Good tolerability of workload within 30-40 min. was observed. The improvement in the EDSS was detected in 9 patients, in whole, a significant positive trend (p<0.01) was shown. The study of the biomechanics of the walk showed its significant impairment compared to healthy individuals: reduction of parameters of rate, speed and step length, significant instability, pronounced asymmetry, the decrease in support and shock lower limb function, high coefficient of variability of the parameters, the phenomenon of recurrence of the vertical component of support reactions. After a course of exercise of walking in the exoskeleton, the walking speed and stability increased, oscillation of the body decreased, support function increased, the phenomenon of cyclical changes of the vertical component of support reactions reduced. The results of the pilot study showed promising future research opportunities for robotic-assisted walking and maintenance of the vertical posture with the help of the exoskeleton ExoAtlet to restore the abilities of movement in MS patients with locomotor disorders.
Acute ultraviolet (UV) -induced skin damage is associated with structural alterations, vasodilatation and inflammatory response. Leukocyte infiltration is one of the main features of inflammation and could be found in the area of UV injury. It was shown that porphyrins which have well-known autofluorescent properties play a role in the chemoattraction of immune cells to the area of local damage. This study examined the possibility of application of laser fluorescence spectroscopy (LFS) in the assessment of ultraviolet-induced immune response in ICR mice. Animals (N=25) were exposed by UVB light and LFS was conducted on the dorsal skin of each mice 0, 0.5, 3, 6 and 24 hours after UV irradiation. Moreover, in every time point we performed skin biopsy and histology. Using LFS, time-dependent dynamic changes in the fluorescence parameters of porphyrins were found. Mentioned indices were in a good agreement with histological findings. Statistically significant correlation was found between the severity of inflammatory infiltrate and the tissue content index (η) of porphyrins (Pearson correlation coefficient: r = 0.912, p = 0.031). Achieved results not only have fundamental value but could be further investigated and applied in clinical practice: e.g. to objectively predict individual immunologic reaction to UV-light.
It is known that neurotic disorders can be independent risk factors for the development of complications in patients with cardiovascular diseases. This requires the creation of new rehabilitation programs that include not only medicines but also physiotherapy. To this end, an analysis was made of the effect of complex treatment, including visual color impulse therapy, on the psychoemotional state of 80 patients with cardiovascular diseases, hypertensive disease and coronary heart disease, who also have asthenic-depressive symptoms. In this case, patients in two control groups received standard drug therapy, and in the two main groups received additional color pulse therapy from the «Mellon» apparatus. Dynamics of the psychoemotional state of patients was studied with the help of a test that determines the quantitative changes in the state of health, activity and mood, as well as the stress index that characterizes the regulation of the heart rhythm under stress. Prior to the start of treatment, functional disintegration of regulatory mechanisms was characteristic for all patients, which was accompanied by psychoemotional disorders that indirectly reflected the state of highly organized nervous processes in the cerebral cortex, which altered the values of SI stress index indicators and the «well-being, activity, mood» test. The regression statistical analysis was used in this work. As a result, the indications and contraindications to the use of this method of treatment were determined. After the course of complex treatment, including drug and visual color impulse therapy, positive dynamics in the patients’ state was revealed, which indicates an increase in the functional capabilities of their body, and also, given the good tolerability of visual color impulse therapy, the feasibility of including this method in complex treatment at any stage of rehabilitation.
