Spinal cord injuries remain an important factor of morbimortality in current society, involving mainly males from adolescence to adulthood. Among the sequelae caused by spinal cord injuries, the impairment of the sexual system is highly relevant since it affects the quality of sexual life and paternity. Infertility is secondary to multiple events such as erectile dysfunction, anejaculation, seminal biochemical modification and morphology of spermatozoa. Current therapies for the infertile spinal cord injured patient focus on the ejaculation stimulus followed by intrauterine insemination, leaving seminal low quality as the major factor of infertility in these patients. In this scenario, therapy with hyperbaric oxygenation, which is still being studied, represents an alternative treatment since it focuses on the central nervous system injured by the trauma and the testicular tissue in order to decrease spinal damage and to preserve the physiological regulation of the urogenital system as a form of avoiding infertility.

Agrupar e atualizar conhecimentos em relação à atuação fisioterapêutica no trauma raquimedular (TRM) em unidade de terapia intensiva (UTI). Método. Foi realizada atualização de literatura nas bases de dados Lilacs, PubMed e Scielo, cruzando os descritores spinal cord injury, cinesiotherapy, physiotherapy, mobilization, rehabilitation, intensive care unit, respiratory therapy e electrotherapy, no período de 2005 a 2010. Resultados. Foram encontrados 21 estudos, porém, apenas cinco artigos preencheram os critérios de inclusão. A cinesioterapia é imprescindível desde a fase de choque medular, pois favorece a manutenção da amplitude de movimento articular e flexibilidade, além de prevenir complicações circulatórias decorrentes da imobilização prolongada no leito. A fisioterapia respiratória promove a higiene brônquica, correção de padrões ventilatórios anormais e de algumas patologias respiratórias. A eletroterapia é um recurso ainda pouco utilizado por fisioterapeutas no setor de cuidados intensivos. Conclusões. A atuação fisioterapêutica no TRM em UTI encontra-se focada na reabilitação motora por meio da cinesioterapia e na intervenção respiratória por meio da higiene brônquica e treinamento dos músculos respiratórios. Novas modalidades de tratamento, como a eletroterapia, têm surgido no ambiente intensivo, porém há necessidade de maiores estudos para confirmar benefícios e riscos deste recurso nos lesados medulares. ABSTRACTObjective. Collate and update knowledge in relation to physical therapy in spinal cord injury (SCI) in the intensive care unit (ICU). Method. We performed a literature update in the databases Lilacs, PubMed and Scielo, crossing the descriptors spinal cord injury, cinesiotherapy, physiotherapy, mobilization, rehabilitation, intensive care unit, respiratory therapy and electrotherapy in the period of 2005 to 2010. Results. We found 21 studies, however, only five articles met the inclusion criteria. Kinesiotherapy is essential since the phase of spinal shock, since it favors the maintenance of joint range of motion and flexibility, and to prevent circulatory complications caused/ resulted from prolonged immobilization in bed. Respiratory therapy promotes bronchial hygiene, correction of abnormal respiratory patterns and respiratory diseases. The electrotherapy is a feature still little used by physiotherapists in the intensive care units. Conclusions. The physical therapy in SCI in ICU is focused on motor rehabilitation through kinesiotherapy and intervention through the respiratory bronchial hygiene and training of respiratory muscles. New treatment modalities such as electrotherapy, there have been in intensive environment, there is a need for more studies to confirm benefits and risks of this feature in the spinal cord.
