In this pilot case-control study, immunonutrition is associated with a decrease in postoperative complications, urinary tract infections, Clavien's grade for complications, and paralytic ileus in patients undergoing cystectomy for bladder cancer. Prospective randomized placebo control studies are needed to confirm these promising results.
Objective To develop experimental models of erythrocyte transfusion, the first step is to ensure the viability of the red blood cells transfused. In this pilot study, we assessed the viability of transfused red blood cells with validation in vitro and in vivo of homologous swine erythrocytes stored for 14 days.Methods Blood collected from one Agroceres® swine was stored in two red blood cell units. In vivo validation was performed by labeling the red blood cells with Na251CrO4 and recovering the viable erythrocytes after 24 hours of infusion in one autologous and four homologous animals. In vitro validation was performed at baseline and after 14 days in sixteen red blood cell units by measuring hemoglobin, hematocrit, hemolysis index and free hemoglobin. A post-mortem splenectomy was performed to evaluate the splenic sequestration of erythrocytes, and the radioactivity of the supernatant samples was counted to evaluate intravascular hemolysis.Results After 14 days of storage, the red blood cell units had lower volumes and equivalent total concentrations of hemoglobin and hematocrit compared to human standards. The free hemoglobin concentration increased from 31.0±9.3 to 112.4±31.4mg/dL (p<0.001), and the hemolysis index increased from 0.1±0.1 to 0.5±0.1% (p<0.001). However, these tests were within the acceptable range for human standards. The percentage of radioactivity in supernatant samples was similar at baseline and after 24 hours, thus excluding significant hemolysis. No evidence of splenic sequestration of radioactive erythrocytes was found.Conclusion Swine red blood cells stored for 14 days are viable and can be used in experimental studies of transfusion. These validation experiments are important to aid investigators in establishing experimental models of transfusion.
ROLL and SLN biopsy are emerging procedures that can be used simultaneously. They permit exact breast lesion excision, immediate local breast surgery, and intraoperative SLN biopsy in a single procedure. The combination of radioguided nonpalpable lesion localization and SLN biopsy is a suitable alternative to working up subclinical imaging-detected breast carcinomas.
PurposeThe paper aims to clarify the characteristics of the entrepreneurial university, beyond commercial entrepreneurship to other aspects of entrepreneurship, such as social or different demands for solutions to problems in society, and integrate the entrepreneurial practices between teaching, research, extension and innovation within an entrepreneurial ecosystem. The purpose of this study was to propose an entrepreneurial university model that contemplates elements and guidelines.Design/methodology/approachThe paper opted for qualitative research of exploratory methodological approach and case study with units of analysis (management, technological innovation nucleus, academic units and ecosystem) in a Brazilian public university. For data collection, we used semi-structured interviews as primary data, and as a secondary, documentary survey, using as reference the theoretical model based on the bibliographic mapping.FindingsThe analyzed university is still in the process of transforming into an entrepreneurial university, and for that, it is necessary to devise strategies in a set. In addition, to giving new meaning to the concept of the entrepreneurial university, we reorganized the characteristics into dimensions, and we suggested guidelines.Research limitations/implicationsDue to the search approach chosen, the research results may lack generalizability. Therefore, researchers are encouraged to test the proposed model further in different contexts, as non-technological, for example.Practical implicationsThis study presents the understanding that different areas of knowledge can incorporate extension activities and contribute to the transformation of traditional universities into entrepreneurs and, starting point for other empirical studies.Social implicationsAn entrepreneurial university incorporates its social role as it meets the demands of society.Originality/valueThis paper fulfills an identified need to expand the characteristics of an entrepreneurial university within an entrepreneurial ecosystem.
The authors report their experience with a new strategy for radioguided breast surgery that combines radioguided occult lesion localisation (ROLL) and sentinel lymph node (SLN) mapping. The study population comprised 38 women with non-palpable breast lesions suspicious for breast cancer (BI-RADS 4-5). On the day before surgery, 0.2 ml solution containing particles of dextran labelled with approximately 15 MBq of technetium-99m was injected under stereotaxic guidance by mammography. All patients underwent open surgical biopsy guided by gamma probe, radiographic control of the surgical specimen and frozen section analysis. The rate of confirmation of lesion removal was 100% and the rate of simultaneous SLN mapping was 97.3% (37/38). Eleven cases of infiltrating carcinoma and eight of ductal carcinoma in situ (DCIS) were diagnosed intraoperatively. In the first eight invasive lesions, the SLN was biopsied and complete axillary lymph node dissection was performed; in the three other invasive lesions and in two aggressive cases of DCIS, only the SLN was dissected. The intraoperative results of SLN analysis and the definitive histopathological examinations of the SLN were always negative. It is concluded that ROLL and SLN can be employed simultaneously when dextran is used as a tracer; this technique allows frozen section diagnosis and intra-operative node analysis, and has many advantages over the conventional two-step procedure.
