We aimed to determine the possible relationship between sarcopenia, Helicobacter pylori and 25‐OH vitamin D. A total of 101 patients aged 65 years and over were included in the study, and grouped as sarcopenia (+) and (−) according to the skeletal muscle index (SMI) measured by bioelectrical impedance analysis (BIA). Also, patients were divided into two groups according to the presence and absence of H. pylori infection. There was no significant difference in the frequency of H. pylori infection in patients with and without sarcopenia (37.0% vs 29.7%; P = .485); 25‐OH vitamin D levels were significantly lower (18.25 [6.20‐32.00] μg/L vs 26.30 [9.33‐50.00] μg/L; P < .05) and the frequency of vitamin D deficiency (<20 μg/L) was significantly higher (80.0% vs 29.4%; P < .05) in sarcopenic patients with H. pylori infection than those without H. pylori infection. In addition, H. pylori infection was found to be associated with vitamin D deficiency in patients with sarcopenia (odds ratio: 12.53[1.48‐105.87]; P < .05). Within these findings, we concluded that the presence of H. pylori infection may lead to vitamin D deficiency in patients with sarcopenia.
ÖzAmaç: Yaşlı bireylerde bitkisel, raf üstü satılan ürünler ve vitaminleri içeren ve reçetesiz kullanılan ilaç ve tıbbi ürünler sık kullanılmaktadır. Bu çalışma ile yaşlılarda reçetesiz kullanılan ürünlerin kullanım sıklığını ve ilişkili olabilecek faktörleri değerlendirmeyi amaçladık. Materyal ve Metot: Geriatri polikliniğine başvuran 60 yaş ve üzeri 388 hasta yüz-yüze yöntemle anket formu kullanılarak değerlendirildi. Ayrıca hastaların tüm demografik özellikleri kayıt edildi. Bulgular: Çalışmaya katılan hastaların ortalama yaşı 73,85±7,36 (yaş aralığı 60-96; kadınların oranı %62,11). Tüm katılımcılar arasında reçetesiz ürün kullanım sıklığı %20,62 idi (n=80). Kadınların reçetesiz ürün kullanım sıklığı, erkeklerden daha fazlaydı (sırasıyla %72,50, %27,50, p=0,032). Reçetesiz ürün kullanımı ile medeni durum dışında yaş, eğitim, gelir düzeyi, çocuk sayısı, yaşanılan bölge ve sosyal güvence olup olmaması arasında anlamlı fark bulunamadı. Çalışmaya katılanlardan 186 yaşlıda (%47,68) polifarmasi (≥5 ilaç kullanımı) mevcuttu. En az 1 kronik hastalığı olan yaşlı oranı %79,12 (n=307) bulundu. Çalışmaya katılanların önemli bir bölümü (%62,50) reçetesiz kullandıkları ürünü doktoruna söylememişti. Sonuç: Türk yaşlılarında reçetesiz ürün kullanım sıklığı özellikle kadınlarda yüksektir. Reçetesiz ürün kullanan kişilerin çoğunluğu bitkisel veya doğal kabul edilen ürün ve kullandığı ilaçlar ile potansiyel etkileşime maruz kalmaktadır. In this study, we aimed to evaluate the prevalence and associated factors of non-prescription medication and product use in the older individuals. Materials and Methods:We assessed 388 older outpatients aged 60 and over, who were admitted to our geriatric clinic using a face-to-face questionnaire form. Also, the demographic characteristics of all patients were recorded. Results: The mean age of study population was 73.85±7.36 years (range 60-96; 62.11% female). The prevalence of non-prescription product use among all participants was 20.62 % (n = 80). The prevalence of non-prescription product use was higher in women than in men (respectively 72.50%, 27.50%, p=0.032). Apart from the marital status, there was no significant difference between non-prescription product use and age, education, income level, number of children, region of residence and social insurance existence, respectively. One-hundred-and-eighty-six participants (47.68%) in the study sample reported polypharmacy. 