Резюме. Неалкогольна жирова хвороба печінки (НАЖХП) та цукровий діабет (ЦД) 2-го типу є компо-нентами метаболічного синдрому, захворюваність на який у світі зростає з кожним роком. У статті на-ведені результати лікування 110 пацієнтів iз НАЖХП та її поєднанням iз ЦД ВступЗа даними Всесвітньої організації охорони здоров'я, у світі нараховується понад 2 мільярди (майже 30 % на-селення) людей із захворюваннями печінки. Це наба-гато перевищує поширеність цукрового діабету (ЦД) чи інших хронічних захворювань [2]. Європейська асо-ціація з вивчення печінки (EASL, 2013) повідомила, що поширеність неалкогольної жирової хвороби печінки (НАЖХП) у загальній європейській популяції стано-вить 2-44 %, у пацiєнтiв iз ЦД 2-го типу -42,6-69,5 %, а у людей iз надмірною масою тіла -до 57-74 %. По-ширеність НАЖХП у США сягає 46 %, за наявності неалкогольного стеатогепатиту (НАСГ) -12,2 % [8, 9]. На думку провідних міжнародних організацій у галу-зі гепатології (AASLD, AGA, ACG), під НАЖХП слід розуміти наявність ознак стеатозу за відсутності при-чин для вторинного накопичення жиру. Таким чином, НАЖХП -самостійна нозологічна одиниця хронічно-го ураження печінки невірусного генезу, представлена спектром морфологічних змін, що включають стеатоз печінки, НАСГ, фіброз і цироз печінки і розвиваються в пацієнтів, які не вживають алкоголь в гепатотоксич-них дозах [12]. За Міжнародною класифікацією хвороб 10-го перегляду НАЖХП відповідає шифру К76.0 та разом з алкогольною входить до рубрики «Жирова хво-роба печінки» [3].Щорічне зростання захворюваності на НАЖХП спонукає науковців до дослідження нових ланок па-тогенезу та причин виникнення стеатозу печінки. Од-ним iз важливих і недостатньо вивчених механізмів розвитку та прогресування НАЖХП є підвищення рівня гомоцистеїну в плазмі крові. Завдяки наявності у складі гомоцистеїну SH-групи він має прооксидантнi властивостi: при підвищеному вмістi його в крові він окислюється і утворює вільні радикали, що індукує пе-рекисне окислення ліпідів, порушує виділення окси-ду азоту ферментом NO-синтазою, змінює гомеостаз і активує запальні процеси у печінці через активацію нуклеарного фактора NF-kB. Високі концентрації го-моцистеїну шляхом індукування пошкодження ендо-плазматичного ретикулуму призводять до порушень регуляції стеролзв'язуючого регуляторного білка (SREBPs), підвищеного синтезу і засвоєння триацил-гліцеридів та холестерину, зумовлюючи жирову інфіль-трацію печінки.Гіпергомоцистеїнемія (ГГЦ) у плазмі крові через зниження вмiсту плазмової протеїнліпази провокує збільшення рiвнiв ліпопротеїнів низької (ЛПНЩ) та дуже низької щільності (ЛПДНЩ) у мембранах гепато-Адреса для листування з авторами: Кучма Н.Г.
Arterial hypertension (AH) belongs to the diseases with genetic predisposition that determines the necessity of research on the genetic component’s influence on this disease development. It is suggested that one of the salt-sensitive arterial hypertension potential markers may be the alpha-adducin gene because its protein product is involved in the ion transport regulation in the renal epithelium. Thus, the aim of the study was to investigate the association between ADD1 Gly460Trp-polymorphism and the AH development risk among patients with different risk factors in the Ukrainian population. The study included 232 Ukrainians: 120 patients with diagnosed arterial hypertension and 112 practically healthy individuals. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) analysis was used for ADD1 Gly460Trp-polymorphism genotyping. The ADD1 Gly460Trp-polymorphic locus is an important predictor of arterial hypertension development in the Ukrainian population, but other nongenetic factors should be considered in further studies.
