Objectives: To compare the knowledge on surgical safety through the team-based learning methodology and lecture classes for undergraduate Nursing students, and evaluate the learning satisfaction with team-based learning. Methods: Randomized, controlled, parallel, two-arm, unblinded clinical trial developed in the Faculty of Medicine of a public university in Botucatu, Brazil. The groups included 14 students for team-based learning and 11 students for lecture classes. Results: Students’ apprehension of knowledge in the team-based learning group was significantly higher compared to the control group (p<0.002) by considering the pre-test results. After 30 days, there was no significant difference between groups. The experience with the methodology was considered positive among students. Conclusions: Team-based learning is an important pedagogic tool available and has proven effective in education and learning with students playing the role of protagonists.
Objective: To analyze whether an increase in patient severity and nursing workload are correlated to a greater incidence of adverse events (AEs) in critical patients. Method: A prospective single cohort study was performed on a sample of 138 patients hospitalized in an intensive care unit (ICU). Results: A total of 166 AEs, occurred, affecting 50.7% of the patients. Increased patient severity presented a direct relationship to the probability of AEs occurring. However, nursing workload did not present a statistically signifi cant relationship with the occurrence of AEs. Conclusion: The results cast light on the importance of using evaluation tools by the nursing personnel in order to optimize their daily activities and focus on patient safety. Resultados: Ao todo, foram evidenciados 166 EAs, que acometeram 50,7% dos pacientes. O aumento da gravidade do paciente apresentou relação direta com a chance de ocorrência de EAs. Entretanto, a carga de trabalho de enfermagem não apresentou relação estatisticamente signifi cativa, na ocorrência de EAs. Conclusão: Os resultados permitem refl etir acerca da importância da equipe de enfermagem, em utilizar instrumentos de avaliação, com o objetivo de melhorar e planejar suas ações diárias, com foco na segurança do paciente. RESUMO:A gravidade do paciente ou o número de intervenções nem sempre serão proporcionais à carga de trabalho de enfermagem. Este estudo descritivo teve como objetivo demonstrar a trajetória da construção de um aplicativo (software) com o conteúdo do Nursing Activities Score e suas características operacionais. Foi realizado um teste piloto com 12 pacientes seguindo-se a coleta de dados por 90 dias consecutivos em 123 pacientes. Houve compatibilidade na transmissão de dados do Personal Digital Assistent para o computador de mesa, via wireless. A construção do aplicativo resultou em um sistema com coleta e administração de dados e permitiu realizar a interface gráfica. A utilização do aplicativo possibilita o uso de um sistema tecnológico para aplicação diária, com alimentação de um banco de dados sobre as características dos cuidados requeridos. Conhecendo a evolução destas variáveis durante a internação, o enfermeiro poderá planejar, intervir e avaliar a qualidade do cuidado. DESCRITORES:Enfermagem. Unidade de terapia intensiva. COMPUTER APPLICATION WITH THE NURSING ACTIVITIES SCORE: AN INTENSIVE CARE MANAGEMENT INSTRUMENT ABSTRACT:Patients severity or the number of interventions will not always be proportional to the nursing staff's work load. This descriptive study aimed at demonstrating the trajectory for designing a computer application (software) with Nursing Activities Score content and its operational characteristics. A pilot test was conducted with 12 patients, which was followed by data collection from 123 patients for 90 consecutive days. Compatibility was observed in wireless data transmission from the Personal Digital Assistent to the desk computer. This application's construction resulted in a data collection and administration system, as well as enabled graphic interface. The use of the software allows for the utilization of a technological system with daily applications, with the support of a database concerning the characteristics of required care. Objective: To identify prevalence and correlate diagnosis, results, and nursing interventions in patients with acute renal injury (ARI) who were hospitalized in an intensive care unit (ICU). Methods: This was a cross-sectional study including 98 patients older than 18 years old with ARI who were undergoing hemodialysis treatment in the ICU . The study was carried out in an ICU a large public hospital located in the city of São Paulo, Brazil. For statistics analysis we used the SPSS v21.0 to estimate prevalence, the 95% of confidence interval and sample error of 0.05. Data were collected from March to July 2016 using structured interviews, anamnesis and physical exam of patients using an instrument designed by