Low-intensity He-Ne laser radiation induces metabolic rearrangements in platelet lipids and activates some lipid-dependent systems of cell regulation as well as the synthesis of lipidlike second messengers. Modification of the lipid phase and physicochemical state of biological membranes are the molecular basis of biochemical adaptation of the platelets to laser radiation.

Целью исследования явилось изучение эффективности ремаксола у больных механической желтухой неопухолевого происхождения в зависимости от генетических полиморфизмов генов антиоксидантных ферментов. Обследованы 78 пациентов, оперированных по поводу механической желтухи (МЖ) неопухолевого происхождения. Выделены 2 группы: первая (n = 39) — пациентам в раннем послеоперационном периоде проводилось базисное лечение и вторая (n = 39) — в терапию включены и инфузии ремаксола (400 мл). В динамике проведены лабораторные и биохимические исследования: оценивали функциональные показатели печени, интенсивность перекисного окисления мембранных липидов, активность фосфолипазы A2, выраженность гипоксии и др. Также проведен молекулярно-генетический тест полиморфизмов генов супероксиддисмутазы (SOD2, C47T), каталазы (CAT, -262C/T) и глутатион-S-трансферазы (GSTP1, C114T), выделенных из венозной крови при помощи модернизированного метода Laura-Lee Boodram. Анализ генетических локусов произведен с помощью наборов DNA-Extran-1 при использовании полимеразной цепной реакции в реальном времени (CFX96 Touch™ Real-Time PCR Detection System, США). Установлено, что применение ремаксола на раннем периоде развития механической желтухи способствовало ингибированию липопереокисления, фосфолипазной активности, восстановлению функционально-метаболического состояния печени. Результаты генетического исследования показали, что полиморфизмы SOD2, каталазы и GSTP1 играют важную роль не только в прогрессировании патологии, но и эффективности терапии. Оказалось, что в группе пациентов с высокой частотой встречаемости редких генетических маркеров (C/T и T/T составили 28,2 и 33,3 %) полиморфизмов генов антиоксидантных ферментов выраженность оксидативного стресса выше. Об этом свидетельствует повышение содержания диеновых конъюгатов (ДК) на весь период исследования на 24,5 – 33,1 % (p = 0,01), малонового диальдегида (МДА) — на 22,6 – 32,6 % (p = 0,01) и значения индекса гипоксии (ИГ) — на 12,6 – 14,9 % (p = 0,01), активирование фосфолипазы (ФЛ) A2 на 23,6 – 35,6 % (p = 0,01), и ингибирование интенсификации антиоксидантной супероксиддисмутазы (СОД) на 19,8 – 26,5 % (p = 0,01). При этом эффективность ремаксола проявляется в большей степени (интенсивность ДК, МДА и ИГ уменьшалась на 29,6 – 35,6, 28,4 – 38,5 и 22,2 – 26,7 % (p = 0,02), активность ФЛ A2 падала на 39,6 – 42,5 % (p = 0,01), а СОД возрастала на 18,8 – 23,7 % (p = 0,01). Таким образом, результаты исследования являются основанием для своевременного проведения тестов на определение частоты встречаемости патологических генотипов полиморфизмов генов антиоксидантной ферментной системы при МЖ и установление их сопряженности с гомеостатическими показателями, что позволит персонифицировано прогнозировать течение патологии и оценить схему лечения больных механической желтухой.

Аim. Тo evaluate the structural and functional condition of platelets as a cellular component of the hemostasis system in chronic generalized periodontitis of different severity, and to establish its role in the progression of the disease. Materials and Мethods. Clinical studies included 83 patients (47 men and 36 women) with chronic generalized periodontitis, comparable in age and sex, divided into three groups according to the severity of the disease: the first group (n=21) – periodontitis of mild severity; the second group (n=36) – moderate degree of severity; the third group (n=26) – severe periodontitis. Methods of the study included evaluation of the structural and functional characteristics of periodontal tissues, general hygienic condition of the oral cavity, the severity of the inflammatory process according to index parameters; study of microcirculation of periodontal tissues, determination of the evidence of oxidative processes in saliva, blood plasma, platelets, study of lipid composition of platelets. Results. In chronic generalized periodontitis, pathological changes in the lipid composition of platelet membranes (an increase in the percentage of lysophospholipids and free fatty acids) were associated with systemic activation of oxidative processes and of phospholipase enzyme systems, which was accompanied by an increase in their aggregation activity.The degree of increase in the activity of platelets was associated with the severity of microcirculatory disturbances in the periodontal tissues. The condition of the cellular component of the hemostasis system correlated with the severity of the pathological process in the periodontium. In severe degree of chronic periodontitis, structural and functional changes in platelets were maximal. Conclusion. In chronic periodontitis, the systemic activation of oxidative processes leads to a modification of the lipid composition of platelet membranes, which contributes to increase in their aggregation activity playing an important role in the induction of hypercoagulation, microcirculation disturbances and progression of inflammation in the periodontal tissues.

Significant metabolic alterations in the liver were observed in dogs with modeled acute peritonitis. These changes significantly impaired detoxification function of the liver, which was seen from the increase in the titer of toxic products in the early post-surgery period not only in the lymph, but also in the blood plasma. The key pathogenic mechanism leading to acute liver failure is destabilization of cell membrane resulting from LPO, phospholipase activity, and tissue hypoxia. Activation of LPO and increase in phospholipase activity in the liver tissues were observed within 12 h after peritonitis modeling.

The purpose of work was studying of a role of systemic lipid metabolism disturbances in a pathogenesis of homeostasis disorders in the early posttraumatic period at fractures of pelvic bones in experiment and clinic. Pilot and clinical studies are put in a basis of work with application of the techniques estimating structure phospholipid membranes of erythrocytes, cells of a liver, kidneys, heart, lungs, and also intensity of the processes participating in a regulation of a lipid metabolism. Results. It is established that at a pelvic trauma change of lipids structure arise not only in a blood, but also other tissues structures (a liver, kidneys, lungs, heart) that is caused by activization of phosfolipaze systems, an intensification of lipids peroxidation process. There is a systemic lipid distress syndrome. The arisen changes - one of significant in a pathogenesis of traumatic illness. At modification of lipid structure of a biomembrane the morphological and functional condition of a cell starts suffering. It is substrate of emergence of disregulation pathologies. The received scientific facts can be the base for intelligent medical actions for the purpose of the prevention of emergence of a "favorable" background of disregulation conditions by vectorial impact on one of its main pathogenetic links - a systemic lipide distress syndrome.