Objective. To increase the efficiency of the complex of therapeutic and rehabilitation measures in preparation for hip joint endoprosthetics. Materials and methods. 66 patients in whom it was planned to perform hip joint endoprosthetics took part in the research. They were divided into two groups. The main group (n = 32) included patients who underwent 5 binaural beats as a relaxation program. In the comparison group (n = 34) the patients received a standard set of measures to prepare for this operation. All patients were screened for depression and anxiety using the standardized hospital scale for anxiety and depression (HADS) before the study. We also recorded the initial levels of reactive and personality anxiety using the Spielberger Hanin test. We repeated the test in both groups after 7 days to evaluate the dynamics of the test. Results. The conducted research showed that in the main group on the background of binaural beating procedures, reactive (from 57.2 3.8 to 42.4 5.2 points, p = 0.014) and personal anxiety (from 58.9 4.1 to 44.7 3.8 points, p = 0.003) were significantly reduced. In addition, the application of binaural beats method resulted in a significant decrease of HADS alarm subscale indexes (p 0.001) in patients of the base group from 12.8 2.8 to 8.5 0.7 points. While patients in the comparison group had significantly less expressed decrease of this parameter from 11.7 3.1 to 10.9 1.6 points (p 0.01). On the HADS depression subscale there was also a marked decrease in the main group to 7.1 0.8 points, this value was statistically significantly lower (p = 0.011) than in the comparison group 10.2 1.2 points. Conclusion. The conducted research has shown that the use of the binaural beats method in the complex of measures to prepare patients for hip joint endoprosthetics helps to improve their psycho-emotional status. This is manifested by a decrease in personal and reactive anxiety in the Spielberger Hanin test, as well as the severity of depression and anxiety on the HADS scale. The advantages of the method are non-invasive and easy to use, and the disadvantages include the duration of the procedure and poorly studied mechanism of action.
Актуальность. Ограниченная склеродермия часто приводит к необратимым косметическим изменениям, а в далеко зашедших случаях -к нарушению функции органа и инвалидности. Медикаментозная терапия недостаточно эф-фективна, она может вызывать побочные явле-ния и осложнения. В связи с этим большое вни-мание уделяется немедикаментозным методам лечения данного заболевания, способным оказывать положительное клинико-патогене-тическое влияние и потенцировать эффекты медикаментозной терапии. Цель -изучение влияния локального длинноволнового инфра-красного излучения на течение ограничен-ной склеродермии и оценка терапевтической эффективности данного метода в комплексе с традиционно проводимой медикаментозной терапией. Материал и методы. Под наблюде-нием находились 55 пациентов с ограничен-ной склеродермией. Пациенты контрольной группы (n = 20) в течение 14-20 суток получали стандартную медикаментозную терапию: анти-бактериальные препараты, протеолитические ферменты, блокаторы кальциевых каналов, ан-тиоксиданты, ангиопротекторы и корректоры микроциркуляции для местного применения, а также аппликации кортикостероидных гор-монов. Пациентам основной группы (n = 35) на фоне стандартной медикаментозной терапии проводилось локальное длинноволновое ин-фракрасное облучение патологических очагов (длина волны 4-16 мкм, 1 раз в сутки 10 дней). Оценивали динамику интенсивности отека и гиперемии по периферии очага поражения, распространенность и выраженность патоло-гического процесса с помощью модифициро-ванной методики балльной системы mRODNAN Skin Score, а также состояние микроцирку-ляторного русла на аппарате «Спектротест». Результаты. У пациентов, получавших в до-полнение к стандартной медикаментозной терапии ограниченной склеродермии локаль-ное длинноволновое инфракрасное излуче-ние, по сравнению с контрольной группой было отмечено более выраженное снижение индекса mRODNAN (6,3 ± 0,5 и 2,2 ± 0,1 соот-ветственно, р < 0,05), значительное снижение показателей сатурации кислорода в пато-логических очагах (0,9 ± 0,001 и 0,646 ± 0,04 у.е., р < 0,05) и повышение объема кровотока в микроциркуляторном русле (0,091 ± 0,002 и 0,23 ± 0,045 у.е., р < 0,05), а также более бы-строе наступление клинического эффекта -на 14 ± 2,3 дня (р < 0,05). В контрольной группе кли-нический эффект наступал лишь на 20 ± 4,2 дня (р < 0,05), в очагах поражения была отмечена тенденция к снижению сатурации кислорода крови с 0,87 ± 0,01 до 0,817 ± 0,005 у.е. и уве-личению объема кровотока в микроциркуля-торном русле с 0,086 ± 0,004 до 0,1 ± 0,003 у.е. Стандартная медикаментозная терапия не обе-спечила полного разрешения элементов огра-ниченной склеродермии, что подтверждалось результатами оценки индекса mRODNAN (до лечения его среднее значение в этой группе составляло 6,4 ± 0,5, после лечения -5,3 ± 0,2, р ≥ 0,05). Заключение. Доказана клиниче-ская эффективность применения локального длинноволнового инфракрасного излучения в комплексной терапии ограниченной скле-родермии.
