-Background -Although is complex to identify the factors responsible for the important growth in obesity all over the world, the main causes are increased consumption of energy, highly saturated fats and sugars, and reduced physical activity. Aim -To compare rats with normal and supplemented diet with sucrose in relationship to body mass, weight of gonadal and retroperitoneal fat and Lee index. MethodsForty rats were divided into two groups: 20 in the control group that received normal chow diet and water for three months, and 20 animals in the experimental group who received the same diet but supplemented with sucrose 300 g/l of water. The animals were weighed once a week during 91 days. At scheduled death, they had measured the naso-anal length, body weight and Lee index. After laparotomy, retroperitoneal and gonadal fat were isolated, dried and the percentage of weight in relation to body weight at the date of death was evaluated. Results -There was a statistic significant difference between the 14 th and 78 th day favoring the experiment group indicating that sucrose interferes with weight gain in rats. The average weight was higher in the experimental group in all periods in comparison to initial weight. There was also significant difference in the weight of the gonadal and retroperitoneal fat. There was no significant difference comparing the Lee index. Conclusion -The body mass index was higher in animals treated with diet supplemented with sucrose and had higher gonadal and retroperitoneal fat, but no difference in the Lee index.RESUMO -Racional -Embora seja muito complexo identificar o que levou ao grande crescimento da obesidade, as causas principais são o aumento do consumo de alimentos energéticos e ricos em gorduras saturadas e açucares, e a redução das atividades físicas. Objetivos -Comparar a massa corpórea, o peso da gordura retroperitoneal e gonadal e o índice de Lee em ratos submetidos à dieta normal e à suplementada com sacarose. Métodos -A amostra foi de 40 animais, divididos em dois grupos: 20 do grupo controle que receberam dieta de ração normal e água por três meses e 20 do grupo experimento que receberam a mesma ração suplementada com sacarose 300 g/l na água. Os animais foram pesados uma vez por semana durante 91 dias. Na data da morte foram aferidos o comprimento nasoanal, peso corporal e foi calculado o índice de Lee. Após, foram submetidos à laparotomia e a gordura retroperitoneal e gonadal foram individualizadas, ressecadas e avaliada a porcentagem do peso dela em relação ao corporal na data da morte. Resultados -Verificou-se diferença estatisticamente significativa entre o 14º até o 78º dia, quando comparados os grupos indicando que a sacarose interfere no ganho de peso dos ratos. A média de peso foi maior no grupo experimento em todos os períodos, tendo como referência o peso inicial. RESUMO Introdução:A cicatrização das feridas é processo altamente complexo com várias fases. Inúmeras substâncias têm sido usadas desde os tempos remotos para estimulá-la. Entre elas, o extrato da Orbignya phalerata com possível ação estimulante da cicatrização. Objetivo: verificar a ação cicatrizante da Orbignya phalerata na cicatrização das feridas cirúrgicas da pele através de análise comparativa das alterações histológicas e morfológicas. Métodos: Foram utilizados 60 ratos, da linhagem Wistar, adultos e machos. Para o experimento, eles foram distribuídos de forma aleatória em dois grupos de 30 cada e usada a substância Orbignya phalerata intraperitoneal na dose de 50 mg/Kg, no primeiro dia da operação em um dos grupos. O procedimento experimental constituiu-se em incisão cutânea circular de dois centímetros de diâmetro com punch metálico. No grupo controle não foi usada a substância. Os ratos foram mortos nos 7°, 14°, 21° dias do pós-operatório. Realizou-se a análise macroscópica com lupa e paquímetro, para avaliação da evolução do aspecto da lesão cicatricial e morfométrica da ferida feita por análise histológica; as lâminas foram coradas com Hematoxilina-Eosina (HE) e tricrômio de Masson e observaram-se a proliferação vascular, células mononucleares, células polimorfonucleares, proliferação fibroblástica, fibras colágenas e reepitelização. Resultados: Mostraram na macroscopia dos animais do grupo experimento de sete dias que um apresentou pequena quantidade de secreção sem outras alterações dignas de nota; nos animais de 14 dias identificou-se todos com ferida de bom aspecto; nos de 21 dias, foi observada a presença de cicatrização completa em todos os animais. Quanto à análise histológica, houve diferença significativa entre os grupos nas variáveis monocelulares e fibras colágenas em todos os dias, e no 7º dia foi observada diferença significativa na proliferação fibroblástica e reepitelização. Conclusão: observou-se efeito favorável do extrato aquoso do mesocarpo do babaçu em nível microscópico do processo de cicatrização, nas variáveis mononucleares e fibras colágenas, em todos os dias e entre os