Patients who present a background of bone defects seek for harmonic facial contours through surgical rehabilitation. To facilitate the maxillofacial reconstruction, there are two groups of grafts that can be used: the bone and alloplastic materials, such as the polymethyl methacrylate (PMMA), which is one of the alloplastic materials usually used by surgeons in facial rehabilitation. The aim of this study is to present a clinical report about a frontal cranioplasty using PMMA and compare its use with other materials in the literature. A male patient, 35 years old, ASA I, had a motorcycle accident, and a physical examination showed a sinking in the frontal region, deviation of right nasal dorsum and restriction of air passage through the left nostril. At the imaging examination, it was found suggestive signs of naso-orbit-ethmoid fracture and external table fracture in the middle region of the frontal bone. The patient was submitted to a surgical procedure for reconstruction of bone defect and a PMMA graft was used in the frontal bone region. PMMA is usually used because of its ease of adaptation to the bone, reduction of time of the surgery and decrease of blood loss and possibility of infection. PMMA is an attractive alternative for maxillofacial reconstructions.
Objetivo: O presente estudo consiste em relatar o caso clínico de um pacientecom fratura panfacial e abordagem cirúrgica de emergência pararealização de fixação interna rígida e sua reabilitação. Descrição do Caso:Paciente do gênero masculino, 46 anos, vítima de acidente automobilístico,deu entrada no serviço de emergência do Hospital Geral do Estado daBahia (HGE-BA) cursando com múltiplas fraturas em face. Foi planejadaabordagem, sob anestesia geral e intubação orotraqueal com derivaçãosubmentual, em razão de fratura de base anterior de crânio, fratura dosossos próprios nasais e necessidade de bloqueio maxilomandibular notrans-cirúrgico. Paciente acompanhado no pós-operatório, evoluindocom projeção facial satisfatória, oclusão estável, boa permeabilidade nasale sem deformidade dentofacial. Conclusão: As fraturas panfacias sãodesafiadoras, e seu planejamento cirúrgico deve ser estabelecido visandoo posicionamento adequadado dos fragmentos fraturados e a preservaçãodas estruturas anatômicas faciais, devolvendo função e garantindo omínimo de sequelas para o pacientePurpose: The present study consists in reporting the clinical case of apatient with panfacial fracture, with an emergency surgical approach toperform rigid internal fixation for patient rehabilitation. Case description:A 46-year-old male patient, victim of an automobile accident, was admittedto the emergency department of the Hospital Geral do Estado daBahia (HGE-BA), attending multiple fractures in the face. The approachwas planned under general anesthesia and orotracheal intubation withsubmental shunt, due to anterior skull fracture, fracture of the nasal bones and the need for maxillomandibular block in the trans-surgical. Patient followed postoperatively, evolving with satisfactory facial projection, stable occlusion, good nasal permeability, and no dentofacial deformity.Conclusions: Panfacial fractures are challenging, and their surgical planningmust be established aiming at the adequate positioning of fracturedfragments and preservation of facial anatomical structures, restoring functionand ensuring a minimum of sequelae for the patient.
O traumatismo dento-alveolar acomete grande parte das vítimas de trauma em região buco-maxilo-facial, especialmente quando se trata de quedas de altura, acidentes automobilísticos, desportivos ou agressão física. O objetivo deste trabalho consiste em relatar o caso clínico de um paciente que evoluiu com luxação intrusiva de unidades dentárias para a cavidade nasal decorrente de acidente motociclístico. Paciente do sexo masculino, 18 anos de idade, vítima de acidente motociclístico. Cursou com múltiplas lesões e fraturas no complexo buco-maxilo-facial, incluindo traumatismo dento-alveolar com intrusão das unidades dentárias 22 e 23 para a cavidade nasal. Foi realizada abordagem cirúrgica para remoção das unidades e desbridamento da região anterior da maxila, que se encontrava bastante cominuída, bem como redução de fratura dos ossos próprios do nariz. Paciente em acompanhamento pós-operatório, clínico e radiológico.
Cistos residuais são resultantes da proliferação dos restos epiteliais de Malassez em decorrência de processo inflamatório por necrose pulpar em que houve curetagem inadequada após exodontia, correspondendo ao terceiro tipo mais comum de cisto odontogênico. A lesão normalmente é assintomática, podendo assumir dimensões que levam a expansão de corticais ósseas e consequentes fraturas patológicas. Porém, em casos raros pode se tornar sintomático, devido a infecção secundária. Os cistos residuais são normalmente descobertos em exames radiográficos de rotina, caracterizados por área radiolúcida arredondada, com tamanho variável, circunscrita por linha radiopaca. O objetivo do trabalho é relatar o manejo cirúrgico de um paciente apresentando um cisto residual infectado em mandíbula, com sintomatologia dolorosa local associada. Paciente A.P.S, gênero masculino, 59 anos, compareceu ao ambulatório de cirurgia bucomaxilofacial do serviço de CTBMF UFBA/OSID referindo queixas álgicas e presença de secreção após 06 meses da exodontia da unidade 3.2. Ao exame físico, pôde-se notar presença de secreção purulenta à ordenha em alvéolo referente ao dente 3.2, sem aumento de volume. Ao exame radiográfico, observou-se área radiolúcida circunscrita, limites bem definidos, compatível com lesão cística. Foi realizada a enucleação cirúrgica da lesão, a peça cirúrgica fixada em formol a 10% foi encaminhada para análise histopatológica, confirmando a suspeita diagnóstica de cito residual. O acompanhamento de 07 meses subsequentes a cirurgia comprova que tanto o diagnóstico como o tratamento realizado obtiveram resultados satisfatórios, onde não existem sinais de recidiva da lesão e adequado reparo ósseo.
Bernard-Soulier syndrome is a rare inherited disease characterized by thrombocytopenia, presence of extremely large platelets and prolonged bleeding time. Objective: This paper aims to present an alternative management of surgical dental procedures in a female patient, faioderma, 27 years old, carrier of Bernard-Soulier syndrome. Case Description: On clinical examination and history, the patient reported a history of recurrent pericoronaritis associated with units 38 and 48, and a history of bleeding associated with menstruation. Imaging examination revealed impacted and impacted dental units. The treatment plan included exodontia of elements 18, 28, 38, and 48 under general anesthesia, transfusion of platelet concentrates (01U apheresis or 07U pool) two hours before surgery, fresh frozen plasma reserve in case of transoperative bleeding, and RBC concentrate reserve, according to the guidelines prescribed by the hematologist. At the moment, in a seven-month postoperative follow-up, the patient evolves without signs and symptoms of coagulation disorders associated with exodontia. Conclusion: Thus, it is essential that dental professionals are aware of the multidisciplinary management strategies for this group of patients in order to minimize postoperative complications.
