BackgroundThere has been a dramatic increase in T cell receptor (TCR) sequencing spurred, in part, by the widespread adoption of this technology across academic medical centers and by the rapid commercialization of TCR sequencing. While the raw TCR sequencing data has increased, there has been little in the way of approaches to parse the data in a biologically meaningful fashion. The ability to parse this new type of 'big data' quickly and efficiently to understand the T cell repertoire in a structurally relevant manner has the potential to open the way to new discoveries about how the immune system is able to respond to insults such as cancer and infectious diseases.
In view of the fact that insufficiency in immune response often correlates with poor prognosis, research in recent years has focused on the task of describing the precise status and function of the immune system and its possible effect on cancer patients. Although more than two thirds of treated patients respond to endocrine therapy, most patients with metastatic breast cancer develop a resistance to it. Estrogen modulates angiogenesis, partially through its effects on vascular endothelial growth factor (VEGF). It also appears that transforming growth factor-beta (TGF beta) could be another factor contributing to this resistance. TGF beta is a highly immunosuppressive factor that inhibits natural and specific immunity against tumors and stimulates the production of VEGF. The purpose of the study was to monitor immune responses in patients with hormone receptor-positive breast cancer who were resistant to hormone therapy. The examination of cellular components (CD4, CD8, HLA-DR, NK cells) and humoral immunity (IgG, IgG subclasses, IgA, IgM,). TGF beta and VEGF production were monitored with special attention, along with an analysis of the changes that occurred during the hormonal treatment. 68 patients included in the research project were implemented with routine cancer treatment with endocrine therapy. Basic parameters (the histological type and grade, the degree of expression of estrogen receptors (ER) and progesterone receptors (PR), human epidermal growth factor receptor 2 (HER2), and the proliferative marker) were established. Patients were evaluated by a cancer clinical immunologist to exclude immune disorders, allergic or autoimmune origin. TGF beta and VEGF were measured by ELISA and antitumor cellular immunity (CD4, CD8) was measured by flow cytometry. Patients who failed in the first line of hormone therapy treatment were considered as resistant to hormone therapy.Depression in cellular immunity was found especially in patients with resistance to endocrine therapy. In addition, immunoglobulin plasma levels were decreased (mainly IgG4 subtype). Most patients showed clinical symptoms of immunodeficiency (frequent infections of respiratory or urinary tract, herpetic infections). Significant increases in TGF beta and VEGF plasma were also detected.The correlation of these factors with resistance to hormonal therapy and the state of anticancer immunity could be helpful in the task of predicting resistance to hormonal therapy and could contribute to the selection of targeted immune therapy in cancer patients in the future.

Východiska: Dědičné mutace v genu CHEK2 kódujícím CHK2 proteinkinázu způsobují středně zvýšené riziko vzniku karcinomu prsu (breast cancer-BC) a dalších nádorových onemocnění. Vysoká populační variabilita CHEK2 mutací a výskyt vzácných missense variant nejasného významu (variants of unknown clinical significance-VUS) komplikuje odhad rizika vzniku nádorových onemocnění u nosičů germinálních variant. Soubor pacientů a metody: Mutační analýzu CHEK2, vč. analýzy velkých přestaveb, jsme provedli u 1 526 vysoce rizikových pacientek s BC a 3 360 kontrol z ČR. Nalezené VUS jsme klasifikovali pomocí funkční analýzy v modelovém systému lidské buněčné linie RPE1-CHEK2-KO, ve které byly obě endogenní alely inaktivovány metodou CRISPR/ Cas9. Výsledky: Četnost 10 různých trunkačních mutací CHEK2 byla významně vyšší u pacientek s BC (2,62 %) než u kontrol (0,11 %; p = 4,1 × 10 −12), 23 různých missense variant jsme nalezli u 4,5 % pacientek a 4,0 % kontrol. Nejčastější alteraci představovala p.I157T se srovnatelnou četností u pacientek a kontrol (3,08 vs. 3,10 %). Funkční analýza identifikovala u 9 VUS zásadní poruchu kinázové aktivity, zatímco u dalších 9 zachovanou kinázovou aktivitu. Zbývající VUS a p.I157T byly částečně funkční. Riziko BC zvyšovaly trunkační mutace (OR 8,19; 95% CI 4,11-17,75) a nefunkční missense mutace (OR 4,06; 95% CI 1,37-13,39). Částečně funkční (vč. p.I157T) a plně funkční missense varianty riziko neovlivňovaly. Pacientky s trunkačními a funkčně-defektními missense variantami CHEK2 vyvinuly