Introdução: o setor sucroalcooleiro vem apresentando franca expansão no país nos últimos anos, contando com cerca de 400 usinas processadoras, mais de 1 mil indústrias de suporte e gerando 1 milhão de empregos diretos. Objetivo: compreender, no trabalho dos cortadores de cana-de-açúcar, os determinantes organizacionais que intensificam a carga de trabalho e afetam a saúde dos trabalhadores. Métodos: utilizou-se a Análise Ergonômica do Trabalho, integrada com a avaliação de aspectos fisiológicos e ambientais. Foram avaliados 40 trabalhadores de uma turma de cortadores escolhida por conveniência. A sobrecarga térmica foi monitorada, bem como a frequência cardíaca e a produção diária de cada trabalhador. Resultados: o corte manual da cana durou em média 8 horas diárias de trabalho, com ritmos intensos, alta frequência de movimentos repetitivos e exigências posturais inadequadas, associadas a condições insalubres. Conclusão: o efeito nocivo das variáveis fisiológicas e do aumento da carga cardiovascular foi demonstrado. O ritmo de trabalho é acelerado por medidas gerenciais e organizacionais, com destaque para o pagamento por produção, responsável pelo aumento do desgaste físico dos trabalhadores, ultrapassando seus limites fisiológicos. Recomenda-se, dentre outras medidas, a alteração na forma de remuneração do trabalho no corte manual de cana.
Self-reported measures have been used to obtain weight and height information in some epidemiological surveys. The validation of such information is necessary to guarantee data quality. This study assessed the validity of self-reported weight and height to determine weight status. Data were obtained in the Brazilian National Health Survey, a Brazilian household-based nationwide survey carried out in 2013. In this survey, 40,366 individuals (aged ≥ 18 years) provided self-reported and measured information about weight and height. Student's paired t-test was used to verify the differences between self-reported and measured data. The agreement between measurements was obtained using the intraclass correlation coefficient (ICC) and Bland-Altman method. To evaluate variations in weight status categorizations, the weighted kappa coefficient and exact agreement were used. Sensitivity and specificity were estimated for the self-reported information to classify overweight and obese individuals. There was high agreement between self-reported and measured weight, height, and body mass index (ICC > 0.88). The mean agreements estimated by the Bland-Altman method were 99.6% for weight and 100.6% for height. The weighted kappa coefficient showed substantial agreement among the weight status categories (> 0.66); the exact agreement was 77%. Sensibility and specificity for overweight (83% and 87.5%, respectively) and obesity (73.4% and 96.7%, respectively) were considered high for the sociodemographic characteristics evaluated. According to our results, self-reported measurements of weight and height can be used cautiously as valid alternatives to determine weight status.
Objective: To describe the working conditions and aspects related with food consumption amongst manual workers in sugar-cane crops intending to contribute to developing public policies towards workers' health surveillance and delivering comprehensive services. Methods: Direct observation at the work field in upstate São Paulo and a semistructured questionnaire were conducted with a group of forty sugar-cane migrant workers who came from Ceará state to work as hand harvesters in São Paulo state, over 15 days during the 2007/2008 crop season. Socio-demographic data, water consumption, food consumption and cultural habits, hydroelectrolyte reposition, work pauses, body pains and duration of working days were registered. Results: Workers ingested 5 to 10 liters of water/day and the dilution of electrolytes replacement was below the adequate recommendations. Food consumptions during the crop season did not ensure food and nutritional security. Food consumption was monotonous, conserved and consumed at inadequate temperature, and incompatible with cultural habits, implying reduction and wastage of food. Workers reported pains and cramps during the work day. Pauses for resting were insufficient. Payment by results, the working process and payment practices were taken as determinants of a wide range of precarious conditions to which these workers were subjected. The hand harvesting of sugar-cane is extenuating and the payment by results may be a grievance for health as it implies reducing the work resting pauses. Food consumption and adequate hydration could minimize the working wear and pains during the job. ResumoObjetivo: Descrever as condições de trabalho e aspectos relacionados à alimentação do trabalhador no corte manual de cana para contribuir para desenvolver políticas públicas de vigilância e atenção integral em Saúde do Trabalhador. Métodos: Foram utilizados a observação direta do trabalho no campo em colheita no estado de São Paulo e aplicado questionário semi-estruturado a um grupo de 40 cortadores manuais de cana migrantes procedentes do estado do Ceará, durante 15 dias da safra 2007/2008. Aspectos sócio-demográficos, consumo de água, reposição eletrolítica, alimentação e aspectos culturais, pausas, dores e a jornada de trabalho foram registrados. Resultados: Os trabalhadores ingerem de 5 a 10 litros de água/ dia e a diluição dos repositores eletrolíticos foi inferior à adequada. A alimentação durante a safra não garantia a segurança alimentar e nutricional. A alimentação foi monótona, conservada e consumida em temperatura inadequada, e incompatível com os hábitos culturais, gerando desperdício e redução do consumo alimentar. Os trabalhadores relataram dores e câimbras no decorrer da jornada. As pausas para descanso foram insuficientes. The scope of this paper is to describe the work of manual sugarcane harvesters, assessing the nutritional behavior and body composition between the beginning and the end of the harvest. A descriptive longitudinal study was made of harvesters in Piracicaba, São Paulo, Brazil, who answered a socio-demographic questionnaire and authorized measurement of Body Mass Index, Body Fat Percentage and Arm Muscle Circumference at three stages. Creatine kinase on the skeletal isoform, C-reactive protein and plasma urea were measured at the end of the harvest. Thirty male migrant harvesters with ages ranging from 18 to 44 from the Northeast (Ceará) were assessed over a nine-month period. The workers suffered significant body fat and weight loss in the first half of the harvest. Eighteen workers had abnormal levels of creatine kinase and four - out of 24 who had donated blood - had altered urea levels. Sugarcane harvesting work causes weight and body fat loss and gains in the lean body mass index, which suffers wear-out when working on consecutive harvests. It can also cause changes in biochemical markers of chronic systemic inflammation. Further studies will make it possible to comprehend the relationships between stress, wear-out, labor longevity and health in sugarcane harvesting.
