П роблема серцево-судинної патології вже давно знаходиться в центрі ува-ги клініцистів та дослідників, і актуальність її не знижується. Незважаючи на великі успіхи в профілактиці та лікуванні цієї групи захворювань, низка невирішених питань патогенезу заважає раціональному застосуван-ню засобів профілактики та патогенетичної терапії.Так, існують доволі складні відношення між ліпідним та жирнокислотним складом їжі, крові, тканин і розвитком, наприклад, атеро-склерозу та артеріальної гіпертензії [1].Жирні кислоти (ЖК) відіграють важли-ву роль у перебігу різноманітних процесів у клітинах. Наприклад, насичені жирні кислоти (нЖК) є основним енергетичним субстратом для кардіоміоцитів. Завдяки своїй здатності підвищувати ненасиченість ацильних ланцюгів фосфоліпідів, знижувати мікров'язкість плаз-матичних мембран клітин та підвищувати їхню текучість ненасичені жирні кисло-ти (ннЖК) є важливим фактором регулю-вання проникності мембран, нормального функціонування мембранозв'язаних протеїнів. У всьому світі: майже 6 млн осіб щорічно звертаються по медичну допомогу з приводу опікових ран. На сьогоднішній день основним методом місцевого лікування ран та опіків, в тому числі інфікованих, є використання лікарських засобів, що містять антисептики, антибіотики, сорбенти, а також їх сполучення. Мета. Визначити вплив композиту нанодисперсного кремнезему з полігексаметиленгуанидіну гідрохлоридом на імунологічні показники та показники окислювально-антиоксидантного гомеостазу у крові та осередку ураження щурів з неінфікованим термічним опіком. Матеріали і методи. Дослідження проводилося на 72 щурах, яким моделювали термічний опік ІІ ступеня. Тварини були розподілені на 4 групи: інтактні, контроль, ліковані хлоргексидином, ліковані композитом нанодисперсного кремнезему з полігексаметиленгуанидіну гідрохлоридом. На 7-у, 14-у, 21-у та 28-у добу тварин виводили з експерименту та визначали імунологічні показники та показники окислювальноантиоксидантного гомеостазу у крові та шкірі в осередку ураження. Результати. Термічна травма безпосередньо пов'язана з біохімічними змінами в організмі. Нами порівнювалися рівні проти-та прозапальних цитокінів, маркерів вільнорадикального окислення-дієнового коньюгату (ДК), активних продуктів тіобарбітурової кислоти (ТБК-АП), активність каталази (Кат) та супероксидисмутази (СОД). Було доведено, що композит НДК+ПГМГ-ГХ проявляє значно більш виражений, порівняно з хлоргексидином, фармакологічний ефект щодо зниження рівня прозапальних цитокінів IL-1β і TNF-α та збільшення вмісту IL-10 у крові та вогнищі термічного опіку протягом усього періоду спостереження. Також композит ефективно впливає на окиснювально-антиоксидантний гомеостаз, що виявляється нормалізацією вмісту ДК і ТБК-АП на 14-ту добу, відновленням активності Кат на 14-ту добу, СОД-вже на 7-му добу, порівняно з хлоргексидином. In this review article an analysis of literature data on the pharmacological and clinical study of a fixed combination of medicinal substances (artichoke leaf extract, ursodeoxycholic acid, taurine, and Angelica sinensis roots extract), as well a scientific substantiation of the pharmaceutical quality profile of the corresponding finished solid dosage form has been conducted. Chronic diseases of the hepatobiliary system are some of the most common human diseases and inferior only to atherosclerosis . The fact that cholecystectomy is the most common surgical operation in the abdominal organs is evidenced by the widespread distribution of the pathology of the biliary system. The fact that there is an increasing number of diagnoses of cholelithiasis in children and infants is a cause for concern. Diseases of the biliary system are closely related to violations of the functional state of the liver. Synthesis of cholesterol supplemented bile with reduced bile acid content significantly increases the risk of gallstones, as well as gallbladder cholesterol. We have substantiated that the developed preparation is a fixed combination of medicinal substances with well-researched medical applications in the treatment of dyspeptic disorders with functional disorders of the biliary system, biliary dyskinesia of the hypokinetic type, and gastritis with reflux of bile. Each of the components of the fixed combination has an important influence on the human hepatobiliary system. The effect of ursodeoxycholic acid is due to the relative replacement of lipophilic toxic bile acids, improving the secretory capacity of hepatocytes and immunoregulatory processes, which is especially important in liver and cholestatic diseases. Taurine is a synergist of ursodeoxycholic acid, since it forms biliary conjugates in the liver. The artichoke extract has choleretic, hepatoprotective and diuretic effects, while the A. sinensis roots extract demonstrates moderate spasmolytic and anti-inflammatory properties. We conducted a general description of active pharmaceutical ingredients and a review of biopharmaceutical tests, analyzed relevant pharmacokinetic and pharmacodynamic studies, and summarized the results of clinical efficacy and safety trials. Particular attention is paid to the results of clinical studies of the developed fixed combination. It should be noted that artichoke leaf extract, ursodeoxycholic acid, and taurine are widely used throughout the world in official medicine, at the same time, A. sinensis roots extract is more widely used in traditional Chinese medicine. The fixed combination has a favorable safety profile, is investigated in clinical conditions in vivo both in the form of individual components and in the form of a single drug. A fixed combination pharmaceutical profile is based on the general requirements for solid dosage forms, as well as experimentally substantiated specific indicators and research methods.
