the Primary Care Assessment Tool, child version. Results: in the logistic adjustment, children received assistance (OR: 0.54; 0.32-0.91) and accessibility to the Primary Health Care Unit (OR: 0.49; 0.31-0.76). identified as possible protective factors for episodes of diarrhea. The variables associated with pneumonia were unsatisfactory access (ORaj: 2.13; 95% CI 1.18-3.82), a likely risk factor, and coordination (ORaj: 0.33, 95% CI 0.12-0,94), as a probable protection factor. Conclusion: primary care attendance resulted in a higher chance of avoiding hospitalization for pneumonia, a condition considered sensitive to care in this care model. Complementing family income and children's accessibility to the health unit were protective factors for episodes of diarrhea. The search to ensure the presence and extension of the Primary Health Care attributes has the potential to positively modify the child's health and, therefore, both should be ensured as rights of this population.
Bipolar disorder (BD) is a condition characterized by an alternation between phases of depression and mania. Its beginning is influenced by both genetic and environmental factors, causing neurological and psychological changes. In children, it is manifested through episodes of mood disturbance and pronounced psychosocial impairment, causing sleeping disorders and presenting a high rate of suicidal ideals. It has a very complex treatment, since you cannot neglect or exacerbate neither the episodes of mania or depression, taking them both in consideration when choosing the right medication and monitoring the symptoms and response to treatment. It is also a disease which has not a well defined process of etiology and pathophysiology. This study is a current literature review of a recurrent disease in psychiatry.
Background: Popular medicine has been used to treat some diseases including mental disorders since the ancient times. The development of this powerful tool can bring to the actual world some benefits to improve the way of taking care of mental illnesses. Objective: To show relevant studies about biopharmaceuticals and to infer the equivalence of using biopharmaceuticals, regarding to efficacy and safety as the chemical-type drugs in the treatment for mental disorders, assessing whether there are biomolecules that might work as biopharmaceuticals to treat, cure or mitigate different mental disorders. Methods: A systematic review from 01.01.2004 to 25.28.2014 was performed on the databases PubMed, Scopus and Science Direct. The search was performed with the descriptor ''mental disorders'' and with the keywords ''biopharmaceuticals'' and ''biomolecules''. The information pertinent to the study was selected, categorized and analyzed. Of the 339 articles found, 20 met the eligibility criteria. Results: Eastern culture, followed by Amazonian region concentrated most of the species related with popular treatment of mental disorders. Several species were revealed to act upon receptors or neurotransmitters in the Central Nervous System (CNS), which, sometimes, is a similar mechanism of action of a lot of well-known synthetic drugs on the market.
