The cardiovascular diseases are among the main death causes in the developed world. They have been increasing epidemically in the developing countries. In spite of several alternatives for the treatment of the coronary artery disease; the surgery of the myocardial revascularization is an option with proper indications of medium and long-term with good results. It provides the remission of the angina symptoms contributing to the increase of the expectation and improvement of the life quality. Most of patients undergoing myocardial revascularization surgery develop postoperative lung dysfunction with important reduction of the lung volumes, damages in the respiratory mechanism, decrease in the lung indulgence and increase of the respiratory work. The reduction of volumes and lung capacities can contribute to alterations in the gas exchanges, resulting in hypoxemia and decrease in the diffusion capacity. Taking this into account, the Physiotherapy has been requested more and more to perform in the pre as well as in the postoperative period of this surgery. This study aimed at updating the knowledge regarding the respiratory physiotherapy performance in the pre and postoperative period of the myocardial revascularization surgery enhancing the prevention of lung complications. The Physiotherapy uses several techniques in the preoperative period; such as: the incentive spirometry, exercises of deep breathing, cough, inspiratory muscle training, earlier ambulation and physiotherapeutic orientations. While in the postoperative period, the objective is the treatment after lung complications took place, performed by means of physiotherapeutic maneuvers and noninvasive respiratory devices, aiming at improving the respiratory mechanism, the lung reexpansion and the bronchial hygiene. Respiratory physiotherapy is an integral part in the care management of the patient with cardiopathy, either in the pre or in the postoperative period, since it contributes significantly to a better prognosis of these patients with the use of specific techniques. Amyotrophic Lateral Sclerosis (ALS) is a fatal progressive neurodegenerative disease with multifactorial etiology for which, so far, there is no effective medicinal treatment. However, by means of kinesiotherapy intervention and patient guidance and care, physiotherapy can delay physical functional losses, muscle fatigue and immobility of the joint-muscle system, thereby improving the quality of life. This survey had the aim of reviewing the physiotherapeutic conduct currently used in ALS cases. Monthly monitoring is recommended, with changes in goals and conduct at each stage of the disease, activities to be pursued around the home, and emphasis on stretching, muscle strengthening, posture adequacy and respiratory kinesiotherapy.
Trauma raquimedular é uma lesão neurológica incapacitante que ocasiona paralisia, perda sensorial e disfunção fisiológica, envolvendo uma série de funções corporais com grande impacto na sociedade. Representa um problema de saúde pública e afeta principalmente a população de adultos jovens, do sexo masculino, podendo resultar em morte ou deficiência. Este estudo teve como objetivo agrupar e atualizar conhecimentos em relação à atuação fisioterapêutica no trauma raquimedular em ambiente hospitalar. Foi realizada uma pesquisa da literatura nas bases de dados SciELO, Lilacs e PubMed, cruzando-se os descritores spinal cord injury, cinesiotherapy, physiotherapy, mobilization, rehabilitation, physical therapy department, respiratory therapy e electrotherapy, no período de 2005 a 2010. A cinesioterapia é imprescindível desde a fase de choque medular, pois favorece a manutenção da amplitude de movimento articular e flexibilidade, além de prevenir complicações circulatórias decorrentes da imobilização prolongada no leito. A fisioterapia respiratória promove a higiene brônquica e a correção de padrões ventilatórios anormais e de algumas patologias respiratórias. A eletroterapia é um recurso ainda pouco utilizado por fisioterapeutas no âmbito hospitalar. O presente estudo evidenciou a escassez de pesquisas específicas que abordem a temática da atuação do fisioterapeuta sobre o trauma raquimedular em ambiente hospitalar. O objetivo deste estudo foi agrupar e atualizar conhecimentos referentes à intervenção fisioterapêutica na incontinência fecal no idoso. A pesquisa da literatura foi realizada nas bases de dados eletrônicas: MEDLINE, LILACS, Cochrane, PubMed e SciELO, no período de janeiro de 2006 a julho de 2011. As palavras-chave usadas em várias combinações foram: fecal incontinence, aged, physical therapy, physiotherapy, exercises, rehabilitation e electrotherapy. A intervenção fisioterapêutica incluiu a avaliação da incontinência fecal constituída pelos sinais e sintomas, exame físico e exames complementares, que incluíram a manometria anorretal, a retossigmoidoscopia, a defecografia, a eletromiografia anorretal, a ultrassonografia endoanal e a latência motora terminal do nervo pudendo; os exercícios para fortalecimento da musculatura do assoalho pélvico, o biofeedback, o treinamento com balão retal e a eletroestimulação constituem a base do tratamento. Essa atualização demonstrou a escassez de estudos abordando tal temática, encontrando apenas cinco estudos publicados nos últimos anos com pacientes idosos incontinentes, os quais enfatizaram em sua maioria os procedimentos já consagrados na literatura refutando as evidências já conhecidas. Porém, uma modalidade consideravelmente nova de tratamento, a neuroestimulação sacral, tem sido investigada com resultados aparentemente positivos, suscitando a necessidade de novos estudos com desenhos metodológicos específicos sobre este problema.
