OBJECTIVE To analyze oral health work changes in primary health care after Brazil’s National Oral Health Policy Guidelines were released.METHODS A literature review was conducted on Medline, LILACS, Embase, SciELO, Biblioteca Virtual em Saúde, and The Cochrane Library databases, from 2000 to 2013, on elements to analyze work changes. The descriptors used included: primary health care, family health care, work, health care policy, oral health care services, dentistry, oral health, and Brazil. Thirty-two studies were selected and analyzed, with a predominance of qualitative studies from the Northeast region with workers, especially dentists, focusing on completeness and quality of care.RESULTS Observed advances focused on educational and permanent education actions; on welcoming, bonding, and accountability. The main challenges were related to completeness; extension and improvement of care; integrated teamwork; working conditions; planning, monitoring, and evaluation of actions; stimulating people’s participation and social control; and intersectorial actions.CONCLUSIONS Despite the new regulatory environment, there are very few changes in oral health work. Professionals tend to reproduce the dominant biomedical model. Continuing efforts will be required in work management, training, and permanent education fields. Among the possibilities are the increased engagement of managers and professionals in a process to understand work dynamics and training in the perspective of building significant changes for local realities.

No cenário mundial, o Cirurgião-Dentista atua, sobretudo, no setor privado. No Brasil, esta realidade começa a mudar com a inclusão desse profissional na Estratégia Saúde da Família (ESF). Este estudo de caso realizado no Centro-Oeste do Brasil busca compreender o trabalho do dentista nesse contexto, através de análise documental, observação do trabalho e entrevistas. Da análise temática de conteúdo emergiram duas categorias: condições para o trabalho na ESF e modo de trabalho na ESF. A influência histórica do modelo de atenção individual, o contexto adverso do trabalho, as concepções de ESF dos profissionais e a autonomia que lhes é conferida restringem sua atuação.

The implementation of interdisciplinarity in the work routine of the family health care team

Based on ergology and work process theorization, the study aims to contribute to reflections on health collective work, emphasizing its specificity and difficulties in building and managing groups of workers. It deals with work as a human activity that dialectically comprises the application of a prescribed protocol and a unique and historical perspective. Health work involves a relationship among individuals who act in the drama of using themselves and manage their own work; it is influenced by the history of health professions and macro-political determinations. In conclusion, this health work complexity needs to be considered in the process of management of professional teams/groups of workers, in a way that actions can interact and enable the implementation of a new health care project in the perspective of comprehensiveness.

O ensaio propõe uma reflexão sobre as possibilidades da Estratégia de Saúde da Família significar uma ruptura com o modelo clínico/biológico/flexneriano historicamente hegemônico na conformação das políticas de saúde do Brasil. Aborda a mudança do modelo de atenção à saúde implicada na consolidação do SUS, com foco principal em sua dimensão político-operacional, a partir de categorias propostas por Thomas Kuhn (1922-1996) acerca do desenvolvimento da ciência. Elege o modelo flexneriano e o SUS como expressões paradigmáticas exemplares para a análise do processo de transição e de crise que caracteriza o atual momento de reformulação do pensamento e das práticas em saúde. Finaliza considerando que a mudança paradigmática na saúde não se limita à Estratégia Saúde da Família, mas engloba todo o Sistema de Saúde. A explicação da realidade social do setor saúde vai além da possibilidade de aplicação das categorias estudadas.

Resumo O objetivo é analisar a inserção do trabalho do farmacêutico na atenção primária no Brasil. Foram realizadas buscas nas bases de dados BVS, SciELO, Lilacs e Medline, no período de 1998 a 2016. Dos 157 artigos encontrados, excluindo-se os duplicados, teses, dissertações e revisões e após leitura integral, foram incluídos 9 que tratavam do trabalho do farmacêutico e que relatavam experiências, atribuições, potencialidades, dificuldades e desafios para a prática profissional. Os resultados apontam produção incipiente e predomínio de estudos qualitativos a partir de 2007. A inserção na equipe, como espaço de qualificação das ações, é tema central dos estudos, que apontam desafios e dificuldades quanto ao reconhecimento e aceitação das intervenções do farmacêutico. As potencialidades estão no âmbito das ações voltadas para o usuário, as famílias e a equipe, assim como na formação profissional e na difusão dos resultados das ações farmacêuticas. Há predomínio do isolamento do farmacêutico na atenção primária, com perspectivas de fortalecimento da sua integração à equipe, que tem sido impulsionada pelas recentes mudanças institucionais e normativas no cenário nacional. Os pesquisadores focam nas potencialidades para a prática profissional, com o olhar para o futuro em construção.

Objectiveto identify the workloads of nursing professionals of the Family Health Strategy, considering its implications for the effectiveness of universal access. Methodqualitative study with nursing professionals of the Family Health Strategy of the South, Central West and North regions of Brazil, using methodological triangulation. For the analysis, resources of the Atlas.ti software and Thematic Content Analysis were associated; and the data were interpreted based on the labor process and workloads as theorical approaches. Resultsthe way of working in the Family Health Strategy has predominantly resulted in an increase in the workloads of the nursing professionals, with emphasis on the work overload, excess of demand, problems in the physical infrastructure of the units and failures in the care network, which hinders its effectiveness as a preferred strategy to achieve universal access to health. On the other hand, teamwork, affinity for the work performed, bond with the user, and effectiveness of the assistance contributed to reduce their workloads. Conclusionsinvestments on elements that reduce the nursing workloads, such as changes in working conditions and management, can contribute to the effectiveness of the Family Health Strategy and achieving the goal of universal access to health.
