A comunicação bucosinusal ocorre após a exodontia de molares superiores por sua proximidade anatômica ao seio maxilar, caracterizando pelo acesso entre a cavidade oral e o seio maxilar devido ao defeito em tecido ósseo e tecido mole. Objetivou-se apresentar um relato de caso de fechamento de comunicação buco sinusal com bola de bichat. Paciente masculino, 54 anos, compareceu a Clínica Solon Galvão (Universidade Potiguar – UnP, Natal-RN, Brasil) apresentando uma comunicação buco sinusal em lado direito da maxila decorrente de uma exodontia do elemento 17. Foi solicitada uma tomografia computadorizada cone bean de maxila, onde foi comprovada que durante a exodontia do elemento 17, foi retirado também a parede anterior do seio maxilar, causando a comunicação. Foi feito a limpeza do seio maxilar, retirando cistos inflamatórios de dentro da cavidade. Após, houve exposição e mobilização da bola de bichat, seguido de sutura da mesma ao redor da lesão em mucosa e sutura da mucosa por cima da bola de bichat, utilizando fio absorvível. Conclui-se que é uma técnica segura, de fácil execução, beneficiando um resultado satisfatório para o paciente.
A osteonecrose dos maxilares relacionada ao uso de medicamentos (OMRM) é uma situação clínica, proveniente de efeitos secundários da terapêutica com alguns fármacos, como os bifosfonatos, os inibidores do RANK-L e alguns antiangiogênicos. O objetivo do presente trabalho foi realizar uma revisão sistemática sobre OMRM. As pesquisas realizadas permitiram a seleção de 9 artigos, a partir de 759 artigos encontrados. Os resultados do presente estudo mostram que a doença subjacente mais comum foi o câncer de mama, a via de administração mais prevalente foi a intravenosa, extração dentária foi o principal procedimento odontológico desencadeador e a região mais acometida pela alteração foi a mandíbula. O atendimento pelo cirurgião-dentista é fundamental antes do início da terapia com os medicamentos estudados, principalmente, com o bifosfonatos.
O edentulismo é um desafio extremamente frequente no dia-a-dia do cirurgião dentista. A busca de soluções funcionais e estéticas para estes casos têm proporcionado diversas inovações no mercado de materiais odontológicos como, por exemplo, os implantes dentais osseointegráveis, próteses, coroas e guias cirúrgicas. O aprimoramento de materiais e métodos em Implantodontia trouxeram possibilidades de intervenção, que por meio do planejamento digital e confecção de guias cirúrgicas 3D, concedem menor tempo de trabalho para assim, ofertar ao paciente maior conforto através de técnicas menos invasivas e cruentas, com a finalidade de reestabelecer a função, estética e condição biopsicossocial do paciente. Este caso clínico visa descrever o planejamento e instalação de implante dentário cone morse imediato (Drive, Neodent®, Curitiba, Brasil) utilizando a técnica com guia cirúrgica 3D (3D Studio – Planejamento Virtual da Face®, Natal- RN, Brasil), planejada através de tomografia computadorizada cone beam analisada através do software Blue Sky Bio LLC (Libertyville, Illinois), além da realização de enxertia com estrutura óssea mineral bovina (Lumina-Bone Fino, Criteria®, SP- São Paulo, Brasil) seguido da confecção de coroa unitária e instalação de carga imediata em área estética.
O uso de prótese obturadora objetiva a correção das sequelas deixada pelo procedimento cirúrgico, tem como função separar a comunicação entre a cavidade oral e as áreas superiores, protegendo o trauma ou defeito cirúrgico, contribuindo para a restauração de funções importantes para a qualidade de vida do paciente como a mastigação, deglutição e audição, além dos benefícios estéticos. Foram tratados 12 pacientes, provenientes do Hospital Dr. Luiz Antônio, que passaram por uma anamnese, exame físico extra e intra oral, responderam a questionários específicos Portuguese Brazilian, versão 3.0 (EORTC QLQC30) e Portuguese Brazilian (EORTC QLQ-H&N35) pré e pós 15 dias da instalação protética, comparando o antes e depois através dos testes de Wilcoxon (α=0,05). Houve uma melhora significativa na qualidade de vida dos pacientes (p= 0.009), nos problemas relacionados à fala (p=0.0131), a deglutição (p=0.0029) e o convívio social (p=0.0038). Ocorreu aumento significativo na qualidade de vida dos pacientes, redução nos problemas relacionados à deglutição, em comer socialmente e aos problemas relacionados com a fala e a maioria dos pacientes evoluíram de forma satisfatória, tendo na fala sua remissão da hipernasalidade proporcionando uma forma inteligível.
