The changing epidemiology of carbapenem-resistant Klebsiella pneumoniae in Southern European countries is challenging for infection control, and it is critical to identify and track new genetic entities (genes, carbapenemases, clones) quickly and with high precision. We aimed to characterize the strain responsible for the first recognized outbreak by an NDM-1-producing K. pneumoniae in Portugal, and to elucidate its diffusion in an international context. NDM-1-producing multidrug-resistant K. pneumoniae isolates from hospitalized patients (2018–2019) were characterized using FTIR spectroscopy, molecular typing, whole-genome sequencing, and comparative genomics with available K. pneumoniae ST11 KL105 genomes. FT-IR spectroscopy allowed the rapid (ca. 4 h after incubation) identification of the outbreak strains as ST11 KL105, supporting outbreak control. Epidemiological information supports a community source but without linkage to endemic regions of NDM-1 producers. Whole-genome comparison with previous DHA-1-producing ST11 KL105 strains revealed the presence of different plasmid types and antibiotic resistance traits, suggesting the entry of a new strain. In fact, this ST11 KL105 clade has successfully disseminated in Europe with variable beta-lactamases, but essentially as ESBL or DHA-1 producers. We expand the distribution map of NDM-1-producing K. pneumoniae in Europe, at the expense of a successfully established ST11 KL105 K. pneumoniae clade circulating with variable plasmid backgrounds and beta-lactamases. Our work further supports the use of FT-IR as an asset to support quick infection control.
A 23-year-old man presented with cough and progressive shortness of breath. Echocardiogram showed a biscupid aortic valve with a large vegetation causing severe regurgitation. Blood cultures were positive for Neisseria gonorrhoeae sensitive to cefotaxime and penicillin. Despite direct antibiotherapy, the patient required cardiac surgery with aortic valve replacement.
RESUMOIntrodução: A osteonecrose medicamentosa dos maxilares (ONM) é uma patologia que acomete com frequência os ossos da face, principalmente a mandíbula, devido à limitada vascularização e pouca maleabilidade óssea. Metodologia: Paciente compareceu ao Serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial do Hospital do Oeste -Bahia, devido a dor e presença de secreção após o uso de prótese dentária inferior, relatando uso prévio de Alendronato de Sódio. Ao exame físico observou-se exposição óssea em região de corpo mandibular bilateral com presença de secreção purulenta. O exame imaginológico evidenciou áreas de sequestro ósseo em região de corpo mandibular bilateral, com limites definidos e envolvida por halo radiolúcido. Sob anestesia geral, foi realizada a remoção cirúrgica da lesão e inserção de membranas de L-PRF no leito cirúrgico. A peça cirúrgica foi coletada e enviada para exame anatomopatológico, confirmando diagnóstico clínico. Resultados: A paciente evoluiu sem queixas álgicas, com ausência de exposição óssea e remissão completa do quadro clínico inicial. Discussão: A intervenção cirúrgica é um tratamento invasivo, indicado em estágios avançados de OMN, e objetiva eliminar sequestros e regularizar margens ósseas, podendo resultar em um defeito na continuidade óssea com potencial de cura comprometido pela terapia previa, nesse contexto, o L-PRF colabora para o controle da infecção e age como barreira de proteção mecânica local. Conclusão: A associação entre intervenção cirúrgica e uso de L-PRF, se mostrou eficaz no tratamento da OMNs. Contudo, mais estudos acerca do uso do L-PRF são necessários para garantir a eficácia da terapêutica a longo prazo. Palavras-chaves:Osteonecrose Associada a Bifosfonatos, L-PRF, Mandíbula. As fissuras labiopalatinas (FLP) são as anomalias craniofaciais de origemembrionária com maior prevalência, acometendo cerca de 2 a 3% dosrecém-nascidos, de etiologia multifatorial, que envolve predisposiçãogenética e exposição a fatores teratogênicos. O objetivo do presentetrabalho é, através de uma revisão da literatura, contribuir para o maiorconhecimento sobre os fatores etiológicos envolvidos nessa má-formação.Realizou-se uma busca nos bancos de dados PubMed, ScienceDirect e Lilacs, alem de referencias cruzadas com os artigos científicospesquisados. Foram selecionados 18 artigos, utilizando como critériosde seleção o ano de publicação, posterior a 2010, e relevância para atemática. Houve um consenso na literatura consultada quanto à multifatoriedadee quanto ao período de risco de desenvolvimento da FLP,os primeiros três meses de VIU, frente aos agentes etiológicos da máformação, sendo o uso de fumo e álcool, a exposição a radiações ionizantese agentes infecciosos e a carência nutricional os fatores ambientaismais citados. A literatura, no entanto, não apresenta consensoacerca da importância de cada um desses fatores. Conclusão: Desdeo nascimento, o indivíduo convive com a deficiência em processosfisiológicos básicos, tais como sucção, mastigação e fonação, o quegera um comprometimento nutricional, estético e psicológico, o quedenota a necessidade de estudos adicionais a respeito da etiologia.ABSTRACTCleft lip and/or palate (CLP) are the most prevalent craniofacial anomaliesof embriologic origin, affecting about 2 to 3% of the newborns,with a multifactorial etiology, involving genetic predisposition and exposureto teratogenic factors. Literature, however, does not provideconsensus on the importance of each of these factors. The objective ofthe present study is, through literature review, contribute to the greaterknowledge about the etiological factors involved in this malformation.It was conducted a search on PubMed, Science Direct and Lilacs databases,in addition to cross-references of scientific papers researched.We selected 18 articles, using as selection criteria: year of publication,after 2010, and relevance to the theme. There was a consensus in theliterature consulted regarding the multifactorial and the period of riskof developing the CLP, the first three months of IUL, against the etiologicalagents of malformation, being the use of cigarettes and alcohol,exposure to ionizing radiation and infectious agents and the nutritionaldeficiency the most cited environmental factors. Since birth, theindividual coexists with deficiency in basic physiological processes,such as sucking, chewing and phonation, which causes a nutritional,aesthetic and psychological impairment, which indicates the need foradditional studies on etiology.
