IntroductionMassive gastrointestinal bleeding is an emergency that can sometimes require immediate surgery. We report the first case, to the best of our knowledge, of massive rectal bleeding due to Yersinia enterocolitica, requiring ileocecal resection.Case presentationA 41-year-old North African woman was admitted to our emergency department for massive rectal bleeding. She had a history of an iron deficiency anemia of unknown cause, and diarrhea 2 months before the admission. On admission to our emergency unit, she was in a state of hemodynamic collapse. An examination showed discolored conjunctivas, massive rectal bleeding with clots and no abdominal pain. The first medical treatment included the use of noradrenaline. An upper gastrointestinal endoscopy was performed and did not show any lesions. Computed tomography of her abdomen showed significant and hypervascular wall thickening of her terminal ileum suggestive of a tumor. Because her massive rectal bleeding worsened and her collapse persisted, an exploratory laparotomy and ileocecal resection were immediately performed on the patient. Histopathological analysis showed enteritis caused by Yersinia enterocolitica. Her outcome was favorable.ConclusionEnteritis due to Yersinia enterocolitica can take a pseudotumoral form and mislead the diagnosis of gastrointestinal bleeding.
Les métastases thyroïdiennes sont très peu fréquentes. Elles peuvent de façon exceptionnelle révéler le cancer primitif. Nous rapportons le cas d'une patiente qui a présenté une dysphonie secondaire à un gros nodule thyroïdien lobaire droit. L'examen anatomopathologique de la pièce de l'hémi thyroïdectomie a révélé la présence d'un adénocarcinome d'origine pulmonaire. Le bilan d'extension a confirmé la présence d'une masse au niveau du Fowler droit ainsi qu'une métastase du trochanter fémoral droit et une récidive au niveau de la loge thyroïdienne. Une chimiothérapie à base de Paclitaxel, Carboplatine et Bevacizumab a été débuté. L’évaluation après 4 cures est en faveur d'une stabilité radiologique avec amélioration des symptômes.
Faire une mise au point sur la place de la chimiothérapie dans le traitement des cancers des voies biliaires. Méthode : Revue de littérature au sujet des cancers des voies biliaires et de façon plus particulière les essais relatifs à la chimiothérapie. Résultats : Les études réalisées évaluant l'apport de la chimiothérapie dans les cancers biliaires présentent des limites sur le plan méthodologique et un effectif faible. Ceci ne permet donc pas de se prononcer sur la place de cette dernière. Néanmoins, il paraît bénéfique de proposer une chimiothé-rapie ou une radio-chimiothérapie en adjuvant chez des patients sélectionnés, ainsi qu'une chimiothérapie dans les formes localement avancées et métastatiques de la maladie.

Method: Literature review of trials and recommendations on biliary cancers, and more specifically those including chemotherapy. Results: Reported studies only used small samples, and were not specific to biliary cancers. This doesn't allow us to conclude on the benefice of chemotherapy in biliary neoplasms. However, it seems to propose adjuvant chemotherapy or radiochemotherapy for these cases, as well as chemotherapy in palliative care.
