Цель. Оценить клинические и гемодинамические параллели у детей дошколь-ного возраста с аритмиями в различных возрастных группах. Материал и методы. Обследовано 195 детей с идиопатическими аритмиями в возрасте от 0 до 7 лет -82 пациента с синдромом WPW, 55 детей с пред-сердными тахикардиями, 7 детей с АВУРТ, 13 -с желудочковыми тахикарди-ями (ЖТ), 38 детей с желудочковыми и наджелудочковыми экстрасистолиями (изолированной, групповой и их сочетанием). Возрастные группы: 1 -дети до 1 года (n=72; 37%); 2 -дети от 1 до 3 лет (n=37; 19%); 3 -дети от 3 до 7 лет (n=87; 44%). Всем пациентам проводилось общеклиническое обследование, электрокардиография в 12 стандартных отведениях, Холтеровское монитори-рование, эхокардиография. Результаты. При анализе вариантов течения тахикардий выявлено прео-бладание пароксизмального течения тахикардии у детей от 3 до 7 лет (p=0,001) и постоянного течения тахикардии у детей до 1 года (p<0,001). Частота приступов при пароксизмальных тахикардиях у детей до 1 года была выше по сравнению с детьми от 1 до 3 лет (p=0,028) и детьми от 3 до 7 лет (p<0,001). При оценке показателей ЭхоКГ у детей с аритмиями в зави-симости от возраста выявлено значимое преобладание признаков АКМП у старших детей по отношению к младшим (p=0,002). Проявления сердеч-ной недостаточности (СН) в большей мере оказались свойственны детям до 1 года по сравнению с детьми от 1 до 3 лет и от 3 до 7 лет (F=44,117; p=<0,001). Заключение. Таким образом, аритмогенное ремоделирование сердца в большей степени свойственно детям в возрасте от 3 до 7 лет. У пациентов с аритмиями в возрасте до 1 года клинические проявления СН сопровожда-ются диастолическими нарушениями, предшествующими развитию АКМП, которую традиционно принято считать систолической дисфункцией. К факто-рам, влияющим на данные гемодинамические закономерности, относятся высокая среднесуточная ЧСС, склонность тахикардий к постоянному и непре-рывно-рецидивирующему течению, большая частота приступов при пароксиз-мальных тахикардиях у детей до 1 года. Conclusion.So the arrhythmogenic heart remodeling mostly common for the children of 3 to 7 y. o. In less 1 year infants with arrhythmias clinical signs of HF are followed by diastolic disorders that develop before ACMP development, that is traditionally regarded as systolic dysfunction. Into the factors that influence these hemodynamic relations we include high mean heartrate, tendency of tachicardias to recurrent and permanent course, high frequency of attacks in paroxysmal tachicardias in infants before 1 year old. Objective: to evaluate cost-effectiveness and budget impact of using single and dual chamber implantable cardioverter-defibrillators (ICD) adjunctive to the standard drug therapy (DT) compared to the standard DT alone for the primary and secondary prevention of sudden cardiac death (SCD).Material and methods. Original partitioned survival analysis model was developed to assess the cost-effectiveness of using ICD within the modelling horizon of 8 years. The following model outcomes were used: life years and quality-adjusted life years (QALY). Primary prevention model was focused on patients after myocardial infarction with left ventricular ejection fraction (LVEF) ≤30%, whilst secondary prevention model considered cardiac arrest survivors and/or patients diagnosed with ventricular tachycardia or ventricular fibrillation with LVEF ≤35%. The model summarizes treatment effect and costs for ICD and DT specific to the healthcare system of the Russian Federation (RF). The main scenario accounted for ICD implantation cost in accordance with general reimbursement price asserted in the high technology medical care list part 2 (HТMC 2). Additionally, alternative scenario of ICD reimbursement level was developed to account for general tariff split onto singleand dual-chamber ICD implantation reimbursement tariffs which can be financed through high technology medical care list part 1 (HТMC 1). Budget impact analysis compared the costs of using ICD within the current volume of the annual increase in ICD implantations and a threefold increased volume of ICD implantations.Results. By the end of the modelling period, additional 34% of patients survived in the ICD group compared to the DT group. Incremental cost-effectiveness ratio (ICER) per 1 QALY constituted 2.8 and 2.2 million rubles for primary and secondary prevention, respectively. ICER values are slightly above or lower than the willingness-to-pay threshold of 2.5 million rubles per 1 QALY in the RF in the segment of primary and secondary SCD prevention, respectively. Additional HТMC 1 scenario incorporating lower ICD implantation prices resulted in an average ICER drop by 13% compared to HTMC 2. Overall patient population requiring SCD prevention comprised of 7,161 and 3,341 patients in primary and secondary prevention, respectively. Budget impact analysis showed that threefold rise in the ICD implantations rate will require additional 648 million rubles for primary prevention cohort to provide additional 573 patients with ICD, and 230 million rubles for secondary prevention cohort with additional 267 patients covered with ICD. ICD reimbursement price drop within the HТMC 1 scenario will save 133 million rubles and allow to provide additional 143 patients with ICDs for a given budget.Conclusion. ICD is a cost-effective option of secondary prevention of SCD. Additional analysis of ICD reimbursement price drop drives ICER downwards to a considerable extent which in turn increases the accessibility of ICDs to patients. In scenario of ICD implantation financing within HТMC 1, ICD is established to be a cost-effective option for primary and secondary prevention of SCD in the RF.

