The Trail Making Test (TMT) has been a useful tool for neuropsychological assessment. The present study analyzed the influence of age and education level on TMT performance in healthy adult Portuguese speakers. The criteria for exclusion were a history of neurological disease, psychiatric illness, substance abuse, learning disabilities, or any other difficulty that may interfere with testing. The sample (n = 318) was divided into four age groups: (i) very young (n = 92; 18-34 years old), (ii) young (n = 66; 35-49 years old), (iii) middle-age (n = 117; 50-64 years old), and (iv) elderly (n = 43; 65-81 years old). The sample was also divided into three education levels: (i) low (2-8 years), (ii) middle (9-11 years), and (iii) high (≥ 12 years). Correlations among the demographic variables and scores on Trails A and B showed that age and education level were the two variables that most affected TMT scores (p < 0.001, analysis of covariance). Post hoc analyses of age did not reveal significant differences between the middle-age and elderly groups in TMT-A performance or between middle and high education level in TMT-A or TMT-B performance. These results are consistent with previous studies and demonstrate that age and education level affect the performance of Brazilian subjects on the TMT.
The Rey Auditory Verbal Learning Test (RAVLT) is an efficient neuropsychological instrument for testing episodic declarative memory. Performance on this test can be influenced by demographic and cultural variables. The objective of the present study was to analyze the influence of demographics variables, such as age, gender, and education, on RAVLT performance in young and elderly adults. A Portuguese version of the RAVLT was administered to 302 healthy participants. Subjects were from both genders, ranging in age from 17 to 85 years (mean 50.6 ± 15.9 years) and in education from 1 to 20 years (mean 11.3 ± 3.7 years). Participants were grouped by age: 17-34 years old, 34-49 years old, 50-64 years old, and 65-85 years old. Pearson's Correlation analysis showed a significant association between RAVLT performance and both age and education (p < .001), but not between RAVLT performance and gender. Two-way analysis of variance revealed significant effects of age on all RAVLT measurements and significant effects of years of education on all measurements, with the exception of recognition. The present data indicate that participant age and education significantly influence performance on the RAVLT.
RESUMO. O crescimento da população idosa é um fenômeno mundial. Em 2025 o Brasil será o 6º país do mundo quanto ao contingente de idosos. Concomitantemente com o aumento da população idosa ocorre o aumento das doenças crônicodegenerativas, como a Doença de Alzheimer (DA). A DA é caracterizada por um declínio progressivo nas áreas de cognição, função e comportamento. À medida que a doença progride surge a demanda por cuidados especiais, função importante desempenhada pelos cuidadores. Pesquisas têm evidenciado que o declínio do paciente e suas demandas específicas são fatores importantes de estresse para o cuidador. Estes apresentam altos índices de sintomas psiquiátricos-especialmente a depressão-, prejuízos no sistema imunológico, assim como altos índices de conflitos familiares. Outros estudos sugerem que altos índices de sobrecarga e impacto no cuidador estão associados à institucionalização do paciente. O presente estudo de revisão tem por objetivo levantar os principais aspectos determinantes do impacto no cuidador de pacientes com DA.
Este estudo comparou e relacionou o desempenho de idosos com Comprometimento Cognitivo Leve e Demência Tipo Alzheimer em testes de controle executivo e de memória episódica verbal. Para a avaliação da memória episódica verbal utilizamos a Tarefa de Recordação de Palavras Imediata e a Tarefa de Recordação de Palavras com Intervalo. Na avaliação do controle executivo foram utilizados, os seguintes testes: Random Number Generation, Trail Making Test, Fluência Verbal semântica e fonológica, Wisconsin Card Sorting Test, Reading Span Test e Brow-Peterson Test. Treze idosos do grupo controle foram equiparados quanto à idade e escolaridade com nove idosos com Comprometimento Cognitivo Leve e oito idosos com Demência Tipo Alzheimer. Foram encontradas diferenças e associações estatisticamente significantes em relação aos testes de controle executivo entre os grupos investigados. Apesar da presença de associações significativas entre a memória episódica verbal e o controle executivo, não foram evidenciados déficits do controle executivo nos idosos com Comprometimento Cognitivo Leve.
ResumoO interesse pelo estudo das funções executivas tem crescido nos últimos anos. O objetivo deste artigo é revisar algumas questões metodológicas e conceituais relacionadas à avaliação neuropsicológica das funções executivas. As principais limitações e dicotomias relacionadas às funções executivas são apresentadas e a importância de um modelo teórico que fundamente os instrumentos de avaliação e suas interpretações clinicas analisada. Conclui-se que as evidências provenientes da investigação das funções executivas ainda não permitem a compreensão satisfatória desses processos. Sugere-se, a adoção de um modelo específico para o estudo das funções executivas. Palavras-chave: Neuropsicologia; Testes neuropsicológicos; Lobo frontal. AbstractThe article aims to review methodological and conceptual issues related to neuropsychological assessment of executive functions. The main limitations and dichotomies of current studies related to executive functions are presented and the importance of theoretical support to validate the instruments and their clinical interpretation are discussed. There is still the need of additional evidences on executive functions that allow a complete understanding of all the processes involved. Finally, it is suggested the adoption of a specific model that may guide the study of the executive functions.
RESUMOEste artigo tem por objetivo, a partir da compreensão histórica de como o campo da Neuropsicologia se constituiu, propor diretrizes para a pesquisa em avaliação e a reabilitação neuropsicológica no Brasil. A história da Neuropsicologia é apresentada através de três grandes temáticas: hipótese cardíaca versus hipótese cerebral, localizacionismo versus holismo e funcionalismo versus cognitivismo. Estes temas influenciaram as pesquisas em Neuropsicologia e delimitaram as práticas atuais na área. O presente artigo enfoca dois temas importantes na atualidade da Neuropsicologia, especialmente no Brasil: primeiro, o tópico da avaliação neuropsicológica no contexto do desenvolvimento humano e, segundo, a temática da reabilitação neuropsicológica em diversos grupos. O estudo sobre as diferenças no funcionamento cerebral nas diferentes faixas etárias, crianças e idosos é brevemente apresentado como campo relevante para a construção teórica e prática da neuropsicologia. Finalmente, as dificuldades encontradas em implementar intervenções em reabilitação no Brasil são discutidas. ABSTRACT Neuropsychological Assessment and Rehabilitation:Historical Development and Current Perspectives The present paper aimed to briefly present a historical view of Neuropsychology and later, to propose research topics in Brazil. Neuropsychology history encompassed at least three major themes: the cardiac hypothesis versus the brain hypothesis, localizationism versus holism and functionalism versus cognitivism. These themes influenced research in neuropsychology and based current practice. Furthermore, two important aspects of neuropsychology are discussed: neuropsychological assessment in different age groups and neuropsychological rehabilitation. Major topics involved on the study of the differences in brain functioning through life span are briefly presented and their role in building evidence-based practice and in supporting theoretical models is showed. Finally, several difficulties found in adapting neuropsychological rehabilitation to the Brazilian context are discussed.