Resumo Os empreendedores sociais convertem assuntos sociais em oportunidades, criam negócios e transformam a experiência empreendedora em conhecimento empreendedor. Sendo assim, é relevante entender o processo de formação desse indivíduo como gerador de mudança da sociedade. Esse é, portanto, o objetivo geral desta pesquisa, que é qualitativa, e cujo método envolveu a análise de três narrativas da história de vida de empreendedores, considerados referências no desenvolvimento de projetos sociais de impacto no Brasil, segundo o Prêmio Empreendedor Social, realizado pelo jornal Folha de São Paulo e pela Fundação Schwab. Para a organização das análises, utilizou-se o software ATLAS. Os resultados demonstraram que a formação do empreendedor social está vinculada aos espaços e contextos de aprendizagem, à trajetória de liderança e à motivação para o empreendedorismo social, sendo essas categorias permeadas pela educação formal e não formal.
Purpose The purpose of this paper is to focus on evaluating relational capability regarding the configuration of a network of technological partners in Brazilian technology-based firms (TBFs). Design/methodology/approach The data were collected using an online questionnaire made available to technology-based companies resident in Brazilian Technological Parks. A total of 73 companies responded. The data were analyzed using bivariate and multivariate statistical techniques and were processed using Statistical Package for Social Sciences software. The statistical tests included factor analysis, Cronbach’s α and multiple regression. Findings The paper shows that the strategic alliance portfolio is influenced by organizational learning, diversity of partners, governance structure, intensity of partnership relations and configuration. In particular, the portfolio of alliances with competence orientation is characterized by tacit knowledge exchanges and learning exploration, homogeneity of partners, informal governance mechanisms, strong bonds of trust and reciprocity with partners and low diversification of actors’ profiles, their attributions and the results obtained in the portfolio. Meanwhile, the characteristics of alliance portfolios with legitimacy orientation include explicit knowledge exchange and learning exploitation, heterogeneity of partners, formal governance mechanisms, weak bonds of trust and reciprocity with partners and high diversification of the profile of the actors, their attributions and the results obtained from the portfolio. Practical implications The configuration of the alliance portfolio plays an important role in innovation. To stimulate the creation of new technological skills, the executive of a technology-based company from emerging countries such as China, Russia and India, can configure the portfolio of strategic alliances with more homogeneous partners in terms of profile and attribution. However, if this executive is challenged to seek legitimacy and complementary resources in these markets he can invest in the diversification of the strategic alliance portfolio, prioritizing partners with differentiated profiles and attributions. Originality/value The originality of the research lies in the adoption of a complementary and multidimensional theoretical prism, considering the relational capacity of TBFs in the configuration of alliances, both in the intra-firm and portfolio perspective. Furthermore, it was considered that the configuration of alliances can be based on both competence and legitimacy factors.
Purpose The purpose of this paper is to analyze the organizational structure and R&D management alignment in an internationalized public company. Design/methodology/approach The authors performed a descriptive qualitative research using the single case method of EMBRAPA. Findings The results indicated that EMBRAPA organizational structure provides a structural framework for R&D management, promoting knowledge and infrastructure sharing. The internationalization, especially through LABEX, allows researchers to interact with research centers of excellence abroad. This makes strategic planning (Sistema de Inteligência Estratégico da EMBRAPA – Agropensa, portfolios and project arrangements) results in R&D projects, through macro programs, generating innovations for Brazilian agriculture. Research limitations/implications It should be noted that this study presents some limitations, among them, the fact that only one company is being analyzed. Practical implications Therefore, EMBRAPA’s organizational structure provides a structural framework for R&D management, promoting knowledge sharing and infrastructure. This makes the strategic planning (Agropensa, portfolios and arrangements) to result in R&D projects, via macro programs, generating innovations for the Brazilian agriculture. On the contributions to the advancement of knowledge with regard to the professional management of public research institutes, it is worth synthesizing, therefore, the following practical evidence in the field and that they were key to the successful management of R&D activities in EMBRAPA. Originality/value The choice of this company is due to its importance for the Brazilian agricultural development, to the innovations generated for the modernization of agriculture and due to it being a public, internationalized R&D company, and with a trajectory of four decades of existence.
