Summary The critical pathway of deceased donation provides a systematic approach to the organ donation process, considering both donation after cardiac death than donation after brain death. The pathway provides a tool for assessing the potential of deceased donation and for the prospective identification and referral of possible deceased donors.
Half an hour after reperfusion, the kidney, transplanted to the infant from an adult brain dead standard criteria donor, became flabby and acquired blue color. Hyperacute rejection was suspected as a consequence of false negative cross match, and eculizumab was administered with the purpose to treat antibody-mediated injury, with fast and clear effect. The patient's blood was tested for donor-specific antibodies on the next day, and results were negative. We attribute graft damage to reperfusion injury and explain eculizumab's effectiveness to its ability to prevent progression of reperfusion injury.

Оценка эффективности применения периферической вено-артериальной экстракорпоральной мембранной оксигенации в качестве средства предтрансплантационной механической поддержки кровообращения у пациентов, нуждающихся в неотложной пересадке сердца. Материалы и методы. 17 реципиентам (14 мужчин и 3 женщины) в возрасте 16-66 (40,1 ± 4,2) лет выполнена двухэтапная трансплантация сердца с использованием периферической вено-артериальной экстракорпоральной мембранной оксигенации в качестве метода предтрансплантационной механической поддержки кровообращения. Во всех случаях использовали канюляцию открытым хирургическим способом из бедренного доступа. Для забора венозной крови использовали венозную канюлю (21-25 Fr), для возврата кровиартериальную канюлю (15-19 Fr), для селективной перфузии нижней конечности использовали отдельную артериальную канюлю или однопросветный сосудистый катетер размером 8 или 10 F. Результаты. Продолжительность экстракорпоральной мембранной оксигенации (ЭКМО) перед трансплантацией сердца составила 81 ± 17 ч. Обеспечивали объемную скорость экстракорпорального кровотока 4,8 ± 0,6 л/мин, или 2,63 ± 0,04 л/мин/м 2 , газоток 4,8 ± 0,6 л/мин, FiO 2 0,86 ± 0,07. У 13 пациентов (76,5%) применение ВА ЭКМО было продолжено в послеоперационном периоде в течение 4,3 ± 0,5 суток, на 6,7 ± 0,8 сутки они были переведены из ОРИТ и в последующем выписаны из стационара на 32,3 ± 4,6 сутки после трансплантации сердца. 4 (23,5%) пациента умерли, в том числе 3на фоне послеоперационного применения ВА ЭКМО. Причинами летального исхода в 3 случаях были сепсис и полиорганная недостаточность, в одном случае внезапная остановка сердечной деятельности. Заключение. Периферическая вено-артериальная экстракорпоральная мембранная оксигенация является перспективным методом предтрансплантационной механической поддержки кровообращения у реципиентов с жизнеугрожающей декомпенсацией сердечной деятельности.
Worldwide, there is a trend towards an increase in the number of patients waiting for liver transplantation, despite an increase in the total number of liver transplantation operations. Solving the problem of donor organ shortage is possible through the use of organs from marginal donors: organs removed after cardiac arrest, organs with a high percentage of steatosis, organs from donors over the age of 60 years. The main reason for refusing to use them is the risk of severe complications and an unfavorable outcome of the operation. Static cold preservation today is the main method of protecting donor organs from thermal damage, which possesses rather effective protective properties. At the same time, the duration of cold preservation has a limited time interval. There is always uncertainty about the viability of the organ. Modern methods for assessing donor organs such as donor history, laboratory data, visual examination and morphology, do not reliably predict liver function after transplantation. In this regard, the development of methods for preserving the organ after removing, which do not degrade the quality of the organ or even are capable of restoring the lost functions, is relevant. The machine perfusion of the liver is one of the new concepts aimed at solving this problem. The article highlights the international experience of using the machine perfusion of the donor liver over the past 15 years. Machine perfusion is a promising trend of transplantation development, which allows reducing the shortage of donor organs and improving their quality.

