RESUMOIntrodução: A osteonecrose medicamentosa dos maxilares (ONM) é uma patologia que acomete com frequência os ossos da face, principalmente a mandíbula, devido à limitada vascularização e pouca maleabilidade óssea. Metodologia: Paciente compareceu ao Serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital do Oeste -Bahia, devido a dor e presença de secreção após o uso de prótese dentária inferior, relatando uso prévio de Alendronato de Sódio. Ao exame físico observou-se exposição óssea em região de corpo mandibular bilateral com presença de secreção purulenta. O exame imaginológico evidenciou áreas de sequestro ósseo em região de corpo mandibular bilateral, com limites definidos e envolvida por halo radiolúcido. Sob anestesia geral, foi realizada a remoção cirúrgica da lesão e inserção de membranas de L-PRF no leito cirúrgico. A peça cirúrgica foi coletada e enviada para exame anatomopatológico, confirmando diagnóstico clínico. Resultados: A paciente evoluiu sem queixas álgicas, com ausência de exposição óssea e remissão completa do quadro clínico inicial. Discussão: A intervenção cirúrgica é um tratamento invasivo, indicado em estágios avançados de OMN, e objetiva eliminar sequestros e regularizar margens ósseas, podendo resultar em um defeito na continuidade óssea com potencial de cura comprometido pela terapia previa, nesse contexto, o L-PRF colabora para o controle da infecção e age como barreira de proteção mecânica local. Conclusão: A associação entre intervenção cirúrgica e uso de L-PRF, se mostrou eficaz no tratamento da OMNs. Contudo, mais estudos acerca do uso do L-PRF são necessários para garantir a eficácia da terapêutica a longo prazo. Palavras-chaves:Osteonecrose Associada a Bifosfonatos, L-PRF, Mandíbula. Apesar de consagrada na literatura, a antrostomiaendoscópica nem sempre consegue fornecer acesso absoluto à mucosa do seio maxilar, fato que suscita debates a respeito da sua extensão. A antrostomia mediante acesso de Caldwell-luc é realizada há mais de 120 anos, com anestesia geral ou local, e busca garantir maior visibilidade da área explorada. Sendo assim, o presente caso versa sobre paciente do sexo feminino, 40 anos, que comparece ao ambulatório de cirurgia e traumatologia bucomaxilofacial do Hospital do Oeste -Barreiras (BA) para avaliação. Referesinusite crônica há aproximadamente um ano, após tratamento endodôntico, com constante obstrução nasal em lado direito e sem melhora com uso de medicamentos. O tratamento proposto foi a antrostomia maxilar com acesso de Caldwell-luc, com anestesia geral, seguido da instalação de dreno rígido para irrigação direta em seio maxilar, através de abertura na parede lateral da cavidade nasal do lado acometido. A antrostomia mediante acesso cirúrgico de Caldwell-luc é um procedimento clássico na literatura, tendo permanecido como técnica cirúrgica hegemônica para tratamento de sinusites até o surgimento da cirurgia endoscópica funcional, que possibilitou uma técnica menos invasiva. No entanto, os avanços da cirurgia endoscópica esbarram na limitação da área acessada neste tipo de procedimento. Apesar das possíveis complicações relacionadas à técnica, a cirurgia de Caldwell-luc permanece como um método viável para tratamento de morbidades do seio maxilar.
O traumatismo dento-alveolar acomete grande parte das vítimas de trauma em região buco-maxilo-facial, especialmente quando se trata de quedas de altura, acidentes automobilísticos, desportivos ou agressão física. O objetivo deste trabalho consiste em relatar o caso clínico de um paciente que evoluiu com luxação intrusiva de unidades dentárias para a cavidade nasal decorrente de acidente motociclístico. Paciente do sexo masculino, 18 anos de idade, vítima de acidente motociclístico. Cursou com múltiplas lesões e fraturas no complexo buco-maxilo-facial, incluindo traumatismo dento-alveolar com intrusão das unidades dentárias 22 e 23 para a cavidade nasal. Foi realizada abordagem cirúrgica para remoção das unidades e desbridamento da região anterior da maxila, que se encontrava bastante cominuída, bem como redução de fratura dos ossos próprios do nariz. Paciente em acompanhamento pós-operatório, clínico e radiológico.
As Fraturas Fronto-Naso-Orbito-Etmoidais, conhecidas como fraturas FNOE,são resultantes de um trauma contundente de alta energia no terço médioe superior da face. São pouco frequentes, mas quando presentes ocorremconcomitantemente com o trauma crânio encefálico (TCE) ou com outrasfraturas complexas da face. Apresentando uma frequência aproximada de5% a 15% dos traumas faciais em crianças e adultos respectivamente, as fraturasNOE são encontradas mais comumente após acidentes por veículosautomotores, agressão física, quedas ou acidentes ciclísticos. O complexonaso-orbito-etmoidal é uma área compacta, porém anatomicamente complexa,possuindo várias apresentações clínicas que dificultam o seu diagnósticoe tratamento. O diagnóstico e tratamento das fraturas NOE é dedifícil realização e por este motivo a realização de um exame clínico minuciosoassociado a uma boa avaliação imaginológica é de grande importâncianestes traumas, pois diagnóstico incorreto e o tratamento inadequadoou tardio geralmente resultam em sequelas estéticas e funcionais. O tratamentodeste tipo de fratura deve ser definido após a identificação da extensão,tipo de fratura e estruturas acometidas de forma a restaurar funçãoe forma face média. O objetivo deste artigo é relatar um caso de sequelade fratura NOE onde haviam queixas estéticas e respiratórias, tratado comacesso coronal e posterior redução de fratura de OPN e correção estéticade dorso nasal e glabela com o uso de cimento de polimetilmetacrilato.Fronto-Naso-Orbito-Etmoidal Fractures, known as FNOE fractures, are theresult of a blunt trauma of high energy in the middle and upper third ofthe face. It is uncommon, but when present it occurs concomitantly withtraumatic brain injury (TBI) or with other complex fractures of the face.Presenting an approximate frequency of 5% to 15% of facial traumas inchildren and adults respectively, NOE fractures are most commonly foundafter motor vehicle accidents, physical aggression, falls or cycling accidents.The naso-orbito-ethmoidal complex is a compact but anatomicallycomplex area, with several clinical presentations that make it difficult todiagnose and treat it. The diagnosis and treatment of NOE fractures is difficultto perform and therefore a thorough clinical examination associatedwith a good imaging evaluation is of great importance in these traumas,since incorrect diagnosis and inadequate or late treatment usually resultin aesthetic and functional. The treatment of this type of fracture shouldbe defined after the identification of the extension, type of fracture andstructures affected in order to restore function and form face to medium.The objective of this article is to report a case of NOE fracture sequelaewhere there were aesthetic and respiratory complaints, treated withcoronal access and subsequent reduction of OPN fracture and aestheticcorrection of nasal dorsum and glabella with the use of polymethylmethacrylatecement.