Цель. Определить значение сократительной функции миокарда правого желу-дочка (ПЖ) для оценки проводимой сердечной ресинхронизирующей терапии (СРТ). Материал и методы. В исследование были включены 80 пациентов с диагно-зом дилатационная кардиомиопатия (49 мужчин и 31 женщина, средний воз-раст 54±10,5 лет), сердечная недостаточность (СН) III функционального класса (ФК) по NYHA, фракция выброса (ФВ) левого желудочка (ЛЖ) составила 30,1±3,8%, дистанция 6-минутной ходьбы -290,5±64,3 м, конечно-диастоли-ческий объем (КДО) -220,7±50,9 мл. Устойчивый синусовый ритм имел место у 45 пациентов, хроническая медикаментозно резистентная фибрилляция предсердий -у 35. У больных были зарегистрированы нарушения внутриже-лудочковой проводимости в виде блокады левой ножки пучка Гиса, ширина комплекса QRS варьировала от 146 мс до 240 мс (183±32 мс). Имплантацию кардиоресинхронизирующего устройства проводили по стандартной мето-дике для бивентрикулярной электрокардиостимуляции. Пациентам с постоян-ной формой фибрилляции предсердий, вторым этапом формировалась пол-ная искусственная атриовентрикулярная блокада. У всех пациентов при помощи радионуклидной равновесной томовентрикулографии (РТВГ) была изучена сократительная функция миокарда левого и правого желудочков до проведения СРТ и через 12 месяцев после имплантации. Результаты. Контрольное обследование было проведено через 1 год, наблю-далась положительная клиническая динамика: ФК СН уменьшился с III до II. Клиническими респондерами СРТ оказались 69 пациентов (86,25%), не отве-тили на проводимую терапию 11 больных (13,75%). Критерием "ответа" паци-ента на СРТ мы считали прирост ФВ ЛЖ на 15% и более в течение 12 месяцев. Среди респондеров наблюдалась положительная клиническая динамика: уве-личилась ФВ ЛЖ с 30,1±3,8% до 42,8±4,8% (p≤0,001), уменьшился КДО ЛЖ с 220,7±50,9 до 197,9±47,8 мл (p≤0,005), у нереспондеров практически не изменилась ФВ ЛЖ с 30,1±3,8% до 33,8±3,8% (p≤0,001), увеличился КДО ЛЖ с 220,7±50,9 до 227,8±27,8 мл (p≤0,001). Все обследованные пациенты ретроспективно были разделены на две группы: в первую вошли пациенты, ответившие на СРТ, во вторую нереспондеры. Выявлено, с помощью радиону-клидной томовентрикулографии, изменение в течение 12 месяцев сократи-тельной функции правого и левого желудочков (ПЖ и ЛЖ). Так, в частности, максимальная скорость наполнения (МСН) и средняя скорость наполнения за 1/3 диастолы (ССН/3) левого и правого желудочков оказались достоверно хуже у больных второй группы на 30% и 60%, соответственно. Другие показа-тели у пациентов первой и второй групп достоверно не различались. Заключение. Таким образом, представленные результаты дают основание говорить о том, что улучшение сократительной функции ПЖ, на фоне СРТ, может оказывать положительное влияние на СРТ наравне с улучшением сократительной функции ЛЖ. Ресинхронизирующая терапия у больных с тяже-лой хронической сердечной недостаточностью (ХСН), на фоне сохраненной сократимости правых отделов сердца, более эффективна, при этом более высокие значения сцинтиграфических показателей МСН и ССН/3 ПЖ...

Aim.Defibrillation lead implantation technique optimization using cardiac scintigraphy in patients with coronary artery disease (CAD).Material and methods.In this study 81 patients (male — 72, age 64,0±8,7 years with CAD and indications for the cardioverter-defibrillator (ICD) implantation were examined. Patients were divided into two groups. In 1-st group before ICD implantation, patients underwent cardiac 99mTc-methoxy-isobutyl-isonitrile scintigraphy for right ventricle wall perfusion disorders assessment. In this group defibrillating lead was implanted to the septal position, if the perfusion disorders were in the apical segments, and to the apical position, if perfusion disorders were in the septal segment. In 2-nd group lead was implanted using conventional approach. Defibrillating lead parameters (threshold, sense, impedance and shock impedance on 1-st, 7-th, 30-th and 180-th follow-up days) were compared.Results.The 1-st group consisted of 45 (55,5%) patients (male — 41, age 62,2±8,8 years). In 28 (62,2%) cases in this group defibrillating lead was implanted to the apical and in 17 (37,8%) — to the septal position. The 2-nd group consisted of 36 (44,5%) patients (male — 31, age 66,3±8,2 years; p=0,03). In 20 (55,5%) cases in this group defibrillating lead was implanted to the septal and in 16 (44,5%) — to the apical position. There were significant differences between groups in terms of: threshold and sense at all follow-up days (p=0,0001) and impedance on 1-st (p=0,003), 30-th (p=0,0001) and 180-th day (p=0,002), respectively.Conclusion.Assessment of right ventricular perfusion before ICD implantation can reduce threshold and raise sense, thereby prolong the ICD longevity and improve life-threatening ventricular tachyarrhythmia detection in the early and long-term period in patients with CAD.