this study researchers. The main instrument was completed by the principal researcher. Nursing consultation lasted for approximately 30 minutes. Of the total sample, 10% was selected and checked randomly in order to evaluate data quality and atypical values. Two patients previously did a pilot test to verify whether information in the instrument achieved the objective of the study. Results: The 98 participatns were aged ≥60 years (33%), men (60%), and classified as pre-renal injury (54%). Prevalent diagnosis was (100%) risk of infection, risk of inefficient gastrointestinal perfusion, risk of ineffective renal perfusion, risk of electrolyte imbalance, excessive fluid volume, and risk of imbalanced fluid volume. Results (100%) were: severity of infection, access for hemodialysis, tissue perfusionabdominal organs, hydric balance, mobility, removal of toxins and renal function. Prevalent nursing interventions (100%) were: promotion against infection, control of infection, maintenance of access for dialysis, hydroeletrolitic control, urinary elimination control, acid-base control, electrolytic control, hypervolemia control, hydric control, hydric monitoring, respiratory physiotherapy, respiratory and positioning monitoring. Correlations were significant (p< 0.001) between diagnosis and nursing interventions and between nursing interventions and results. Conclusion: Main diagnosis, results and nursing interventions related with loss of renal function originated from changes of renal perfusion, volemia, hydroelectrolytic dysfunctions, and risk of infection. The number of diagnosis showed to be correlated with number of nursing interventions and nursing interventions was correlated with results. Pesquisa qualitativa orientada pelos referenciais teórico-metodológicos: Interacionismo Simbólico e Grounded Theory para compreender o processo planejamento-implementação da Sistematização da Assistência de Enfermagem (SAE), segundo dois grupos amostrais: enfermeiros e auxiliares/técnicos de enfermagem de um hospital universitário, e desenvolver uma síntese dos modelos teóricos representativos dessas experiências. A saturação teórica configurou-se mediante a análise da 24ª entrevista não diretiva de 12 enfermeiros e de 12 técnicos de enfermagem, lotados em unidades de internação. Da análise emergiram dois modelos teóricos, cuja síntese originou o terceiro, intitulado "Entre o êxito e a frustração com a operacionalização da SAE: recursos humanos como componente determinante para a visibilidade do enfermeiro no processo de trabalho". Este modelo desvela o déficit de recursos humanos, impulsionando o enfermeiro a realizar uma SAE ilusória, e perpetuando um processo cíclico de sofrimento, por vivenciar a invisibilidade de sua práxis no processo de trabalho.
Objective: To compare and identify the working environment of accredited and non-accredited public hospitals. Methods: A quantitative approach with two cross-sectional studies in parallel, with 106 nurses from the hospitals, conducted between January and September of 2014. Hospital A was not accredited, and Hospital B was accredited. Data collection used a questionnaire with sociodemographic labor information and the Nursing Work Index -Revised -Brazilian version (B-NWI-R), with 57 items and four domains. The reliability of the instrument was measured using Cronbach's alpha. Results: The participants included 50 nurses of Hospital A and 56 of Hospital B. Hospitals were compared; there were statistically significant wage satisfaction (Hospital A showed greater salary satisfaction) and working hours (30-36 hours per week in Hospital B;40 hours in Hospital A). The environment was favorable to the four domains of B-NWI-R in both hospitals, regardless of accreditation status. Conclusion: The administration of the instrument showed that hospital accreditation did not affect the nurses' work environment. ResumoObjetivo: Comparar e identificar o ambiente de trabalho de hospitais públicos sendo um acreditado e o outro não. Métodos: Abordagem quantitativa, dois estudos transversais em paralelo, com 106 enfermeiros dos hospitais entre janeiro a setembro de 2014. Hospital A, não acreditado, e B acreditado. Coleta de dados por meio de um questionário com dados sóciodemográficos laborais e Nursing Work Index -Revised -Versão Brasileira (B-NWI-R) com 57 itens e quatro domínios. A confiabilidade do instrumento foi medida pelo alpha de Cronbach. Resultados: Participaram 50 enfermeiros do hospital A e 56 do B. Comparados os hospitais, houve significância estatística na satisfação salarial (hospital A tem maior satisfação com o salário) e na jornada de trabalho (de 30 a 36 horas semanais no B enquanto no A 40 horas). O ambiente foi favorável aos quatro domínios da B-NWI-R em ambos hospitais, independentemente da acreditação ou não. Conclusão: A aplicação do instrumento evidenciou que a acreditação hospitalar não interferiu no ambiente de trabalho dos enfermeiros
