Аim. Тo evaluate the structural and functional condition of platelets as a cellular component of the hemostasis system in chronic generalized periodontitis of different severity, and to establish its role in the progression of the disease. Materials and Мethods. Clinical studies included 83 patients (47 men and 36 women) with chronic generalized periodontitis, comparable in age and sex, divided into three groups according to the severity of the disease: the first group (n=21) – periodontitis of mild severity; the second group (n=36) – moderate degree of severity; the third group (n=26) – severe periodontitis. Methods of the study included evaluation of the structural and functional characteristics of periodontal tissues, general hygienic condition of the oral cavity, the severity of the inflammatory process according to index parameters; study of microcirculation of periodontal tissues, determination of the evidence of oxidative processes in saliva, blood plasma, platelets, study of lipid composition of platelets. Results. In chronic generalized periodontitis, pathological changes in the lipid composition of platelet membranes (an increase in the percentage of lysophospholipids and free fatty acids) were associated with systemic activation of oxidative processes and of phospholipase enzyme systems, which was accompanied by an increase in their aggregation activity.The degree of increase in the activity of platelets was associated with the severity of microcirculatory disturbances in the periodontal tissues. The condition of the cellular component of the hemostasis system correlated with the severity of the pathological process in the periodontium. In severe degree of chronic periodontitis, structural and functional changes in platelets were maximal. Conclusion. In chronic periodontitis, the systemic activation of oxidative processes leads to a modification of the lipid composition of platelet membranes, which contributes to increase in their aggregation activity playing an important role in the induction of hypercoagulation, microcirculation disturbances and progression of inflammation in the periodontal tissues.
Objective — improvement of methods of treatment of chronic generalized periodontitis based on the analysis of the cytokine profile of patients. Methodology. A study of 120 patients with chronic periodontitis of varying severity aged 30 to 50 years was conducted. All patients were randomized into 5 groups according to the severity of the disease and therapy. all patients received traditional anti-inflammatory therapy. In group 4, in addition to the main treatment, patients received laser therapy sessions. In group 5, patients with severe chronic periodontitis were administered remaxol along with laser therapy. Evaluation of the condition of periodontal tissues was carried out according to clinical indices. PMA, SBI, API, OHI-s, and periodontal indices were used. The serum levels of IL-1β, IL-2, IL-4, IL-6, IL-8, IL-10, IFN-γ, and TNF-α were studied by enzyme immunoassay. Results: In chronic generalized periodontitis of moderate and severe severity, the content of all studied proinflammatory cytokines in the blood serum increases, and more significantly in the group of patients with severe disease. At the same time, the content of anti-inflammatory mediators undergoes ambiguous changes: in group I, the content of IL-4 increases significantly, and in group II it does not change significantly. In turn, IL-10 in group I does not decrease significantly, but in group II it becomes significantly less than normal. The inclusion of laser therapy and remaxol in the complex of standard measures showed high efficiency in reducing the severity of the pathological process, which was expressed in a faster subsiding of inflammatory phenomena in periodontal tissues. Conclusions: An increase in the content of proinflammatory cytokines in the blood serum in chronic generalized periodontitis was revealed, and more significantly in the group of patients with a severe course of the disease. At the same time, an imbalance of the system of pro-inflammatory and anti-inflammatory cytokines is registered. The use of laser therapy and remaxol in the complex treatment of CPT was the most effective.
Ретроспективно обследованы 76 пациентов с хроническим пародонтитом различной степени тяжести. Разделены на группы: первая (n=38)-среднетяжелая степень и вторая (n=38)-тяжелая степень. Для оценки тяжести пациентов использованы балльная классификация Энтина и Миллера и индекс Грин-Вермильона. Коагуляционно-литический гемостаз крови определен по показателям тромбоэластографии (TEG® 5000 Thrombelastograph®, США). Молекулярно-генетический тест полиморфизмов генов системы гемостаза (интегрина бета-3 (T1565C, ITGB3), интегрина альфа-2 (C807T, ITGA2) и фибриногена (G(-455)A, FGB) проведен с помощью метода полимеразной цепной реакции в реальном времени (Real-Time PCR Detection System, США). Установлено, что хронический пародонтит сопровождается большими расстройствами в системе гемостаза в виде увеличения скорости свертывания крови и снижения фибринолитического процесса, что подтверждается по данным тромбоэластографии. Одновременно результаты сравнительного анализа продемонстрировали, что выраженность гемостатических нарушений у пациентов тяжелой степени заболевания была более выраженной, чем группы сравнения на всех исследуемых этапах (до 6 суток) на 18,1-16,9% (р<0,05) (показатель R) и 31,4-47,6% (р<0,05) (показатель CI). При хроническом пародонтите полиморфизмы генов системы гемостаза (интегрина бета-3 (T1565C, ITGB3), интегрина альфа-2 (C807T, ITGA2) и фибриногена (G(-455)A, FGB) достоверно выше, чем в норме. Наличие данных мутаций в генотипе определяет снижение адаптационных резервов, а также предрасположенность к гиперкоагулемии и угнетению фибринолиза вследствие дефектов рецепторов-интегринов тромбоцитов и фибриногена. Особенно выраженными указанные патологические явления оказываются у пациентов с тяжелой формой заболевания.
