ZusammenfassungMit Erlöschen der Ovarialfunktion um das 50. Lebensjahr und anschlieûendem Östrogendefizit für ca. 30 Jahre (mittlere Lebenserwartung derzeit 80 Jahre) stellt sich die Frage der Knochenabbau-Protektion durch Hormonersatztherapie (HRT) mit Nutzen-Risiko-Abwägung [3,5,6,16,19,24]. Lässt sich zunehmender Verlust an Knochendichte mit länger zurückliegender Menopause als biologischer Marker für Östrogen-defizit [11] nutzen, z. B. zur HRT-Beratung hinsichtlich Neuroprotektion? Vermehrte Gehirnleistungsstörungen bei längerem Östrogendefizit sind vielfältig bestätigt [5, 9,10,12,17,18, 21, 22, 25 ± 27, 31, 32, 35, 39 ± 42]. Diese Pilotstudie wollte klären, ob und wie oft Beziehungen zwischen Knochendichte und kognitiven Funktionen bestehen. Kritiker können bei dieser denkbaren Beziehung anführen, dass Osteoporose-Probleme mit eingeschränkter Mobilität und weniger sozialen Kontakten zu Kognitionsverlusten führen. Deshalb wurde als leicht erfahrbarer östrogenbezogener Parameter die Östrogendefizit-Zeit (Lebensalter abzüglich Menopausealter und den Jahren mit Hormonersatztherapie) erfasst. Auch Fettdepots als extraovarielle Östrogenquelle interessierten. Die Werte dieser beiden Parameter sollten auch mit den Kognitions-Kurztest-Ergebnissen in Beziehung gebracht werden. Wenn bei sonst gesunden, postmenopausalen Frauen der Verlust an Knochenmasse mit Kognitionsminderung häufiger als zu erwarten einhergeht, dann wäre das von beraterischem Interesse. AbstractMany women will live for 30 years or longer after menopause, raising the issue of hormone replacement therapy. The loss of bone density with increasing duration after menopause is a potential biological marker for an estrogen deficit. Also, a number of studies have indicated cognitive loss associated with long estrogen deficits. We studied whether bone density is associated with cognitive function. Osteoporosis may lead to restricted mobility and limited social contacts and thus to decreased cognitive function. We analyzed the duration of the estrogen deficit (years since menopause without hormone replacement) and fat depots as extraovarian estrogen sources in light of results of tests of short-term cognitive function. If bone loss were associated with decreased cognitive function in otherwise healthy postmenopausal women, this would be an issue in counseling with regard to hormone replacement. Die Erfolge der endokrinen Therapiemöglichkeiten beim metastasierenden Brustkrebs sind beeindruckend. Die adjuvante endokrine Therapie befindet sich momentan im Wandel. Soll weiter wie seit 30 Jahren Tamoxifen eingesetzt werden oder Aromatasehemmer bis 5 oder mehr Jahre? Ehe eine groûzügige Umstellung erfolgt, sind mögliche längerfristige Nachteile auf das ZNS zu klären. Das Enzym Aromatase sichert die elementare Östro-genversorgung der Neuronen in vielen Hirnregionen. Bei komplett inhibierter Östrogensynthese durch einen Aromatasehemmer lässt die Synapsenplastizität im Hippocampus nach. Neurobiologisch erklärbar ist das durch fehlende neuroprotektive Östrogeneffekte. Das kann neurodegenerative Veränderungen mit Lern-und Gedächtnisproblemen bewirken. Daher ist multizentrisch zu klären, ob längerfristig Aromatasehemmer für das Gehirn riskant sind. Bisher wurde kein deutlich längeres Überle-ben unter Aromatasehemmer gegenüber Tamoxifen nachgewiesen. Die Aromataseforschung weist auf mögliche Neurodegeneration hin ± evtl. längerfristig bis zum Alzheimer-Risiko. Objective: We studied the lean body mass (LBM) as a potential, easily measured prognostic factor for osteoporosis. Methods: Lean body mass was measured by bioelectrical impedance analysis and lumbar bone mineral density (BMD) with dual energy x-ray absorptiometry in 821 postmenopausal women (age 45 ± 80 years). Patients were classified according to LBM (£ 40 kg, 28%; > 40 ± £ 45 kg, 40 %; > 45 ± £ 50 kg, 22%; > 50 kg, 10%). Contingency table analysis was performed to analyze associations between BMD, age, and risk factors for osteoporosis, particularly hormone replacement therapy (HRT). BMD values £ 0.8 g/cm 2 and > 1.0 g/cm 2 were classified as low and high, respectively. Results: Among women without HRT, those with LBM £ 40 kg had a twice as high rate of low BMD than those with LBM > 50 (41% vs. 19 %, respectively) and a lower rate of high BMD (11 % vs. 32%, respectively). Among women with > 5 years of HRT, those with a LBM £ 40 kg had a similar rate of low BMD as those with a LBM > 50 kg (17% and 12%, respectively). There were no significant differences in LBM according to age. There were no age-related associations with LBM in women with a fat mass > 43% or £ 29%. A history of osteoporosis was more common in women without HRT and a LBM £ 40 kg than in those with a LBW > 50 kg (33% vs. 11%, respectively); in women with HRT the proportions were 29% and 19 %, respectively. There was no significant correlation between LBM and physical activity. Conclusions: Measurement of LBM is useful for estimating muscle mass. A bioelectrical impedance technique is economical in time and cost and well tolerated by patients. In postmenopausal women the LBM is helpful for evaluating the risk of osteoporosis. Methode: Dazu wird ein Risikofaktor dargestellt, der heute leicht und kostengünstig erfahrbar ist ± ohne zusätzliche Belastung der Patienten. Bei 821 postmenopausalen Frauen mit Knochendichtemessung (Bone Mineral Density [BMD] mittels DE-XA-Methode) erfolgte eine Körpermasse-Analyse zur Abschätzung der fettfreien Körpermasse (Lean Body Mass [LBM] mittels bioelektrischem Impedanzverfahren). LBM (£ 40 kg [28 %], 40 < x £ 45 kg [40 %], 45 < x £ 50 kg [22 %], > 50 kg [10%]) wurde mit Knochendichtewerten, Alter sowie Osteoporose-Risikofaktoren in Beziehung gebracht. Of 700 women on whom mammograms were carried out for various indications, more than half complained of mastalgia. A quarter of the women had suffered frequently from diffuse breast pain, and one third complained of mastaglia. A quarter of the women had suffered frequently from diffuse breast pain, and one third had had pain for years. One third complained of local breast pain. Changes in the mammogram amongst women with diffuse breast pain were no more common than in women without symptoms. There was no correlation between the frequency of pain and the extent of a "mastopathy". There were no changes which could be related to the pain. Similarly there was no difference in the frequency of biopsies or women kept under observation between those with breast pain and those without. This was also true for those with local breast pain, and also for those with clinically occult conditions. Breast size was of no significance for the occurrence of pain. Neither local nor diffuse breast pain is of any specific significance in breast diagnosis.
