Objective: To evaluate and compare the quality of primary care provided for children with type 1 diabetes mellitus, among the types of public health care services, based on the experience of their main caregivers. Methods: Cross-sectional study, grounded in health assessment, using interviews with 55 caregivers of children with type 1 diabetes mellitus, based on the Primary Care Assessment Tool-Child Version. Results: Most of the attributes of primary health care presented unsatisfactory scores, including the general and essential scores. A greater link with the specialized service (pediatric endocrinology clinics) was found. Although the general and essential scores did not attain satisfactory values, the specialized services achieved better results than the primary health care services, showing, from the perception of the consumers, relevant differences between services. Specialized services were perceived as regular sources of care, and better providers of primary health care practices. This point may be related to the greater contact of the participants with the professionals from the specialized services, and the readiness of these services to care for children with type 1 diabetes mellitus, suggesting fragilities in the primary health care services. Conclusion: The health care of children with type 1 diabetes mellitus, in the studied municipality, shows little presence and extension of primary care attributes, with fragmented and disconnected actions, leading to losses in the integration and expansion of the care network. ResumoObjetivo: Avaliar e comparar a qualidade da atenção primária ofertada às crianças com diabetes mellitus tipo 1 entre os tipos de serviços públicos de atenção à saúde na experiência dos seus principais cuidadores. Métodos: Estudo transversal, fundamentado em avaliação em saúde, a partir de entrevistas com 55 cuidadores de crianças com diabetes mellitus tipo 1, com base no instrumento de avaliação Primary Care Assessment Tool-versão criança. Resultados: Grande parte dos atributos da atenção primária à saúde apresentou escores considerados insatisfatórios, inclusive os escores Geral e Essencial. Foi encontrado maior vínculo com o serviço especializado (ambulatórios de endocrinologia pediátrica). Apesar dos escores Geral e Essencial não terem alcançado valores satisfatórios, os serviços especializados apresentaram melhores resultados que os serviços de atenção primária à saúde, mostrando, na percepção dos usuários, diferenças relevantes entre os serviços. Os serviços especializados foram percebidos como fontes regulares de atenção e melhores fornecedores de práticas de atenção primária à saúde. Tal apontamento pode estar relacionado ao maior contato dos participantes com os profissionais dos serviços especializados e prontidão desses serviços na atenção às crianças com diabetes mellitus tipo 1, sugerindo fragilidades nos serviços de atenção primária à saúde. The level of knowledge and training of nursing staff has been related to indicators of quality of care in health, although the application of indicators is still rare related to continuing education programs. The aim of this study was to use the training indicator to evaluate the efficiency of a Continuing Education Program focused on nursing staff in a Brazilian university hospital. In addition, it also checked if different nursing staff characteristics interfere in the training indicator. The number of hours that each nursing professional spent in the program was measured, and this indicator was related to professional features like professional category, unit of work, employment link and department. We note that inpatient sectors and high-complexity care feature a greater average number of hours when compared to units of lower complexity or support or administrative sectors. Another relevant point was the lower number of hours of training of nurses compared to other nursing categories, highlighting the need to consider different strategies for each nursing category. Despite this, nurses that perform assistance compared to those who perform only administrative activities showed no differences in participation. We observe that the profile of professionals was a relevant factor in participating in the program, and that this must be taken into consideration in the planning of continuing education programs. The program in this institution, which has compulsory participation during working hours, was also effective compared to the few studies that have measured the number of hours of training in hospitals of high complexity.
As Doenças Cardiovasculares (DCVs) são as principais causas de morbimortalidade e o tratamento muitas vezes são intervenções cirúrgicas. Para prevenir ou reabilitar, é preciso de mudança nas atividades diárias e seus hábitos. Objetivos: identificar o perfil, principais cirurgias e analisar as atividades diárias e laborais dos pacientes antes e após cirurgia. Método: Trata-se de um estudo retrospectivo, transversal. Foi realizada no Hospital de Clínicas de Uberlândia (HCU) e a população alvo foram pacientes que se submeteram a qualquer tipo de cirurgia cardíaca no ano de 2016. Resultados: Analisados 46 prontuários, observou à prevalência do sexo masculino (76%) e a idade média foi 62 anos. Dentre eles prevaleceu Hipertensão Arterial Sistêmica, dislipidemia e diabetes como comorbidades. Das cirurgias cardíacas (67%) foram de revascularização do miocárdio. Observamos que muitos dos hábitos de vida mudaram após o procedimento cirúrgico, mas uma parcela expressiva de paciente não muda seus hábitos de vida que estão diretamente associados às comorbidades. E muitos desses hábitos estão associados à piora dos resultados dos procedimentos e da condição de vida. Hábitos como fumar, beber e se alimentar inadequadamente permanecem mesmo após um procedimento invasivo e de risco. Sendo que se mostra essencial entender o porquê dessa ausência de mudanças em alguns dos pacientes e como o atendimento multiprofissional pode auxiliar nesse processo.