Until now, chronic generalized periodontitis (CGP) has been a severe and common pathology of periodontal tissues. It is characterized by a prolonged course of a disease with periods of remission and relapse. CGP can often cause a significant dysfunction of the dentition, loss of the retaining dental apparatus and tooth loss. Local inflammatory phenomena certainly depend on changes in homeostasis at the body level. There is no doubt, that successful treatment of this severe periodontal disease is possible not only in case of local treatment, but also in case of remodeling at the body level. In this regard, systemic changes in homeostasis are of great importance, in particular information on the structural and functional state of red blood cells. The aim of the paper was to study the structural and functional state of red blood cells in various forms of CGP. Materials and Methods. The study involved 60 patients with mild CGP (n=32) and moderate CGP (n=28); the patients suffered from the disease for 5 to 15 years. The authors conducted a comprehensive examination of patients using clinical, laboratory and radiological diagnostic techniques. They also estimated structural and functional states of periodontal tissues according to various indices; erythrocytes were estimated according to the phospholipid composition of biomembranes, the level of primary and secondary products of lipid peroxidation, activity of catalase, superoxide dismutase, and phospholipase A2. Results. It has been established that inflammatory phenomena in periodontal tissues with CGP are accompanied by significant changes in the lipid metabolism of red blood cells and the dysfunction development. The severity of functional and metabolic changes in these blood elements correlates with the disease severity. Conclusion. The results are of particular importance as a separate component of chronic periodontitis pathogenesis. Chronic periodontitis is one of the initiators of the pathological reaction cascade leading to the formation of microcirculatory disorders and the occurrence of hemic and circulatory hypoxia, which is a universal link in the pathogenesis chain of any pathological process and a central problem of somatic pathology. До настоящего времени тяжелой и распространенной патологией тканей пародонта является хронический генерализованный пародонтит (ХГП), который характеризуется длительным течением с периодами ремиссий и рецидивов, часто приводит к существенной дисфункции зубочелюстной системы, утрате удерживающего аппарата зубов и потере последних. Безусловно, местные воспалительные явления во многом зависят от изменений гомеостаза на организменном уровне. Не вызывает сомнений факт, что успешная терапия этой тяжелой болезни пародонта возможна не только при местном лечении, но и при коррекции изменений на организменном уровне. В связи с этим важны сведения о системных изменениях гомеостаза, в частности о структурно-функциональном состоянии эритроцитов. Целью исследования явилось изучение структурно-функционального состояния эритроцитов при различной тяжести ХГП. Материалы и методы. В исследовании приняли участие 60 пациентов с ХГП легкой (n=32) и средней (n=28) степеней тяжести и продолжительностью заболевания от 5 до 15 лет. Проведено комплексное обследование больных с использованием клинико-лабораторных и рентгенологического методов исследования. Выполнена оценка структурно-функционального состояния тканей пародонта по различным индексам; эритроцитов – по фосфолипидному составу биомембран, уровню первичных и вторичных продуктов липопероксидации, активности каталазы, супероксиддисмутазы, фосфолипазы А2. Результаты. Установлено, что воспалительные явления в тканях пародонта при ХГП сопровождаются значительными изменениями липидного метаболизма эритроцитов с развитием дисфункциональных явлений. Выраженность функционально-метаболических изменений этих элементов крови коррелирует с тяжестью заболевания. Выводы. Выявленный факт представляет особую значимость как отдельный компонент патогенеза хронического пародонтита – одного из инициаторов каскада патологических реакций, приводящих к формированию микроциркуляторных расстройств и возникновению гемической и циркуляторной гипоксии, которая является универсальным звеном цепи патогенеза любого патологического процесса и центральной проблемой соматической патологии.