The influence of selenium derivatives selenoline, cysteine selenate, and methionine selenate on the main indicators of HSP70-dependent mechanisms of endogenous neuroprotection under conditions of acute cerebrovascular accident (CVA) modeling has been investigated, and pathogenetic substantiation of prospects for their further study as potential neuroprotectors has been given. The experiments have been performed on white Wistar rats by bilateral irreversible ligation of the common carotid artery. Cysteine selenite and methionine selenite at a dose of 30 μg/kg, and selenoline at a dose of 50 μg/kg were introduced intraperitoneally to the experimental animals once-daily for 4 days. Every day for 4 days, the severity of neurological reactions was assessed in points on a stroke-index scale, and the mortality rate in animals was recorded. In the process of biochemical studies of brain tissue, the state of mRNA expression, HSP70 level, hypoxia-induced factors (HIF) -HIF1α and HIF-3α were evaluated using the method of polymerase chain reaction (PCR) with realtime reverse transcription. While false-operated animals developed mild neurological disorders, the control group developed bilateral ptosis, circling movements, paresis and paralysis of the limbs. On the 4th day the mean score on Stroke-index P. McGraw neurological deficit assessment scale was 16.7 ± 0.52, and 40 % of animals survived. Relevance. Glucosamine hydrochloride, as a medicament, is introduced in various fields of medicine. Its frigidoid and cardioprotective properties have been determined, but its protective properties have not been determined yet. There is a need to substantiate the expediency of using glucosamine hydrochloride as a metabolic and metabolitotropic agent, as improving performance and accelerating recovery processes under extreme conditions and under physical stress. Objective. Investigate possible acto-protective properties of glucosamine hydrochloride in an experiment on rats at high-speed physical activity. Materials and methods. Experiments were performed on 35 white rats in males of the Wistar line, weighing 180-200 g. Rapid loading was created by running rats at Tredbani with a tape speed of 42 m / min and on the test "swimming with additional load to full exhaustion". Glucosamine hydrochloride was administered intragastrically at a dose of 50 mg / kg using a probe for 10 days. Under the thiopental-sodium anesthesia, decapitations were performed, myocardial, liver, and skeletal muscle were removed, in which the content of the components of prooxidant-antioxidant homeostasis (level of LO-diene conjugates (DK), secondary malonic dialdehydes (MDA), superoxide dismutase enzymes activity (SOD), catalase). Results. Introduction of glucosamine hydrochloride lengthened swimming time in rats by 59% and running time by 52%, which indicates the adaptive and act-protective effect of the agent. During a high-speed run, without the use of an actoprotector, the level of lipid peroxidation (DC, MDA) increases and the activity of antioxidant protection enzymes (SOD, catalase) decreases. However, when using glucosamine hydrochloride, the level of lipid peroxidation (DC, MDA) decreases and the level of antioxidant protection (SOD, catalase) increases, which proves its act-protective properties. So, glucosamine hydrochloride prevents a change in the indices of prooxidant-antioxidant homeostasis, which is explained by the presence of a membranotropic and antioxidant effect in it. Conclusions. Glucosamine hydrochloride has its protective properties. The implementation of the actoprotective action may be mediated due to antioxidant effects.
