Rev Esc Enferm RESUmEnObjetivo: Identificar el costo directo de los procedimientos relacionados con el tratamiento de quimioterapia ambulatoria para las mujeres con cáncer de mama. Método: Investigación cuantitativa, del tipo de estudio de caso, realizada en el ambulatorio de quimioterapia de un hospital privado Se calculó el costo total mediante la multiplicación del tiempo dedicado por los profesionales involucrados en el proceso, por el costo unitario de la mano de obra directa, sumándose al costo de los materiales, fár-macos y soluciones. Para la realización de los cálculos se utilizó la moneda brasileña (R$). Resultados: El costo total medio, por sesión de quimioterapia, correspondió a R$ 1.783,01 (100%), de los cuales R$ 1.671,66 (93,75%) fueron con fármacos, R$ 74,98 (4,21%) con materiales, R$ 28,49 (1,60%) con mano de obra y R$ 7,88 (0,44%) con soluciones. Conclusión: Los resultados logrados podrán servir como base para las discusiones y la toma de decisiones con respecto a la gestión de los costos relacionados con la quimioterapia, con vistas a la reducción de los gastos y la eliminación de los desperdicios, sin perjuicios a la atención prestada. dEScRiPtoRESNeoplasias de la mama Quimioterapia Costos de la Atención en Salud Costos y análisis de costo Enfermería oncológica. RESUmoObjetivo: Identificar o custo direto de procedimentos relacionados ao tratamento quimioterápico ambulatorial de mulheres portadoras de câncer de mama. Método: Pesquisa quantitativa, do tipo estudo de caso, realizada no ambulatório de quimioterapia de um hospital privado. O custo total foi calculado multiplicando-se o tempo despendido pelos profissionais envolvidos nos procedimentos terapêu-ticos, pelo custo unitário da mão de obra direta, somando-se ao custo dos materiais, fármacos e soluções. Objective: to analyze managers and professionals' perceptions on the changes in hospital management deriving from accreditation.Method: descriptive study with qualitative approach. The participants were five hospital quality managers and 91 other professionals from a wide range of professional categories, hierarchical levels and activity areas at four hospitals in the South of Brazil certified at different levels in the Brazilian accreditation system. They answered the question "Tell me about the management of this hospital before and after the Accreditation". The data were recorded, fully transcribed and transported to the software ATLAS.ti, version 7.1 for access and management. Then, thematic content analysis was applied within the reference framework of Avedis Donabedian's Evaluation in Health.Results: one large family was apprehended, called "Management Changes Resulting from the Accreditation: perspectives of managers and professionals" and five codes, related to the management changes in the operational, structural, financial and cost; top hospital management and quality management domains.Conclusion: the management changes in the hospital organizations resulting from the Accreditation were broad, multifaceted and in line with the improvements of the service quality.

Objective: To learn the perceptions of hospital quality managers about the benefits and difficulties that arise from accreditation. Methods:A descriptive, qualitative and exploratory study was developed with five quality managers from hospitals of each of the three levels of national accreditation and one which lost its accreditation. Taped interviews were carried out with the participants, which were guided by the question: Tell us about the benefits and difficulties to obtain and maintain hospital accreditation.Transcribed data were submitted to thematic content analysis. Results: Categories of "benefits" and "difficulties" related to accreditation were created, each of them with two specific subcategories. Conclusion: In spite of the barriers related to the organizational culture and staff turnover, accreditation was mentioned by managers as beneficial to the quality of management and care because, through the standardization of processes and user centralization, the provision of care services is focused on quality.Keywords: Hospital accreditation; Quality management; Hospital administration; Health manager; Nursing. resumoObjetivo: Apreender as percepções de gestores da qualidade hospitalar quanto às vantagens e dificuldades advindas da Acreditação. Aos participantes, fez-se entrevista gravada e norteada pela questão: Conte-me sobre as vantagens e dificuldades de obter e manter a certificação pela Acreditação. Os dados transcritos foram submetidos à Análise de Conteúdo, modalidade temática. Resultados: Foram apreendidas categorias referentes às "vantagens" e às "dificuldades" relacionadas à Acreditação, cada qual com duas subcategorias pontuais.
