Os dados foram coletados junto a oito enfermeiros de uma unidade de oncologia pediátrica de um hospital público de São Paulo, por meio de entrevistas semiestruturadas. A análise dos dados permi u a iden fi cação de cinco categorias: sen r-se sem autonomia para a tomada de decisão; cuidar da família; oferecer conforto sico; valorizar o cuidado humanizado e aprender a lidar com a morte e o morrer. Este estudo contribui para ampliar a compreensão do processo de cuidar e permite avançar na postulação de um quadro teóri-co que contemple a integração de saberes e ações que cons tuem uma assistência integral, transcendendo o atendimento de necessidades apenas clínicas e biológicas. We used Symbolic Interac on Theory as the theore cal framework and narra ve research methods. The data were collected from eight nurses in the pediatric oncology unit of a public hospital in Sao Paulo through semi-structured interviews. The data analysis revealed fi ve categories: feeling no autonomy in decision-making, caring for the family, off ering physical comfort, valuing humanized care and learning to deal with death and dying. This study helps to extend the understanding of this process of care and postulates a theore cal framework that integrates the knowledge and ac ons that cons tute care that transcends clinical and biological needs. Esse estudo teve como objetivos desvelar os elementos do cuidado humanizado presentes no encontro entre enfermeiro, família e criança com câncer, identificar a percepção desses enfermeiros quanto à humanização da assistência e verificar em que situações o enfermeiro percebe que a humanização está ancorada ao cuidado. Trata-se de estudo exploratório-descritivo, com abordagem qualitativa. Utilizou-se como referencial teórico a Teoria do Cuidado Humano de Watson. Os dados foram coletados por meio de entrevista semiestruturada com nove enfermeiros de um hospital público especializado em oncologia pediátrica em São Paulo. Os resultados apontaram para o fenômeno "Desvelando o cuidado humanizado dispensado à família e à criança com câncer". Os elementos teóricos emergiram das descrições de eventos clínicos ou situações apresentadas pelos enfermeiros. Os dados permitiram reflexões sobre possibilidades de construção do processo humanístico interpessoal no ambiente de cuidado na oncologia pediátrica e de avanços e limitações quanto à aplicabilidade deste referencial na prática.
OBJECTIVES: to understand the family's experience of the child and/or teenager in palliative care and building a representative theoretical model of the process experienced by the family. METHODOLOGY: for this purpose the Symbolic Interactionism and the Theory Based on Data were used. Fifteen families with kids and/or teenagers in palliative care were interviewed, and data were collected through semi-structured interviews. RESULTS: after the comparative analysis of the data, a substantive theory was formed "fluctuating between hope and hopelessness in a world changed by losses", composed by: "having a life shattered ", "managing the new condition", "recognizing the palliative care" and "relearning how to live". Hope, perseverance and spiritual beliefs are determining factors for the family to continue fighting for the life of their child in a context of uncertainty, anguish and suffering, due to the medical condition of the child. Along the way, the family redefines values and integrates palliative care in their lives. CONCLUSION: staying with the child at home is what was set and kept hope of dreaming about the recovery and support of the child's life, but above all, what takes it away even though temporarily is the possibility of their child's death when staying within the context of the family.
Conclusion:Professional maturity, the ability to transmit information and the ability to negotiate are directly related to the inclusion of nurses in the decision-making process.
