INTRODUÇÃO: Nas regiões interioranas do Brasil o cataplasma feito com folhas de Passiflora edulis tem sido usado pela população como cicatrizante, para tratar infecções e inflamações cutâneas, sem a comprovação científica dos seus potenciais benefícios. OBJETIVO: Avaliar a cicatrização de feridas abertas em ratos com a aplicação tópica de extrato hidroalcoólico de Passiflora edulis. MÉTODOS: Foram utilizados 60 ratos da linhagem Wistar, machos, adultos, divididos em dois grupos: o grupo Passiflora e o grupo Controle. Os animais do primeiro grupo foram tratados com o extrato de Passiflora edulis, e os do segundo grupo, com água destilada. A aplicação diária do extrato e da água destilada foi realizada sobre ferida circular padronizada de 2cm de diâmetro na região dorsal de cada animal. A avaliação da ferida foi feita do ponto de vista macro e microscópico nos períodos pré-determinados (7º, 14º e 21º dias). Fez-se a análise macroscópica da evolução do aspecto da lesão e medida da retração cicatricial da ferida, por planimetria digital. O estudo histológico em lâminas coradas pela Hematoxilina-Eosina e Tricômico de Masson considerou os parâmetros de elementos celulares inflamatórios incluindo colagenização e reepitelização. RESULTADOS: Não houve diferença estatisticamente significante entre as áreas cirúrgicas das feridas tratadas com Passiflora edulis e o grupo Controle; entretanto, histologicamente houve colagenização significantemente maior no 14º dia de pós-operatório nos animais do grupo Passiflora (p = 0,012). CONCLUSÕES: O uso tópico do extrato de Passiflora edulis não apresenta macroscopicamente efeito significativo na cicatrização de feridas na pele de ratos; entretanto microscopicamente apresenta aumento da proliferação fibroblástica no 7º dia e colagenização maior no 14º dia de pós-operatório.
Background: Incidental gallbladder cancer is defined as a cancer discovered by histological examination after cholecystectomy. It is a potentially curable disease. However, some questions related to their management remain controversial and a defined strategy is associated with better prognosis. Aim: To develop the first evidence-based consensus for management of patients with incidental gallbladder cancer in Brazil. Methods: Sixteen questions were selected, and 36 Brazilian and International members were included to the answer them. The statements were based on current evident literature. The final report was sent to the members of the panel for agreement assessment. Results: Intraoperative evaluation of the specimen, use of retrieval bags and routine histopathology is recommended. Complete preoperative evaluation is necessary and the reoperation should be performed once final staging is available. Evaluation of the cystic duct margin and routine 16b1 lymph node biopsy is recommended. Chemotherapy should be considered and chemoradiation therapy if microscopically positive surgical margins. Port site should be resected exceptionally. Staging laparoscopy before reoperation is recommended, but minimally invasive radical approach only in specialized minimally invasive hepatopancreatobiliary centers. The extent of liver resection is acceptable if R0 resection is achieved. Standard lymph node dissection is required for T2 tumors and above, but common bile duct resection is not recommended routinely. Conclusions: It was possible to prepare safe recommendations as guidance for incidental gallbladder carcinoma, addressing the most frequent topics of everyday work of digestive and general surgeons.
Objective: To test the degree of agreement in selecting therapeutic options for patients suffering from colorectal liver metastasis (CRLM) among surgical experts around the globe. Summary/Background: Only few areas in medicine have seen so many novel therapeutic options over the past decades as for liver tumors. Significant variations may therefore exist regarding the choices of treatment, even among experts, which may confuse both the medical community and patients. Methods: Ten cases of CRLM with different levels of complexity were presented to 43 expert liver surgeons from 23 countries and 4 continents. Experts were defined as experienced surgeons with academic contributions to the field of liver tumors. Experts provided information on their medical education and current practice in liver surgery and transplantation. Using an online platform, they chose their strategy in treating each case from defined multiple choices with added comments. Inter-rater agreement among experts and cases was calculated using free-marginal multirater kappa methodology. A similar, but adjusted survey was presented to 60 general surgeons from Asia, Europe, and North America to test their attitude in treating or referring complex patients to expert centers. Results: Thirty-eight (88%) experts completed the evaluation. Most of them are in leading positions (92%) with a median clinical experience of 25 years. Agreement on therapeutic strategies among them was none to minimal in more than half of the cases with kappa varying from 0.00 to 0.39. Many general surgeons may not refer the complex cases to expert centers, including in Europe, where they also engage in complex liver surgeries. Conclusions: Considerable inconsistencies of decision-making exist among expert surgeons when choosing a therapeutic strategy for CRLM. This might confuse both patients and referring physicians and indicate that an international high-level consensus statements and widely accepted guidelines are needed.
In the chronic care model, a missed appointment decreases continuity, adversely affects practice efficiency, and can harm quality of care. The aim of this study was to identify predictors of a missed appointment and develop a model to predict an individual's likelihood of missing an appointment. The research team performed a retrospective study in an urban, academic, underserved outpatient internal medicine clinic from January 2008 to June 2011. A missed appointment was defined as either a "no-show" or cancellation within 24 hours of the appointment time. Both patient and visit variables were considered. The patient population was randomly divided into derivation and validation sets (70/30). A logistic model from the derivation set was applied in the validation set. During the period of study, 11,546 patients generated 163,554 encounters; 45% of appointments in the derivation sample were missed. In the logistic model, percent previously missed appointments, wait time from booking to appointment, season, day of the week, provider type, and patient age, sex, and language proficiency were all associated with a missed appointment. The strongest predictors were percentage of previously missed appointments and wait time. Older age and non-English proficiency both decreased the likelihood of missing an appointment. In the validation set, the model had a c-statistic of 0.71, and showed no gross lack of fit (P=0.63), indicating acceptable calibration. A simple risk factor model can assist in predicting the likelihood that an individual patient will miss an appointment.
