Eccrine porocarcinoma (EPC) is a rare tumor. It develops from the intraepidermal ductal portion of the eccrine sweat gland. Metastatic disease is rare. We report a new case of metastatic eccrine porocarcinoma with a successful management and a good response to docetaxel. A 54-year-old man was admitted with a mass in the breast. Biopsy specimen found carcinomatous tumor proliferation with large anastomosing ducts. Cellular atypia were noted, with eosinophilic cytoplasm and round to oval nuclei. The tumor showed positive immunoreactivity for ACE and negative to anti-PS-100. Resection was performed. One year later, he presented with local and metastatic recurrences. The patient had received 3 cycles of cisplatin and 5-fluorouracil; he progressed with increase in mass size and number of lung lesions. He has been undergoing three cycles of docetaxel with complete response in the lung and regression of the breast mass. The mass was excised. Porocarcinoma is a very rare entity and poorly understood. In the metastatic phase, it has modest or no sensitivity to anticancer treatment. Docetaxel should be considered in the metastatic eccrine porocarcinoma.
Les métastases cérébrales du cancer de l'endomètre sont rares, peu de cas ont été rapportés dans la littérature. Nous rapportons le cas d'une patiente de 62 ans qui a été traitée en 2009 pour un adénocarcinome de l'endomètre classé initialement stade Ia de la FIGO, grade 3 de l'OMS et qui a présenté deux ans après, une métastase cérébrale unique sans autres métastases à distance, traitée par une irradiation sur l'encéphale total à une dose de 30gy (10x3Gy) suivie d'une chimiothérapie à base de paclitaxel et carboplatine, avec une bonne évolution clinique et radiologique.
Peripheral primitive neuroectodermal tumors (PNET) are rare malignant tumors, affecting mostly children and adolescents and have been described in breast in eight case reports only. In this paper, we present a case of bilateral mammary ES/PNET where distinction between primary and metastatic diseases was discussed through a literature review. The aim of this work is to demonstrate that although rare, the possibility of PNET should be kept in mind while evaluating a palpable breast abnormality in a young female.
Les métastases thyroïdiennes sont très peu fréquentes. Elles peuvent de façon exceptionnelle révéler le cancer primitif. Nous rapportons le cas d'une patiente qui a présenté une dysphonie secondaire à un gros nodule thyroïdien lobaire droit. L'examen anatomopathologique de la pièce de l'hémi thyroïdectomie a révélé la présence d'un adénocarcinome d'origine pulmonaire. Le bilan d'extension a confirmé la présence d'une masse au niveau du Fowler droit ainsi qu'une métastase du trochanter fémoral droit et une récidive au niveau de la loge thyroïdienne. Une chimiothérapie à base de Paclitaxel, Carboplatine et Bevacizumab a été débuté. L’évaluation après 4 cures est en faveur d'une stabilité radiologique avec amélioration des symptômes.
La localisation ORL du synovialosarcome est rare représentant moins de 5% des tumeurs de la région. Sa prise en charge est multidisciplinaire reposant sur une chirurgie large et complète suivie d'une radiothérapie externe. Nous rapportons deux cas de synovialosarcome de l'oropharynx et du sinus maxillaire chez deux adultes jeunes traités par une chirurgie et une radiothérapie externe avec une bonne réponse locorégionale.