Aim. To study the influence of hyperuricemia (HU) on the functional capacity of the myocardium and erythrocytes morphology in patients with arterial hypertension (AH) and to estimate the possibility of their correction using low-level laser therapy (LLLT). Materials and methods. A total of 82 patients with AH were divided into two groups depending on the level of uric acid (UA): group AH-40 patients with AH and normal UA level; group HU + AH-42 patients with AH in combination with HU. Scanning electron microscopy, 24-hour blood pressure monitoring and echocardiography were performed. LLLT course was performed for the therapeutic purpose. Results. A significantly increased degree of poikilocytosis up to 24.4 % of patients in the AH + HU group was caused by HU compared with 17.7 % of patients in the AH group. In Ukraine more than 13,000 lower limb amputations are performed per year, of which about 8,000 are due to vascular lesions, and more than 3,000 are associated with diabetes mellitus (DM). The effectiveness of photodynamic therapy (PDT) in group 1 was assessed by the VCSS-1 and VCSS-2 scales. At baseline, the total score of clinical severity of CVI was 20.9 points, and two weeks later – 15.71 points, which shows an improvement by 24.83% (p≤0.05). The effectiveness of PDT in group 2 was assessed by the S(AD) SAD-1 scales at baseline and S(AD) SAD-2 at 2 weeks. The overall score on the scale S(AD) SAD-1 was 13.91 points, two weeks later it improved by 11.65% (12.29 points), which shows a much slower rate of normalization of the clinical picture in patients with diabetes compared with CVI. The effectiveness of plasma therapy in group 1 was assessed by the VCSS-1 and VCSS-3 scales. The total score of clinical severity of CVI after 6 weeks of treatment was 9.72 points, indicating an improvement in clinical status by 53.49% from baseline (20.9 points) (p≤0.05). In group 2, the evaluation of the effectiveness of plasma therapy was based on a comparison of the scales S(AD) SAD-1 and S(AD) SAD-3. The overall score of clinical severity at 6 weeks was 6.39 points, indicating an improvement by 54.06% from baseline (13.91 points). Analysis of the ulcer healing dynamics in group 1 revealed 0 points (no active ulcers) in 87.5% of patients (28 patients), while in the remaining 4 patients the size of active ulcers was 1 point. In group 2, the dynamics of the area reduction showed slower results: in 74.2% of patients the TU area was 2 points, 0 points were achieved in 12.9% patients, in the remaining patients (12.9%) – 4 points. Due to using of combined scheme of basic therapy, supplemented with PDT at the initial stage and plasma therapy in the granulation phase, it was possible to improve the dynamics of the rate of healing of TU and reduce the duration of treatment.

На сьогодні ВООЗ розглядає артеріальну гіпертензію (АГ) як найчастішу причину смерті, якій потенційно можна запобігти. Своєчасна діагностика, проведення заходів профілактики, призначання адекватної медикаментозної терапії затримує прогресування захворювання, знижує ризик виникнення ускладнень, сприяє поліпшенню якості життя та збереженню працездатності. Виникнення і прогресування АГ асоційовано з численними факторами ризику, включно з генетичними чинниками навколишнього середовища та хибними звичками. У літературі наведено результати досліджень щодо зв'язку між кількістю споживаної солі, гіпотензивною ефективністю діуретичної терапії у пацієнтів з АГ та поліморфізмом окремих генів. Мета дослідження: вивчення впливу тіазидних діуретиків на рівень артеріального тиску залежно від Gly460Trp поліморфізму гена ADD1. Матеріали та методи. У дослідженні взяли участь 120 пацієнтів української популяції з діагнозом АГ II стадії та 112 практично здорових осіб (група контролю). для визначення типу сольової чутливості застосовували методику Weinberger M.N (1986). Визначення Gly460Trp поліморфізму гена ADD1 проводили методом ПЛР. Пацієнти були розподілені на дві групи лікування: І група (60 осіб) отримувала індапамід ретард 1,5 мг, ІІ група (60 осіб) -гідрохлортіазид 25 мг. Результати. Серед 120 хворих на АГ 75,8% -гомозиготи за G-алелем, 21,7% гетерозиготи та 2,5% -гомозиготи за Т-алелем. Хворих носіїв Т-алеля у 2 рази більше, ніж здорових осіб (р<0,05). У підгрупі Іа у носіїв генотипу GG зниження рівня середньодобового САТ (ΔСАТ) становило 16,5 мм рт.