Bleeding after open heart surgery is still a great concern for the surgeon, especially when the surgical field has been revised accurately and hemostatic stitches and electrical cauterization have been used extensively. Among non-surgical adjuncts, aprotinin has been reported as very effective in reducing complications. At the time we started using this drug, we intended to test two different dosages lower than those reported in the literature. We evaluated three groups of 18 patients: the first (A) received about 350 mg of aprotinin from the start of anesthesia up to the end of operation (140 mg in the priming of cardio-pulmonary bypass and 70 mg/h i.v. during the procedure; the second (A/2) received half that dose (i.e. 70 mg and 35 mg, respectively), and the third (C) did not receive aprotinin. We compared in these groups: postoperative bleeding, blood transfusions, red blood cells, hemoglobin, hematocrit, platelets. The results were good only in the A group: bleeding was reduced and few transfusions were required. The patients in the A/2 and C groups did not show significant differences. From our observations we conclude that aprotinin is a useful adjunct, but has to be given in the proper dose.

Resultados. Foram encontrados 21 estudos, porém, apenas cinco artigos preencheram os critérios de inclusão. A cinesioterapia é imprescindível desde a fase de choque medular, pois favorece a manutenção da amplitude de movimento articular e flexibilidade, além de prevenir complicações circulatórias decorrentes da imobilização prolongada no leito. A fisioterapia respiratória promove a higiene brônquica, correção de padrões ventilatórios anormais e de algumas patologias respiratórias. A eletroterapia é um recurso ainda pouco utilizado por fisioterapeutas no setor de cuidados intensivos. Conclusões. A atuação fisioterapêutica no TRM em UTI encontra-se focada na reabilitação motora por meio da cinesioterapia e na intervenção respiratória por meio da higiene brônquica e treinamento dos músculos respiratórios. Novas modalidades de tratamento, como a eletroterapia, têm surgido no ambiente intensivo, porém há necessidade de maiores estudos para confirmar benefícios e riscos deste recurso nos lesados medulares. ABSTRACTObjective. Collate and update knowledge in relation to physical therapy in spinal cord injury (SCI) in the intensive care unit (ICU). Method. We performed a literature update in the databases Lilacs, PubMed and Scielo, crossing the descriptors spinal cord injury, cinesiotherapy, physiotherapy, mobilization, rehabilitation, intensive care unit, respiratory therapy and electrotherapy in the period of 2005 to 2010. Results. We found 21 studies, however, only five articles met the inclusion criteria. Kinesiotherapy is essential since the phase of spinal shock, since it favors the maintenance of joint range of motion and flexibility, and to prevent circulatory complications caused/ resulted from prolonged immobilization in bed. Respiratory therapy promotes bronchial hygiene, correction of abnormal respiratory patterns and respiratory diseases. The electrotherapy is a feature still little used by physiotherapists in the intensive care units. Conclusions. The physical therapy in SCI in ICU is focused on motor rehabilitation through kinesiotherapy and intervention through the respiratory bronchial hygiene and training of respiratory muscles. New treatment modalities such as electrotherapy, there have been in intensive environment, there is a need for more studies to confirm benefits and risks of this feature in the spinal cord.
