A atrofia dos ossos gnáticos pode estar associada à dificuldades no tratamento protético. Após uma exodontia, por exemplo, o osso sofre remodelação, perdendo volume. Assim, é de extrema importância sua preservação quando existe o planejamento da realização de implante dentário. Diferentes materiais e técnicas foram desenvolvidas e são utilizadas na cirurgia oral, como o PRF, um importante biomaterial. Dentre suas características, estão a promoção da angiogênese, controle imunológico, aproveitamento das células-tronco circulantes e proteção de feridas por cobertura epitelial. Desta forma, o objetivo deste artigo é relatar um caso de utilização de enxerto sticky bone e de membrana L-PRF, após exodontia, como forma de reconstrução do osso alveolar em região anterior de maxila, para posterior instalação de implantes dentários.
Myiasis is a common zoodermatose in individuals living in rural areas, tropical and subtropical regions, mainly in underdeveloped or developing countries. This paper aims to report a case of myiasis involving the oral cavity, in a patient with neurological deficit, presenting bilateral anterior dislocation of the mandible, as well as to demonstrate the clinical and surgical management and characteristics pertinent to this treatment. A 34-year-old male patient, melanodermic, with neuropsychiatric impairment, attended the Hospital Geral do Estado, in Salvador (Bahia), brought by his companion with a bilateral mandibular dislocation. The intraoral examination revealed partial edentulism in both arches, poor oral hygiene and massive presence of larvae in the maxilla and mandible. After clinical evaluation and complementary imaging tests, with the patient under general anesthesia and nasotracheal intubation, a compress soaked in sulfuric ether was applied to the affected sites, the larvae were mechanically removed, the wound was debrided and inspected, and the edges were reapproximated. The mandibular dislocation was reduced and immobilized with a Barton-type bandage. The patient was given Ivermectin 12mg and antibiotic therapy with Cephalothin 01g, and a new dose of Ivermectin 06mg was used two days after the procedure. After 04 days of hospitalization, the patient presented a good aspect of the wound, laboratory tests without significant changes and clinical and physiological conditions for hospital discharge. Thus, although there is no protocol treatment, the mechanical removal and debridement with the administration of anthelmintics proved effective for the treatment of this condition.
O trauma dentoalveolar é uma adversidade frequente para o sistema de saúde e necessita de diagnóstico e intervenção precoce. A gravidade dessas lesões pode variar de acordo com a sua classificação, de simples trincas em esmalte a avulsões do órgão dental e fraturas ósseas. Sendo assim, o objetivo deste trabalho é relatar um caso clínico de trauma dentoalveolar associado a fratura do bloco ósseo em região anterior de maxila, tratado tardiamente por meio de redução bidigital e esplintagem. Paciente do sexo masculino, vítima de acidente automobilístico, compareceu ao serviço de emergência 04 dias após o trauma. Durante o exame físico maxilofacial foi possível observar presença de mobilidade em bloco anterior de maxila e luxação lateral dos dentes anteriores. Na tomografia de face foi possível analisar a presença de linha de fratura em tábua vestibular e palatina da região anterior da maxila, assim como luxação lateral envolvendo as unidades 12, 11, 21 e 22. Apesar do longo período pós-trauma, o tratamento proposto foi a redução fechada, associada a instalação de contenção semirrígida, proporcionando estabilidade ao segmento fraturado e ambiente propício para correta cicatrização óssea. O manejo inicial do trauma dental tem grande influência no prognostico, no entanto, foi possível observar por meio desse caso, que o tratamento tardio apresenta resultados satisfatórios, auxiliando no reestabelecimento da função e estética do paciente de forma menos invasiva.
