The aim of this study was to assess the possible relationship between AAION (arteritic anterior ischemic optic neuropathy) and NAION (non-arteritic anterior ischemic optic neuropathy) with blood platelet parameters and NLR (neutrophil-to-lymphocyte ratio). The medical records of 12 patients with AAION, 33 patients with NAION, and 35 healthy subjects were examined. MPV, PDW, and PCT values showed marked elevation in AAION and NAION groups compared with control group. The mean NLR was statistically significantly higher only in AAION group compared to the NAION and control groups, suggesting that platelet function plays an important role in AIONs and NLR might be used to differentiate AAION from NAION.

Ö ÖZ ZE ET T A Am ma aç ç: : Oküler motor kraniyal sinir paralizisi olan hastaların etiyolojik ve klinik seyrinin retrospektif olarak değerlendirilmesidir. G Ge er re eç ç v ve e Y Yö ön nt te em ml le er r: : Çalışmamıza, Ulucanlar Göz Eğitim ve Araştırma Hastanesi Nörooftalmoloji Bölümüne 2003-2013 yılları arasında ardışık başvuran, düzenli takipleri olan 98 hasta dâhil edildi. Tüm hastaların ayrıntılı tıbbi öyküleri alındı, nörooftalmolojik muayeneleri yapıldı. Hastaların yaşı, cinsiyeti, eşlik eden sistemik hastalıkları, etiyolojisi, pupil çapı, pitozis varlığı ve şiddeti ve Hess ekranı ile değerlendirilen göz hareketleri geriye dönük olarak değerlendirildi. Etiyolojiye yönelik olarak görüntüleme yöntemlerine başvuruldu. B Bu ul lg gu ul la ar r: : Hastaların 45 (%45,9)'i kadın, 53 (%54,1)'ü erkek idi. Başvuru anındaki yaş ortalaması 53,6±17,5 (7-90) yıl idi. Ortalama izlem süresi 6,6±8,1 (3-64) ay idi. Her iki göz tutulumu 5 (%5,1) hastada mevcuttu. En sık izole altıncı kraniyal sinir (%48,9) felci izlenirken, bunu izole üçüncü kraniyal sinir (%34,6) felci izledi. İzole troklear sinir felci ise %15,3 oranında saptandı. Birden çok oküler motor sinir tutulumu ise 2 (%2,0) hastada izlendi. Oküler motor sinir felci ile pitoz %76,4 ve anizokori %17,6 oranında birliktelik göstermekte idi. Etiyolojik dağılımda hem altıncı sinir hem üçüncü kraniyal sinir için vaskü-ler sebepler (sırasıyla %68,7; %63,8), dördüncü kraniyal sinir felcinde ise kafa travması (%37,5) ön planda idi. The mean age at presentation was 53.6±17.5 (7-90) years. The mean follow-up period was 6.6±8.1 (3-64) months. Five (5.1%) cases had bilateral involvement. The most commonly affected nerve was the sixth cranial nerve (48.9%) followed by the third cranial nerve (34.6%). Trochlear nerve palsy was identified at a rate of 15.3%. Multiple ocular nerve palsies were observed in 2 (2.0%) cases. Accompanying ptosis was found in 76.4% and anisocoria in 17.6%. The main cause was vascular disorders for both sixth and third cranial nerve palsy (68.7% and 63.8%, respectively) and head trauma for fourth cranial nerve palsy (37.5%). C Co on nc cl lu us si io on n: : Sixth cranial nerve palsy was the most common ocular motor cranial nerve palsy and a vascular disorder was the most frequent cause of these disorders.

The ultrastructural findings of our cases support that traumatically induced dysfunction of the lens epithelium may lead to an edema in superficial cortical lens fibers that subsequently undergo degeneration and produce a localized zone of vacuolization.

Purpose: This study aimed to report the visual and anatomical outcomes of intravitreal anti-VEGF treatment for diabetic macular edema (DME) in a real-world clinical setting from Turkey over 36 months.Methods: This is a retrospective, multicenter (7 cites) study. The medical records of 1072 eyes (both previously treated and naive eyes) of 706 consecutive patients with visual impairment due to center-involving DME treated with intravitreal anti-VEGF injections between April 2007 and February 2017, were reviewed. The eyes were divided into mutually exclusive three groups based on the duration of follow-up (12, 24, or 36 months). Primary outcome measures were changes in visual acuity (VA) and central macular thickness (CMT) from baseline to final visit in each cohort, frequency of visits, and intravitreal anti-VEGF injections. As secondary endpoints, VA outcomes were assessed in subgroups stratified by baseline VA [<70 ETDRS letters and ≥ 70 ETDRS letters] and loading dose status of anti-VEGF injections.Results: VA increased by a mean of 8.2 letters (12-month cohort, p<0.001), 5.3 letters (24-month cohort, p<0.001), and 4.4 letters (36-month cohort, p=0.017) at final visits. The eyes with <70 VA letters achieved more significant VA improvement at final visits in all cohorts compared with eyes with >70 VA letters (p<0.001). The mean decreases in CMT from baseline to last visits at 12-, 24-, and 36- month cohorts were -100.5, -107.7, and -114.3 µm, respectively (p<0.001). The mean number of injections given were 4.6, 2.3, and 1.8 during years 1 to 3, respectively. Patients who received loading dose showed greater VA gains than those who did not in all follow-up cohorts.Conclusion: Our study revealed that anti-VEGF treatment improved VA and CMT over a follow-up of 36 months. Although these real-life VA outcomes following anti-VEGF therapy for DME were similar to other real-life studies, they were inferior to those noted in randomized controlled trials, mainly due to undertreatment.