grupos. Methods: Methods: patients with symptomatic prostatic hyperplasia and candidates for surgical treatment were selected. Both procedures were explained and they had choosen HoLEP or TURP. At the hospital were collected: age, date of birth, international prostate symptom score, urinary peak flow rate, prostate volume, post-voiding residual urine, globular volume and serum PSA. At the procedure operating time, morcellating time (HoLEP), bladder mucosal injury and intercurrences were collected. At the first postoperative day, globular volume and sodium. Besides that were observed the catheter indwelling time and hospital stay and after 90 days, urinary peak flow rate and international prostate symptom score. Statistical analisys have been done partially by Sinpe® and also by a professional team. Results:Results: Results: Results: Results: twenty patients in HoLEP group and 21 at TURP were operated. Baseline urinary peak flow rate was 8 ml/s in both groups and preoperative international prostate symptom score was 22 in HoLEP and 20 in TURP, very similar. Operative time was 85 minutes in HoLEP and 60 in TURP, p<0.05. Hospital stay was 47 hours for HoLEP and 48 hours to TURP, p<0.05. At 90 day the urinary peak flow rate was raised to 21.5 ml/s in HoLEP group and to 20 ml/s in TURP and the median of international prostate symptom score had been reduced to score 3 in both groups. PURPOSE: To verify the effect of highly concentrated platelet-rich plasma (hPRP) in the pathways of bone repair using non-critical defects in the calvaria of rabbits. METHODS: The hPRP was produced from collected venous blood of 21 rabbits. Four non-critical defects of 8 mm in diameter were created on the calvaria of each animal. The defects were all treated differently: autogenous particled bone (APB, group 1), autogenous particled bone associated with hPRP (APB + hPRP, group 2), isolated hPRP (group 3), and blood clot (control, group 4). Animals were submitted to euthanasia on the 2nd, 4th and 6th week postoperatively. Histological and histomorphometric analysis were carried through. RESULTS: After two weeks, groups 1 and 2 were in more advanced stage of repair than 3 and 4. At this period, comparing the groups 1 and 2, no significant differences were found between both, which also happened between the groups 3 and 4. However, after four and six weeks, the group 1 showed a more advanced stage of repair among all the other studied groups, while group 2 was in more advanced signs of bone repair than groups 3 and 4. Comparing groups 3 and 4, after four and six weeks, the least advanced stage of bone repair was found to be within group 3. CONCLUSION: The use of a highly concentrated PRP was considered prejudicial to the repair of non-critical defects in the calvaria of rabbits, either in the association of autogenous particled bone, when compared to autogenous particled bone alone, or in its isolated form, when compared to blood clot (control).
OBJETIVO: Avaliar a tendência da concentração plasmática e do clearance de procalcitonina (PCT-c) como biomarcadores de prognóstico de pacientes com sepse grave e choque séptico, comparado a um outro marcador precoce de prognóstico representado pelo número de critérios de SIRS no momento do diagnóstico da sepse. MÉTODOS: Estudo de coorte prospectivo observacional onde foram incluídos pacientes com sepse grave e choque séptico. A concentração sérica de procalcitonina foi determinada no momento do diagnóstico da sepse e após 24 e 48 horas. Foram coletados dados demográficos, escore APACHE IV, escore SOFA na chegada, número de critérios de SIRS no momento do diagnóstico, sitio da infecção e resultados microbiológicos. RESULTADOS: Vinte e oito pacientes foram incluídos, 19 clínicos e nove cirúrgicos. Em 13 (46,4%) a fonte da sepse foi pulmonar, em sete abdominal (25,0%), em cinco urinária (17,9%) e de partes moles em três casos (10,7%). Quinze pacientes tinham sepse grave e 13 choque séptico. A mortalidade global foi cinco pacientes (17,9%), três deles com choque séptico. Vinte e oito determinações de PCT foram realizadas no momento do diagnóstico da sepse, 27 após 24 horas e 26 após 48 horas. A concentração inicial não se mostrou expressivamente diferente entre os grupos sobreviventes e não sobreviventes, mas as diferenças entre os dois grupos após 24 e 48 horas alcançaram significância estatística expressiva. Não se observou diferença em relação ao número de critérios de SIRS. O clearance de procalcitonina de 24 horas mostrou-se expressivamente mais elevado no grupo de sobreviventes (-3,0 versus -300,0, p=0,028). Embora o clearance de procalcitonina de 48 horas tenha mostrado resultado mais elevado no grupo de sobreviventes comparado aos não sobreviventes, a diferença não alcançou significância estatística. CONCLUSÃO: Concentrações persistentemente elevadas de procalcitonina no plasma, assim como, redução do PCT-c 24 horas, associaram-se à elevação expressiva da mortalidade de pacientes com sepse grave e choque séptico.