79.12% of older patients (n=307) had at least one chronic disease. The majority of the study participants (62.50%) did not report the usage of non-prescription medicine to their own physicians. Tip 2 Diyabetes Mellitus (DM), bireylerin ve yakın çevresindekilerin yaĢamlarını etkileyen, yaĢam kalitesini azaltan kronik bir hastalıktır. DM prevalansı yaĢ ile artmakta ve 60-74 yaĢ arasında tepe değere (%17,6) ulaĢmaktadır (1). Dünyada 2035 yılı itibariyle 592 milyon kiĢinin DM" den etkileneceği tahmin edilmektedir (2). DM tüm yaĢlarda önemli bir morbidite ve mortalite sebebi olmakla beraber yaĢla bu oranlar artmaktadır (3). YaĢlı DM" lilerde gençlere göre, miyokard infarktüsü, son dönem böbrek yetmezliği gibi DM komplikasyonları daha sık görülmektedir (4, 5). Tip 2 DM" li hastalarda ÖZET Amaç: Diyabetes Mellitus yaĢlılarda önemli morbidite ve mortalite nedeni olan kronik bir hastalıktır. YaĢlı diyabetik hastalarda insülin kullanım hataları glisemik kontrolü etkileyebilir. YaĢlı diyabetiklerin insülin eğitiminden fayda görüp görmedikleriyle ilgili yeterli çalıĢma bulunmamaktadır. Gereç ve Yöntem: ÇalıĢmaya insülin kullanan, çalıĢmaya katılmaya gönüllü, Temmuz 2015 ve Nisan 2016 tarihleri arasında polikliniğe baĢvuran 65 yaĢ üstü 112 diyabetik hasta dâhil edildi. Hastaların insülin kullanımı "Ġnsülin Kullanım Değerlendirme Formu" ile değerlendirildikten sonra insülin eğitimi verildi. Dört hafta sonra hastaların insülin kullanımı yeniden değerlendirildi. Bulgular: Hastaların yaĢ ortalaması 71,85 ± 6,36 olarak saptandı. ÇalıĢmaya katılan hastaların %76,8"i daha önce insülin kullanma eğitimi almıĢtı. % 29,1"i son bir yıl içinde eğitim alırken, %70,9"u bir yıldan önce eğitim almıĢtı. Buna rağmen hastaların hepsinin insülin kullanım basamaklarından en az bir tanesinde hata yaptığı görüldü. Eğitim sonrası ise hatalı uygulamalarda anlamlı oranda düzelme saptandı. Sonuç: Bu çalıĢmada yaĢlı diyabetiklere insülin eğitimi verilmesi ile insülin kullanım hatalarının büyük ölçüde azaldığı görülmüĢtür. Doğru insülin kullanımının kalıcı olması için bu eğitimin belli aralıklarla tekrarlanmasını önermekteyiz. Tekrarlı insülin eğitimi ile uzun dönem laboratuar verilerinde iyileĢme sağlanacağını ve diyabetle ilgili komplikasyon oranlarının azalacağını düĢünmekteyiz.
Geriatrics focuses on health care of the elderly. Geriatrics has a history of about 50 years in Turkey. In order to increase the quality and quantity of geriatric services, geriatric awareness needs to be increased. This is possible only when the government, health workers and the community work together. We decided to do this study to determine the level of public awareness of geriatrics among elderly population in Turkey. Materials and Methods: Between January 2018 and March 2019, we reached a total of 314 people over 60 years of age living in eight different districts of Ankara. It was questioned whether the patients knew geriatrics, and if they knew and truly described geriatrics, and they were asked how they had learned. Patients who knew geriatrics were asked whether they applied to any of the centers with a geriatrician in Ankara. Results: Only 2.88% of the study population had geriatrics awareness. In general, participants with geriatrics awareness were found to be older in age, having higher mental scores and having less children. Conclusion: In our country, the level of awareness of geriatrics is very low in its 50 th year.
