The purposes of the study were to identify the prevalence of defining characteristics (DC) of decreased cardiac output (DCO) in patients with cardiac insufficiency under evaluation for heart transplantation, and to ascertain the likelihood of defining characteristics being Prevalencia del diagnóstico de enfermería de disminución del gasto cardíaco y valor predictivo de las características definidoras en pacientes en fase de evaluación para trasplante cardíacoEl estudio tiene como objetivos identificar la prevalencia de las características definitorias de la disminución del gasto cardíaco (DGC) en pacientes con insuficiencia cardíaca (IC) en evaluación para el trasplante de corazón y ver la probabilidad de las características definitorias sean factores predictivos de la existencia de una disminución del gasto cardíaco. Los datos se obtuvieron mediante el análisis documental retrospectivo de historias clínicas de cateterismo cordiaco derecho en 38 pacientes con insuficiencia cardíaca en la evaluación para el trasplante de corazón entre 2004 y 2009. Los resultados mostraron que el 71% de los pacientes habían reducido el gasto cardíaco (medido por el índice cardíaco). La mayoría de las características definitorias de la NANDA-International para el DGC fueron más frecuentes en individuos con índice cardíaco reducido. Los aspectos más destacados son la razón de probabilidad (odds ratio / OR) RVS mayor OR=4,533, el tercer sonido OR=3,429 y la disminución de la fracción de eyección OR=2,850. Al obtener el valor predictivo de las características definitorias de estudio indica la importancia de las características para el diagnóstico de la disminución del gasto cardíaco.

Objective: Identifying Nursing Diagnoses of fatigue, activity intolerance and decreased cardiac output in hospitalized patients with heart failure and verifying the association between the defining characteristics and the Nursing Diagnoses. Method: A longitudinal and prospective study that followed hospitalized patients with heart failure for three weeks. The data collected through interviews and physical examinations were sent to expert nurses for diagnostic inference. Descriptive and inferential statistical analyses were carried out. Results: Of the 72 patients, 68.0% were male and presented the nursing diagnosis of decreased cardiac output (62.5%) in the first week, reducing to 52.8% and 38% in the second and third weeks, respectively. Fatigue only appeared in one patient. Activity intolerance was the diagnosis that had the greatest discrepancy among the experts. Decreased cardiac output was associated to the defining characteristics: dyspnea, edema, jugular venous distension and reduced ejection fraction during all three weeks of evaluation. Conclusion: Decreased cardiac output was more prevalent in hospitalized patients with heart failure, and the associated defining characteristics were determining factors for this nursing diagnosis.

NURSING DIAGNOSIS APPLIED TO PATIENTS WITH DECOMPENSATED HEART FAILUREABSTRACT: The present study aimed to identify the priority nursing diagnoses for patients with decompensated heart failure. Quantitative study conducted in a cardiac emergency service of a university of Pernambuco, between October 2014 and February 2015. The study sample was composed of 62 patients admitted for the treatment of acute decompensated heart failure, emergency inpatients and patients from any functional class. The main nursing diagnoses were Decreased Cardiac Output (87.3%), Activity Intolerance (79.4%) and Ineffective Breathing Pattern (38.1%). The signs and symptoms of decompensation caused by systemic or pulmonary congestion provided the basis for the recognition of defining characteristics that made it possible to formulate priority nursing diagnoses for these patients. Las señales y síntomas de la descompensación, que vienen de la congestión sistémica o pulmonar, fueron utilizadas como base para el reconocimiento de las características determinantes, las cuales posibilitaron la elaboración de los diagnósticos de enfermería prioritarios para eses pacientes.