Цель исследования -создание модели оценки донорского сердца, основанной на ряде характеристик донора. Материалы и методы. В исследовании использованы данные 650 доноров со смертью головного мозга, у которых были выполнены эксплантации органов в период с 1 января 2012 года по 31 декабря 2017 года. В биноминальной логистической регрессии в качестве зависимой переменной использовался отказ от донорского сердца, в качестве факторных признаков использовались характеристики донора. В регрессионной модели для каждого донорского фактора определена величина ОШ, которая трансформирована в баллы, и сумма баллов каждого из донорских факторов, вошедших в модель, являлась оценкой донорского сердца. Предложенная модель валидизирована на выборке доноров за период с 1 января 2019 года по 31 декабря 2019 года, общим количеством 218. Результаты. В модель включены характеристики доноров -возраст, причина смерти (ЧМТ/ОНМК), ГБ и СД в анамнезе, остановка сердечной деятельности с последующим ее восстановлением, собственная патология и травматические повреждения сердца, а также показатели ЧСС, САД, лактата артериальной крови, потребности в норадреналине непосредственно перед изъятием органов. Основываясь на среднем значении суммы баллов, определены доноры низкого риска (ДНР ≤17 баллов) и доноры высокого риска (ДВР ≥18 баллов). В валидационном пуле доноров удельный вес отказов от сердца среди ДНР составил 4,1% и среди ДВР -78,6%, р < 0,0001, χ 2 Пирсона -130,9. Заключение. Представленная модель по оценке донорского сердца достаточно точно отражает вероятность использования сердца донора для трансплантации и создает условия для оптимального распределения сердечных трансплантатов, особенно от доноров высокого риска.Ключевые слова: модель оценки донорского сердца, донорские факторы риска трансплантации сердца.
The objective of the study was to investigate the evolution and trends in liver donation in the city of Moscow, with special focus on the expansion of liver donor selection criteria for transplantation.Materials and methods. The study included 1,548 effective donors (EDs) in the period from January 1, 2012 to December 31, 2020. Their basic characteristics - age and cause of death - are presented. The dynamics of changes in the age groups of donors and the dynamics of the number of >60-year-old liver donors were studied. The influence of expanded liver donor selection criteria over the dynamics of the number of transplant surgeries and patient flow on the waiting list was assessed.Results. During the study period, the number of effective liver donors (ELDs) in Moscow increased 4.7-fold. The average age of ELDs increased from 37.1 in 2012 to 48.8 in 2020. There was an absolute prevalence of donors who died from cerebrovascular accident compared with donors who died from traumatic causes, 83.4% vs 16.6%, respectively. Since 2016, there has been a progressive increase in >60-year-old liver donors; the number of such donors in 2020 reached 39, accounting for 13.6% of the total pool of EDs. The progressive growth in the number of liver transplants has significantly influenced patient movement on the waiting list. In 2012, there was a 25.2% increase in the number of liver transplants per 100 patients on the waiting list; by 2020, it had reached 86.6%.Conclusion. The results reasonably indicate an increase in liver donation and liver transplantation (LTx) in Moscow. Comparison of Russian data with those of leading foreign donor programs shows that the trends in the donor pool in the context of older age, including >60-year-old donors, and shifting causes of donor death towards cerebrovascular diseases are similar. An overall increase in donor activity and expansion of liver donor criteria contributed to an increase in the number of transplants performed per 100 patients on the waiting list, which, in turn, reduced the waiting time for a donor organ and increased the intensity of patient flow on the waiting list.
Orthotopic liver transplantation is the only way to cure chronic liver failure and certain liver tumors. Deceased donor organs are the prevailing source in most transplantation centers. However, there is an organ shortage because of the increasing number of patients in need of transplantation. Using expanded criteria deceased donors is a routine way to overcome organ shortage. The majority of transplantation centers take older donors depending on the local rules. Successful liver transplantation from 73-year-old deceased donor. Recipient suffered from liver neuroendocrine tumor without signs of extrahepatic spread. Liver function is adequate during 90-day follow-up. Liver transplantation from older deceased donors has good outcome according to world experience. Careful donor-recipient selection is the key to success. The first positive trial in our center enlightens the way for further practice.