BC (převážně ER-pozitivní s vyšším gradingem) v průměrném věku 44,4 a 50,7 roku a signifikantně častěji vyvinuly sekundární tumory než nosičky mutací v BRCA1/ BRCA2/ PALB2/ p53 a nenosičky. Závěr: Dědičné mutace v genu CHEK2 představují významnou komponentu dědičného BC v ČR. Riziko vzniku onemocnění u nosičů patogenních mutací CHEK2 se zvyšuje s počtem příbuzných s BC a dalšími nádory v rodině. U asymptomatických nosičů je indikována dispenzarizace (jednou ročně ultrazvuk, mamografie nebo magnetická rezonance) od 40 let věku a chirurgická prevence v závislosti na rodinné anamnéze. Prevence vzniku dalších nádorů je ke zvážení dle výskytu nádorových onemocnění v rodině. Východiska: Karcinom ovaria, závažné nádorové onemocnění s vysokou mortalitou, je v České republice dia gnostikováno každým rokem přibližně u 1 000 žen. Riziko vzniku onemocnění je zvýšeno u nosiček mutací v ně kte rých nádorových predispozičních genech. S vysokým relativ ním rizikem (RR > 5) jsou spojeny mutace v genech BRCA1, BRCA2, BRIP1, geny Lynchova syn dromu, RAD51C, RAD51D, STK11; s možným zvýšením rizika mutace v genech ATM, CHEK2, NBN, PALB2, BARD1. Cílem práce bylo určit frekvenci mutací v nádorových predispozičních genech v naší populaci. Metody a výsledky: Celkem 1 057 pacientek s karcinomem ovaria a 617 nená dorových kontrol bylo vyšetřeno pomocí panelového sekvenování nové generace na platformě Illumina. Patogenní mutace ve vysoko rizikových genech, vč. velkých genomových přesta veb, byly v našem souboru zachyceny u 30,6 % pacientek; u neselektovaných pacientek byla frekvence mutací téměř 25 %, u pacientek s negativní rodinnou anamnézou 18 %. Nejčastěji mutovanými predispozičními geny byly BRCA1 a BRCA2, součet frekvence mutací v ostatních ovariálních predispozičních genech odpovídal frekvenci mutací v genu BRCA2. Záchyt mutací u pacientek starších 70 let byl více než třikrát vyšší v porovnání s pacientkami ve věku pod 30 let. Závěr: Karcinom ovaria je heterogenní onemocnění s vysokým podílem dědičné formy onemocnění. Vzhledem k nedostatku adekvátních screeningových modalit pro včasnou dia gnostiku onemocnění je identifikace nosiček mutací v ovariálních predispozičních genech klí čová, s vysokým potenciálem k celkovému snížení mortality z důvodu karcinomu ovaria. Východiska: Karcinom slinivky břišní představuje čtvrtou nejčastější příčinu úmrtí na nádorová onemocnění celosvětově. Při porovnání s ostatními solidními nádory jsou léčebné výsledky ve všech stadiích onemocnění nejhorší. Soubor pa cientů a metody: Do hodnocení bylo zařazeno 19 pa cientů léčených ve 2. linii paliativní terapie kombinací gemcitabin + nab-paklitaxel pro lokálně pokročilý nebo metastazující karcinom slinivky břišní v období mezi říjnem 2014 a prosincem 2016 na Onkologické klinice 1. LF UK a VFN v Praze. Všichni nemocní absolvovali kombinovanou chemoterapii gemcitabin (1 000 mg/ m 2 ) + nab-paklitaxel (125 mg/ m 2 ) týdně s týdenní pauzou po třech aplikacích. Byl hodnocen protinádorový účinek (počet objektivních léčebných odpovědí (dis ease control rate -DCR), doba do progrese onemocnění (time to progression -TTP) a celkové přežití (overal survival -OS)) a výskyt nežádoucích účinků. Výsledky: Kontroly onemocnění bylo dosaženo dle RECIST kritérií u 9 pa cientů (56,3 %, u 2 pa cientů došlo k parciální regresi). Bylo dosaženo mediánu TTP 5,5 měsíce a medián OS dosáhl 10,1 měsíce. Závěr: U pa cientů ve velmi dobrém výkonnostním stavu (0-1) s pokročilým karcinomem pankreatu může užití režimu gemcitabin + nab-paklitaxel ve 2. linii paliativní terapie vést k prodloužení TTP a OS při zachování dobré kvality života nemocných. The outcomes at all stages of the dis ease are the worst among patients with solid tumors. Patients and Methods: Analyses were conducted on 19 patients treated with gamcitabine + nab-paclitaxel for locally advanced or metastatic pancreatic cancer as a second line treatment between October, 2014, and December, 2016, at Department of Oncology of First Faculty of Medicine and General University Hospital in Prague. Patients were treated with gemcitabine (1,000 mg/ sqm) + nab-paclitaxel (125 mg/sqm) at days 1, 8 and 15 of each 28-day cycle. Antitumor efficacy (dis ease control rate (DCR), time to progression (TTP), and over all survival (OS)) and adverse events were monitored. Results: Dis ease control accord ing to RECIST criteria was achieved in nine cases (56.3%, two partial regressions were observed). The median TTP was 5.5 months and median OS was 10.1 months. Conclusion: In patients with advanced or metastatic pancreatic cancer with good performance statuses (0-1) gemcitabine + nab-paclitaxel as a second line treatment led to a prolongation in time to progression and higher over all survival with good quality of life.