OBJECTIVETo quantify the occurrence of deaths directly associated with urban violence among fatal work-related accidents.METHODSVerbal autopsies were performed with the relatives and coworkers of residents of Campinas, state of São Paulo, Brazil, who died from external causes in 2015. We have also analyzed police reports and reports of the Legal Medical Institute related to these deaths.RESULTSWe have identified 82 fatal work-related accidents in Campinas in 2015, of which 25 were murders, 35 were traffic accidents not directly related to work activities, and three were suicides at work. The proportional mortality rate for homicides, traffic accidents, and suicides among fatal work-related accidents was estimated at 30.5%, 42.7%, and 3.7%, respectively.CONCLUSIONSUrban violence accounted for three-fourths of the fatal work-related accidents recorded in the period studied.
CONTEXT AND OBJECTIVE: Meat is a food with high nutritional density that has significant participation in the Brazilian diet. However, in excess it can cause harm to health. The aim of this study was to analyze the meat intake (g/day) among adults according to sociodemographic, behavioral and health situation characteristics, and to assess the types of meat most consumed. DESIGN AND SETTING: Cross-sectional population-based study conducted in the city of Campinas, São Paulo, Brazil, in 2008 and2009. METHODS: Two-stage cluster sampling was used. The analysis included 948 adults between 20 and 59 years, who were participants in the Campinas Health Survey. Meat intake was assessed using 24-hour dietary recall. RESULTS:The mean meat intake adjusted for sex and age was 182.3 g (95% CI: 170.6-193.9 g), with significantly lower intake among women, individuals aged 50 years or over, those with the presence of two or more self-reported chronic diseases and those with three or more health complaints. Higher meat intake was found in segments with intermediate monthly family income (between 1 and 3 minimum wages), those with 16 or more appliances per household and those who consumed soft drinks seven days a week. Beef was consumed most frequently (44%) among the meats in the diet, followed by poultry, fish and pork. CONCLUSION: The data from this study reveal high meat intake in the population of Campinas and identify the segments that need to be prioritized for strategies directed towards appropriate meat intake. Introdução: A segunda edição do Guia Alimentar para a População Brasileira utiliza a classificação NOVA de alimentos nas recomendações para uma alimentação saudável. Essa classificação, portanto, desempenha importante papel na formação de nutricionistas. Objetivo: Comparar o conhecimento da classificação NOVA de alimentos entre estudantes ingressantes e concluintes de Nutrição. Métodos: Estudo transversal realizado em 2016 com estudantes ingressantes e concluintes de um curso de Nutrição do Mato Grosso do Sul. Questionário contendo 30 alimentos foi aplicado com estudantes que os classificaram em: in natura ou minimamente processado, ingrediente culinário processado, processado ou ultraprocessado. O conhecimento foi avaliado por meio de escore de classificações corretas, global e por grupo alimentar. Para análise comparativa, foi utilizado teste U de Mann-Whitney (significância p≤0,05). Resultados: Participaram 69 estudantes (64% ingressantes). O escore global de classificações corretas foi significativamente maior (p=0,000) nos concluintes (mediana=17) comparado aos ingressantes (mediana=14). O escore de classificações corretas entre ingressantes e concluintes apresentou diferenças significativas para todos os grupos alimentares, excetuando ingredientes culinários (p=0,117). Entre todos estudantes, o grupo ingrediente culinário obteve o menor percentual de acertos (24,1%), e ultraprocessado, obteve o maior percentual (77,8%). Conclusão: Apesar do maior conhecimento dos concluintes em relação aos ingressantes, identificou-se um conhecimento insatisfatório sobre a classificação NOVA em ambos. É importante garantir o aprendizado de estudantes de Nutrição quanto à NOVA, uma vez que esta classificação é central na orientação nutricional da população brasileira e, portanto, passa a ser ferramenta fundamental na atuação do profissional nutricionista.
