ResumoO texto debate a situação da atenção integral em Saúde do Trabalhador no Brasil. Os avanços e as contradições das políticas públicas no campo são analisados a partir do contexto de crescimento econômico brasileiro, que ocorre em detrimento das políticas sociais, da insuficiente ação do Estado no âmbito da prevenção e, por fim, dos desafios colocados aos serviços pela recém-publicada Política Nacional de Saúde do Trabalhador e da Trabalhadora. Pretende-se assim fomentar a discussão, o entendimento, a implementação e o avanço das ações em Saúde do Trabalhador, que deveriam estar no centro das políticas públicas. This article analyzes reports and data from the investigation of severe and fatal work-related accidents by the Regional Institute of Criminology in Piracicaba, São Paulo State, Brazil. Some 71 accident investigation reports were analyzed from 1998, 1999, and 2000. Accidents involving machinery represented 38.0% of the total, followed by high falls (15.5%), and electric shocks (11.3%). The reports conclude that 80.0% of the accidents are caused by "unsafe acts" committed by workers themselves, while the lack of safety or "unsafe conditions" account for only 15.5% of cases. Victims are blamed even in situations involving high risk in which not even minimum safety conditions are adopted, thus favoring employers' interests. Such conclusions reflect traditional reductionist explanatory models, in which accidents are viewed as simple, unicausal phenomena, generally focused on slipups and errors by the workers themselves. Despite criticism in recent decades from the technical and academic community, this concept is still hegemonic, thus jeopardizing the development of preventive policies and the improvement of work conditions.
Partindo de uma retrospectiva histórica da reabilitação profissional, o objetivo do presente ensaio é refletir sobre as possibilidades de se construir uma política pública, tendo como objetivo a real reinclusão social dos acidentados e adoecidos e não um mecanismo de desfecho burocrático para a redução de custos com benefícios previdenciários. Voltado para os trabalhadores com restrições decorrentes de acidentes traumáticos, o serviço de reabilitação profissional do Instituto Nacional de Seguro Social (INSS), existente até os anos 1990, mostrava-se insuficiente para promover a reinserção de trabalhadores com agravos de perfil epidêmico, particularmente Lesões por Esforços Repetitivos/Distúrbios Osteomusculares Relacionados ao Trabalho (LER/DORT). Nessa época, reflexo da política privatizante, promoveu-se um desmonte nessas estruturas dentro do INSS e a delegação da reinserção dos trabalhadores às empresas. A construção de uma política pública de reabilitação profissional exige: a inserção da saúde do trabalhador nas políticas de desenvolvimento econômico; a desconstrução da cultura e da máquina previdenciária voltada prioritariamente para os custos; a real articulação da Saúde e da Previdência Social em projetos nacionais e locais; a inclusão do caráter distributivo nos planos de modernização; o monitoramento da trajetória dos trabalhadores; e a transparência institucional.
Introdução: o setor sucroalcooleiro vem apresentando franca expansão no país nos últimos anos, contando com cerca de 400 usinas processadoras, mais de 1 mil indústrias de suporte e gerando 1 milhão de empregos diretos. Objetivo: compreender, no trabalho dos cortadores de cana-de-açúcar, os determinantes organizacionais que intensificam a carga de trabalho e afetam a saúde dos trabalhadores. Métodos: utilizou-se a Análise Ergonômica do Trabalho, integrada com a avaliação de aspectos fisiológicos e ambientais. Foram avaliados 40 trabalhadores de uma turma de cortadores escolhida por conveniência. A sobrecarga térmica foi monitorada, bem como a frequência cardíaca e a produção diária de cada trabalhador. Resultados: o corte manual da cana durou em média 8 horas diárias de trabalho, com ritmos intensos, alta frequência de movimentos repetitivos e exigências posturais inadequadas, associadas a condições insalubres. Conclusão: o efeito nocivo das variáveis fisiológicas e do aumento da carga cardiovascular foi demonstrado. O ritmo de trabalho é acelerado por medidas gerenciais e organizacionais, com destaque para o pagamento por produção, responsável pelo aumento do desgaste físico dos trabalhadores, ultrapassando seus limites fisiológicos. Recomenda-se, dentre outras medidas, a alteração na forma de remuneração do trabalho no corte manual de cana.
