BACKGROUND: The role of microcirculatory disorders is progressively being accepted in the pathogenesis of cardiovascular diseases. OBJECTIVE: The purpose of current study is to assess whether we can consider skin microcirculation disorders as a biomarker of cardiovascular events. METHODS: Group 1 consisted of healthy volunteers (n = 31); group 2 (n = 42) consisted of patients with diseases that increase the risk of cardiovascular events; group 3 (n = 39) included patients with the history of cardiovascular events. Skin microcirculation measurement was performed using laser Doppler flowmetry during the heating test. RESULTS: LDF parameters reflecting the rapid response of microcirculation to heating (“Slope 120 s” and “Slope 180 s”) significantly differed in three groups (p < 0.05). A decrease in the “Slope 180 s” parameter less than 0.5 PU/s is associated with cardiovascular events (sensitivity 69.2%, specificity 66.7%; the area under the ROC curve, 0.667; 95% confidence interval [CI], 0.545–0.788, p = 0.01). Multivariable logistic regression analysis revealed that “Slope 180 s≤0.5 PU/s” was significantly related to cardiovascular events (adjusted odds ratio = 3.9, p = 0.019, CI 95% 1.2–12). CONCLUSIONS: Reduced reactivity of the skin microcirculation may be useful as a biomarker of severe damage to the cardiovascular system and is promising as a risk factor for cardiovascular events.
Acute ultraviolet (UV) -induced skin damage is associated with structural alterations, vasodilatation and inflammatory response. Leukocyte infiltration is one of the main features of inflammation and could be found in the area of UV injury. It was shown that porphyrins which have well-known autofluorescent properties play a role in the chemoattraction of immune cells to the area of local damage. This study examined the possibility of application of laser fluorescence spectroscopy (LFS) in the assessment of ultraviolet-induced immune response in ICR mice. Animals (N=25) were exposed by UVB light and LFS was conducted on the dorsal skin of each mice 0, 0.5, 3, 6 and 24 hours after UV irradiation. Moreover, in every time point we performed skin biopsy and histology. Using LFS, time-dependent dynamic changes in the fluorescence parameters of porphyrins were found. Mentioned indices were in a good agreement with histological findings. Statistically significant correlation was found between the severity of inflammatory infiltrate and the tissue content index (η) of porphyrins (Pearson correlation coefficient: r = 0.912, p = 0.031). Achieved results not only have fundamental value but could be further investigated and applied in clinical practice: e.g. to objectively predict individual immunologic reaction to UV-light.
Актуальность. Увеличение тонуса сосудов считается одним из важных факторов, приводящих к артериальной гипертонии (АГ). В клинической практике нет методов прямого измерения тонуса сосудов. Косвенно оценить тонус сосудов системы микроциркуляции можно при помощи исследования перфузии методом лазерной доплеровской флоуметрии (ЛДФ). Перфузия характеризует изменение потока крови в единицу времени в анализируемом участке, следовательно, этот параметр должен быть обратно пропорционален тонусу сосудов. Цель -сравнить перфузию кожи предплечья, измеренную методом ЛДФ, у пациентов с АГ и добровольцев с нормальными показателями артериального давления (АД), рассмотреть практическую применимость этого параметра для оценки тонуса сосудов системы микроциркуляции. Материал и методы. Работа проведена на двух группах: группу 1 составили пациенты с АГ (n = 43; медиана возраста -62 [57; 71] года), группу 2 -добровольцы без АГ (n = 62; медиана возраста -28 [24; 37] лет). Измерение перфузии кожи предплечья проводили методом ЛДФ в течение 2 минут без применения функциональных проб. Для каждого обследуемого рассчитывали «базовую перфузию» -средний показатель перфузии на репрезентативном участке микроциркуляторной кривой. Результаты. Медиана базовой перфузии в коже руки у пациентов с АГ значимо больше, чем у нормотензивных лиц: 4,88 [2,87; 8,98] и 3,41 [2,47; 4,99] ПЕ соответственно (p = 0,013). В качестве условного «нормального» уровня перфузии принято значение, соответствующее интерквартильному диапазону в контрольной группе. У 39,5% пациентов с АГ базовая перфузия соответствовала «норме», 46,5% пациентов имели показатель базовой перфузии выше «нормы», что может быть обусловлено сниженным тонусом периферических сосудов, и лишь у 14,0% пациентов базовая перфузия была снижена. Заключение. Повышение перфузии крови в коже у части пациентов с АГ может свидетельствовать о снижении тонуса периферических сосудов, что, предположительно, является компенсаторной реакцией организма в ответ на повышение артериального давления. На полученные результаты, вероятно, влияли возрастные изменения сердечно-сосудистой системы, лекарственная терапия и другие факторы. Представляется целесообразным продолжить исследования состояния периферических сосудов. Их результаты могут внести вклад в понимание механизмов развития АГ у конкретного пациента, а в перспективе -применяться для персонализированного подхода в подборе терапии.