OBJETIVO: Estudar se a prevalência da deficiência de vitamina D em indivíduos com úlcera de perna de causa venosa é maior do que em população controle. MÉTODOS: Estudaram-se os níveis séricos de 25-OH-vitamina D por quimioluminescência em 27 portadores de úlcera venosa crônica e 58 controles do Hospital Universitário Evangélico de Curitiba. RESULTADOS: Os níveis de 25-OH-vitamina D3 eram inferiores a 8 ng/dl em 11,1% dos pacientes com úlcera e 3,4% dos controles; entre 8 e 20 ng/dl em 46,1% dos pacientes com úlcera e 25,8% dos controles; entre 21 e 30 ng/dl em 22.2% dos pacientes com úlcera e 27,5% dos controles e acima de 30 ng/dl em 43,1% dos controles e 18,5% dos pacientes com úlcera (p=0,04). CONCLUSÃO: Existe aumento de prevalência de deficiência de vitamina D em pacientes com úlceras venosas crônicas de pernas.
ObjectiveTo describe the prevalence of carpal tunnel syndrome in a sample of 200 healthy hospital workers, establishing the respective epidemiological associations.Materials and MethodsTwo hundred individuals were submitted to wrist ultrasonography to measure the median nerve area. They were questioned and examined for epidemiological data, body mass index, carpal tunnel syndrome signs and symptoms, and submitted to the Boston carpal tunnel questionnaire (BCTQ) to evaluate the carpal tunnel syndrome severity. A median nerve area ≥ 9 mm2 was considered to be diagnostic of carpal tunnel syndrome.ResultsCarpal tunnel syndrome was diagnosed by ultrasonography in 34% of the sample. It was observed the association of carpal tunnel syndrome with age (p < 0.0001), paresthesia (p < 0.0001), Tinel’s test (p < 0.0001), Phalen’s test (p < 0.0001), BCTQ score (p < 0.0001), and years of formal education (p < 0.0001). Years of formal education was the only variable identified as an independent risk factor for carpal tunnel syndrome (95% CI = 1.03 to 1.24).ConclusionThe prevalence of carpal tunnel syndrome in a population of hospital workers was of 34%. The number of years of formal education was the only independent risk factor for carpal tunnel syndrome.
Purpose:To evaluate the influence of hypothyroidism in tracheal wound healing process. Methods: A controlled study was designed with 48 male Wistar rats, divided into two groups: study and control groups. In the first one hypothyroidism was surgically induced and 10 weeks after, a tracheal opening followed by suture was performed in both groups, subdivided into 7, 14, and 21 days in accordance with the date of animals death. A laboratorial evaluation was performed to prove the decreased in thyroid function in the study group. Also a macroscopic evaluation through a stablished protocol and a microscopic analysis with Hematoxylin-eosin and Sirius-Red staining methods were done. Results: The laboratorial evaluation certified suppressed thyroid function in the study group. The macroscopic evaluation showed the presence of suture blockade in the study group in all the evaluated days. Microscopic analysis showed a prolongated inflammatory process and less collagen with delay in organization in the study group comparing to control group. All these data were statistic significant. Conclusion: Hypothyroidism had an influence in tracheal wound healing process, promoting delay in the inflammatory and organization processes and diminished tissue collagen quantity. Métodos: Estudo controlado, com 48 ratos Wistar machos, divididos em dois grupos, denominados: grupo estudo e grupo controle. No primeiro induziu-se hipotireoidismo cirurgicamente e após 10 semanas, realizou-se nos dois grupos o procedimento de abertura traqueal seguida de sutura. Houve subdivisão destes grupos em 7, 14 e 21 dias, de acordo com a data de morte dos animais. Foi realizada avaliação laboratorial para comprovar o hipotireoidismo no grupo estudo, avaliação macroscópica e microscópica através das colorações de hematoxilina-eosina e Sirius Red. Resultados: A avaliação laboratorial atestou que o grupo estudo encontrava-se em estado de hipotireoidismo. A avaliação macroscópica demonstrou predomínio de bloqueio da sutura no grupo estudo em todos os momentos avaliados. A microscopia comprovou prolongamento da fase inflamatória e diminuição da quantidade total de colágeno bem como atraso na sua organização no grupo estudo em relação ao grupo controle. Todos estes dados apresentaram significância estatística. Conclusão: O hipotireoidismo influencia no processo de cicatrização traqueal prolongando a fase inflamatória, diminuindo a quantidade de colágeno e atrasando sua maturação e organização.