ст., а серед носіїв Т-алеля -49,8 мм рт. ст. У підгрупі Іб -12,6 та 26,3 мм рт.ст. відповідно. Гіпотензивний ефект після лікування носіїв Т-алеля І групи був у 2 рази більше. У підгрупі ІІа у носіїв генотипу GG зниження ΔСАТ становило 18,5 мм рт.ст., а у носіїв Т-алеля -29,8 мм рт.ст. (р<0,05), у підгрупі ІІб -16,4 та 31,3 мм рт.ст. відповідно. Отже, застосування тіазидних діуретиків у хворих носіїв Т-алелю дає кращий гіпотензивний результат та не залежить від солечутливості. Заключення. Хворі на АГ мають достовірно вищу частоту Т-алеля G460T поліморфного маркера гена ADD1 порівняно зі здоровими особами (12,5% vs 6,7%). Присутність у генотипі хворого на АГ Т-алеля є предиктором успішного використання тіазидних діуретиків. Ключові слова: артеріальна гіпертензія, генний поліморфізм, солечутливість, тіазидні діуретики.
The aim of the study was to investigate the effect of thiazide diuretics on blood pressure (BP) depending on Gly460Trp ADD1 gene polymorphism in arterial hypertension (AH) patients of the Ukrainian population in order to predict their individual treatment efficacy. Material and methods. The study included 232 persons: 120 patients with verified stage II AH and 112 healthy individuals. Restriction fragment length polymerase chain reaction (PCR-RFLP) was used to detect genotype (the Gly460Trp-polymorphic locus of the ADD1 gene). The patients received standard therapy, which included ACE inhibitor – ramipril 5 mg, calcium channel antagonist – amlodipine 5 mg, statin – atorvastatin 20 mg, acetylsalicylic acid 75 mg. The patients were randomized into two groups: group I (60 persons) additionally taking treatment with 1.5 mg of indapamide retard and group II (60 persons) – with 25 mg of hydrochlorothiazide. The dynamic reduction of blood pressure has been assessed every 4 weeks for 2 months. Results. Among 120 patients with AH, 91 persons (75.8 %) were homozygous for the G allele (GG), 26 persons (21.7 %) – heterozygous (GT) and 3 persons (2.5 %) – homozygous for the T allele (TT), while the G allele frequency in patients with hypertension was 0.87, and the T allele – 0.13. 98 healthy individuals (87.5 %) were homozygous for the G allele, 13 individuals (11.6 %) were heterozygous, and 1 person (0.9 %) was homozygous for the T allele. The carrier frequency of the G and T alleles was 0.93 and 0.07, respectively. Allelic distribution indicated the predominance of the G allele carriers by Gly460Trp polymorphism of the ADD1 gene among the Ukrainian population, regardless of whether AH symptoms were present. It is noteworthy that the number of the T allele carriers was 2 times large among symptomatic patients than that among healthy individuals. In patients with the T allele, the hypotensive efficacy of indapamide was almost 3 times higher than that in patients with the G allele. The antihypertensive effect of hydrochlorothiazide in patients with the GT and TT genotypes was 2 times greater than that in the GG genotype carriers depending on the presence of the T allele G460T polymorphism of ADD1 gene in the genotype. Conclusions. Allelic distribution indicates the predominance of the G allele carriers by Gly460Trp ADD1 gene polymorphism among the Ukrainian population, regardless of whether AH symptoms are present. Among patients with AH, the accumulation of the T allele G460T polymorphic marker of the α-adducin gene is 2 times more than that in healthy individuals. Patients carrying the T allele demonstrate 2 times higher hypotensive efficacy of indapamide compared with hydrochlorothiazide.
