Одним из самых частых и тяжёлых осложнений сахарного диабета является диа-бетическая полинейропатия, приводящая у многих пациентов к образованию язв и ампутации стоп. Современные методы диагностики нейропатии или не позволяют выявлять изменения на ранних стадиях, или включают инвазивные процедуры. Офтальмологи, оказывающие помощь пациентам с диабетом, по объективным причинам фокусируются на диабетической ретинопатии. Однако дан-ные о том, что состояние роговичных нервов является маркером периферической нейропатии, пред-полагает новую и очень важную роль офтальмолога в ведении пациентов с сахарным диабетом. В ряде исследований получены обнадёживающие данные о том, что структурные и функциональные изменения сетчатки выявляются у пациентов с диабетом до появления ретинопатии, что позволяет предполагать роль нейропатии в их происхождении. * Ключевые слова: сахарный диабет; диабетическая полинейропатия; сетчатка; оптическая коге-рентная томография; периметрия.
OPHTHALMIC MARKERS OF THE DIABETIC POLYNEUROPATHY
Monitoring intraocular pressure in patients with open-angle glaucoma at different stages of the development of the disease using self-measurement by a portable Icare® HOME tonometer. In study, patients were divided into 3 groups depending on the treatment prescribed. With the help of near-day monitoring, hidden IOP elevations that are not recorded during a single IOP measurement on an outpatient appointment with a doctor were detected. Perspective possibilities of prescribing drugs and regulating the mode of instillation on the basis of individual time periods of increasing intraocular pressure on the example of one of the patient. Assessment of the convenience of the method from the personal experience of using the device by patients.
The article presents a case of Urrets-Zavalia syndrome development in a patient after a subtotal penetra ting keratoplasty, carried out for corneal opacity. Since this syndrome was diagnosed in time, it was possible to avoid further complications and worsening of visual functions in 25-year-old patient.
The article reviews the literature on the classification of endothelial-epithelial dystrophy (EED) of the cornea. The description of endothelial structure, etiology and pathogenesis of the corneal EED are described. Based on own multiple observations and modern methods of corneal tissue imaging, a new modification of the EED classification by V.V. Volkov and M.M. Dronov was created and is presented in the article. This classification makes it possible to more accurately determine the etiopathogenesis of changes in corneal layers and to choose a most rational and effective treatment method.