Background: Hepatopancreatoduodenectomy is one of the most complex abdominal operations mainly indicated in advanced biliary carcinoma. Aim: To present 10-year experience performing this operation in advanced malignant tumors. Methods: This is a retrospective descriptive study. From 2004 to 2014, 35 hepatopancreatoduodenectomies were performed in three different institutions. The most common indication was advanced biliary carcinoma in 24 patients (68.5%). Results: Eighteen patients had gallbladder cancer, eight Klatskin tumors, five neuroendocrine tumors with liver metastasis, one colorectal metastasis invading the pancreatic head, one intraductal papillary mucinous neoplasm with liver metastasis, one gastric cancer recurrence with liver involvement and one ocular melanoma with pancreatic head and right liver lobe metastasis. All patients were submitted to pancreatoduodenectomy with a liver resection as follows: eight right trisectionectomies, five right lobectomies, four left lobectomies, 18 central lobectomies (IVb, V and VIII). The overall mortality was 34.2% (12/35) and the overall morbidity rate was 97.4%. Conclusion: Very high mortality is seen when major liver resection is performed with pancreatoduodenectomy, including right lobectomy and trisectionectomy. Liver failure in combination with a pancreatic leak is invariably lethal. Efforts to ensure a remnant liver over 40-50% of the total liver volume are the key to obtain patient survival.
-Background -Postoperative liver failure consequent to insufficiency of remnant liver is a feared complication in patients who underwent extensive liver resections. To induce rapid and significant hepatic hypertrophy, associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) has been recently developed for patients which tumor is previously considered unresectable. Aim -To present the Brazilian experience with ALPPS approach. Method -Were analyzed 39 patients who underwent hepatic resection using ALPPS in nine hospitals. The procedure was performed in two steps. The first operation was portal vein ligation and in situ splitting. In the second operation the right hepatic artery, right bile duct and the right hepatic vein were isolated and ligated. The extended right lobe was removed. There were 22 male (56.4%) and 17 female (43.6%). At the time of the first operation, the median age was 57.3 years (range: 20-83 years). Results -The most common indication was liver metastasis in 32 patients (82.0%), followed by cholangiocarcinoma in three (7.7%). Two patients died (5.2%) during this period and did not undergo the second operation. The mean interval between the first and the second operation was 14.1 days (range: 5-30 days). The volume of the left lateral segment of the liver increased 83% (range 47-211.9%). Significant morbidity after ALPPS was seen in 23 patients (59.0%). The mortality rate was 12.8% (five patients). Conclusion -The ALPPS approach can enable resection in patients with lesions previously considered unresectable. It induces rapid liver hypertrophy avoiding liver failure in most patients. However still has high morbidity and mortality.RESUMO -Racional -Insuficiência hepática pós-operatória devido à remanescente hepático pequeno tem sido complicação temida em pacientes que são submetidos à ressecção hepática extensa. A ligadura da veia porta associada à bipartição do fígado para hepatectomia em dois estágios (ALPPS) foi desenvolvida recentemente com a finalidade de induzir rápida e significante regeneração do fígado para pacientes em que o tumor é previamente considerado irressecável. Objetivo -Apresentar a experiência brasileira com o ALPPS. Método -Foram analisados 39 pacientes submetidos ao procedimento ALPPS em nove hospitais. Ele foi realizado em duas etapas. A primeira operação consistiu em ligadura do ramo direito da veia porta e bipartição hepática. Na segunda, os ramos direito da artéria hepática, via biliar e veia hepática foram ligados e o lobo hepático direito estendido foi removido.
INTRODUÇÃOA doença calculosa da vesícula biliar continua sendo um problema de saúde de proporções variadas em todo o mundo. Sua prevalência varia de acordo com as populações estudadas, tanto retrospectivamente em necropsias, quanto prospectivamente através do exame ultra-sonográfico. Estima-se que 25 milhões de pessoas nos Estados Unidos sejam portadoras de litíase biliar, onde são diagnosticados um milhão de casos novos por ano, o que resulta em 600 mil operações nesse período. Por outro lado, a prevalência de colelitíase em países africanos tende a ser baixa, normalmente inferior a 5%. No Brasil, a prevalência em estudos de necropsia em indivíduos com idade acima de 20 anos variou de 9,1% a 19,4%. Em estudo através de exame ultra-sonográfico a prevalência foi de 9,3% [1][2][3][4] . São considerados assintomáticos os casos que não estão associados à presença ou história de dor (cólica biliar) ou colecistite aguda, independente da presença de sintomas inespecíficos, como flatulência, intolerância a alimentos gordurosos e dispepsia. As conseqüências mais comuns da presença de cálculos na vesícula biliar são a colecistite, colangite, pancreatite e câncer da vesícula biliar [5][6][7] . A evolução clínica dos pacientes com cálculo da vesícula biliar permanece controversa. Alguns estudos mostram uma probabilidade acumulada da ocorrência de sintomas biliares em pacientes com colelitíase assintomáticos de 10% em 5 anos e de 18% em 15 a 20 anos 8,9 . Uma vez que as colecistectomias realizadas tardiamente são grandes responsáveis pelo elevado número de complicações, agravando a morbidade e aumentando a mortalidade, o conhecimento da freqüência da litíase biliar cresce em importância. Portanto, é necessário identificar as características da população com doença biliar para que o diagnósti-co seja feito sem retardo e com menor morbidade [8][9][10]