Conhecendo as práticas de cuidado da equipe de enfermagem em relação ao cuidado na situação de final de vida de recém-nascidosConociendo las prácticas de atención de enfermería ante final de la vida en recién nacidos Através da análise temática dos dados, foi possível encontrar três temas centrais: A "obscuridade da morte na Neo": lidando com morte no início da vida; Os cuidados paliativos e as decisões no final de vida: os desafios da equipe de enfermagem na UTI Neonatal; e As formas de cuidado da equipe de enfermagem no cotidiano da UTI Neonatal. Conclusão: Torna-se essencial compreender as vivências e as necessidades da equipe para que propostas sejam elaboradas, buscando melhorias nas relações de cuidado que ocorrem nesse contexto.Palavras-chave: recém-nascido; cuidados paliativos; neonatologia; enfermagem. resumenObjetivo: Conocer las experiencias de prácticas de atención del equipo de enfermería respecto del cuidado de recién nacidos y sus familias ante la circunstancia de fin de la vida, experimentadas en la UTI Neonatal. Método: Estudio de abordaje cualitativodescriptivo, con participación de ocho profesionales del equipo de enfermería actuante en UTI Neonatal. Resultados: A partir del análisis temático de los datos fue posible encontrar tres temas centrales: La "oscuridad de la muerte en la Neo": afrontando muerte al inicio de la vida; Los cuidados paliativos y las decisiones en el final de la vida: desafíos del equipo de enfermería en la UTI Neonatal; y Las formas de atención del equipo de enfermería en el día a día de la UTI Neonatal. Conclusión: Resulta esencial comprender las experiencias y necesidades del equipo para elaborar propuestas en busca de mejoras en las relaciones de cuidado que ocurren en este contexto.
RESUMEN Introducción:El cuidado al final de la vida deber estar marcado por la revaloración de la dimensión espiritual. Objetivo: Comprender el papel de la espiritualidad en la vida de las familias que tienen un ser llegando al final de su vida. Materiales y Métodos: Investigación cualitativa, realizada con quince familiares de pacientes con enfermedades graves atendidos en la red de atención primaria de Sao Paulo, Brasil, a través de entrevistas semiestructuradas. Los datos fueron organizados y analizados de acuerdo a las figuras metodológicas de la técnica del Discurso del Sujeto Colectivo. Resultados: La espiritualidad tiene un papel único para las familias, ya que ayuda a dar sentido a las experiencias de vida, especialmente aquellos involucrados directamente con la enfermedad y la muerte. Sin embargo, no encontraron espacio para la expresión de estos temas con los profesionales de la salud, con quien deberían tener un vínculo y corresponsabilidad para el cuidado guiado por la integralidad. Discusión y Conclusiones: Estas familias reconocen la espiritualidad mientras como elemento fortalecedor sobre las dificultades que enfrentan con la enfermedad, pero dependen en gran medida de la disponibilidad de personal para aumentar su experiencia. Objective: This work sought to understand the role of spirituality in the lives of families with a loved one in end-oflife situation. Materials and Methods: This qualitative research was conducted through semi-structured interviews with 15 relatives of patients with serious diseases cared for in the primary health care network in São Paulo, Brazil. The data were organized and analyzed according to the methodological figures of the Collective Subject Discourse technique. Results: Spirituality represents a unique role for the families, as it helps to give meaning to life experiences, above all those directly involved with disease and death. However, they find no space to express those issues with the health professionals, with whom they should have a bond and co-responsibility for care based on integrality. Discussion and Conclusions: These families recognize spirituality as a strengthening element against difficulties confronted with the illness, but strongly depend on the health staff's availability to increase their experience.
Purposes To explain parental expectations of support from healthcare providers for their parenting roles and goals during a child’s life-threatening illness (LTI). Design and Methods Qualitative interpretive study guided by the Family Adjustment and Adaptation Response Model. Thematic analyses were conducted with data from 31 semi-structured interviews of parents of children with LTI using systematic strategies to ensure rigor including audit trails and prolonged engagement. Results We identified three themes and one meta-theme or overall theme: (1) “Help us survive this,” (2) “Let’s fight together: please fight with me, not against me, to care for my family,” and (3) “Guide me through the darkness: I am suffering.” Overall, the parents conveyed that they expect mutuality with the health care providers and system in order to keep Fighting together for my family survival. Conclusions In the daily work of caring for their families, parents of children with LTI consider survival on multiple levels. They consider the life, illness, and potential death of one child while considering the on-going survival and sustenance of family relationships. Practice Implications: Parents are distressed and grapple with conflicted feelings about managing competing needs of various family members. Relationships with health care providers can influence parents’ management of the situation and be a source of support as their parenting role changes over the illness trajectory, time, and in response to adversity.
