This study reveals large Algerian families with multigenerational inheritance of epilepsy. Molecular testing such as exome sequencing would clarify the genetic basis of epilepsy in some of our families.
ObjectiveAlcohol use disorders and bipolar disorder commonly co-occur and both are associated with more pejorative outcomes, thus constituting a major public health problem. We undertook this synthetic review to provide an update on this issue in order to clarify the nature of the relationship between the two disorders, improve clinical outcomes, prevent complications and therefore optimize management of patients.MethodsWe conducted an electronic search by keywords in databases MEDLINE, EMBASE, PsychINFO, published in English and French from January1985 to December 2015.ResultsThe AUD prevalence is important among BD patients in whom the effects of alcohol are more severe. However, in terms of screening, it appears that the comorbidity is not systematically sought. The concept of co-occurrence finds its clinical interest in the development of specific screening and therapeutic strategies. To date, there are only few recommendations about the management of dual diagnosis and the majority of them support “integrated” approaches.ConclusionsRecommendations should emphasize this strong co-occurrence and promote systematic screening and offered integrated cares.Disclosure of interestThe authors have not supplied their declaration of competing interest.
Introduction - Les données cliniques, épidémiologiques et neuropathologiques disponibles suggèrent une forte association entre l’épilepsie et la schizophrénie. Cette association sous entend une vulnérabilité génétique commune à ces deux affections.L’objectif de cette étude est de rechercher des variants génétiques de susceptibilité à l’épilepsie et à la schizophrénie chez une famille algérienne.Matériels et méthodes - Tous les membres disponibles de la famille codée EPI-ORASIA ont été évalués cliniquement et par des électroencéphalographies. Une analyse génétique par séquençage de l’exome entier a été réalisée après extraction de l’ADN.Résultats - Cette famille compte deux frères atteints d’épilepsie du lobe temporal, avec schizophrénie chez l’un, et trouble dépressif récurrent sur personnalité de type borderline chez l’autre. Le séquençage exonique a identifié un variant du gène RELN (rs55689103) à l’état homozygote chez l’individu avec schizophrénie, et à l’état hétérozygote chez son frère.
Conclusion - Le polymorphisme rs55689103 pourrait être impliqué dans le phénotype schizophrénique dans cette famille, mais pas dans le phénotype épileptique.Le dosage sérique de la protéine Reelin ainsi que l’étude du niveau de méthylation du promoteur du gène RELN constituent des éléments clés pour l’interprétation des résultats de cette étude.
