Введение. В последние годы отмечается рост заболеваний печени у детей, среди которых одно из ведущих мест занимает хронический вирусный гепатит С (ХГС). Однако до настоящего времени окончательно не изучено воздействие вируса на организм ребенка, а именно, влияние вируса на гемостазиологический гомеостаз. Цель исследования: изучить нарушения системы гемостаза и оценить их клиникопатогенетическое значение у детей с ХГС. Материалы и методы. В исследование включен 31 пациент с ХГС в возрасте от 3 до 17 лет 17 (55) девочек и 14 (45) мальчиков. Контрольную группу составили 15 детей, относящихся к I или II группам здоровьям. У всех участников исследования оценивали состояние эндотелия, системы гемокоагуляции, эндогенных антикоагулянтов и фибринолиза, агрегационную активность тромбоцитов. Результаты. У детей с ХГС в первые 10 лет заболевания, несмотря на высокий уровень виремии, отсутствовали признаки тяжелого поражения печени. Изменения в системе гемостаза проявлялись повышением агрегационной активности тромбоцитов, фактора фон Виллебранда, содержания эндотелина1 и гомоцистеина, активности антитромбина III и ингибито ра активатора плазминогена. Увеличение вирусной нагрузки при ХГС у детей сопровождалось нарушением коагуляционного гемостаза. Длительная персистенция вируса в организме ребенка сопровождалась увеличением плотности печени по данным непрямой эластометрии. Заключение. ХГС у детей в первые 10 лет заболевания характеризуется относительно мягким течением. Независимо от генотипа вируса выявляются нарушения, затрагивающие сосудистотромбоцитарное, антикоагулянтное и фибринолитические звенья системы гемостаза, которые коррелируют с тяжестью патологического процесса. Introduction. In recent years the increasing incidence of hepatic diseases is noted in children and chronic viral hepatitis C (CHC) occupies among them one of the leading places. However, until now the impact of this virus on the childs organism has not been properly studied yet, in particular the virus effect on hemostasiological homeostasis. Aim: to study haemostatic disorders and to assess their clinical and pathogenetic significance in children suffering from CHC. Materials and methods. The study included 31 patients with CHC aged from 3 to 17 years 17 (55) girls and 14 (45) boys. The control group consisted of 15 children with their health category I and II. The state of endothelium, haemocoagulation system, endogenous anticoagulants and fibrinolysis and the platelet aggregation activity were assessed in all patients. The increase in viral load in children with CHC was accompanied by disturbance of coagulation hemostasis. Prolonged persistence of the virus in the childs body was accompanied by an increase in liver density according to indirect elastometry. Results. In the first 10 years of the disease children with CHC had no symptoms of severe hepatic lesion in spite of high viremia level. Haemostatic changes were manifested by increased platelet aggregation activity, von Willebrand factor, endothelin1 and homocysteine levels, antitrombine III activity and plasminogen activator inhibitor. The increased viral load in children with CHC was followed by disturbance of coagulation hemostasis. Prolonged virus persistence in the childs body was accompanied by higher hepatic density according to indirect liver elastometry data. Conclusion. In the first 10 years of the disease CHC in children is characterized by a relatively mild course. Irrespective of viral genotype disorders are identified affecting vascularplatelet, anticoagulative and fibrinolytic links of hemostasis that correlate with the pathologic process severity.
Цель исследования: установить взаимосвязь нарушений системы гемостаза с выраженностью острофазного ответа при гнойном воспалении у больных с нейроишемической формой синдрома «диабетическая стопа» (СДС). Материалы и методы. Проведено изучение системы гемостаза, биохимических метаболитов крови, характеризующих состояние углеводного, белкового и липидного обменов, а также биохимического маркера воспаления — С-реактивного белка у 20 пациентов с нейроишемической формой СДС. Результаты. Установлена взаимосвязь нарушений системы гемостаза с выраженностью острофазного ответа, показателями липидного и белкового обменов у больных с СДС. Показано, что при нейроишемической форме СДС развиваются выраженные гемостазиологические и метаболические нарушения, которые имеют между собой высокую и среднюю степень сопряженности. Заключение. Гемостазиологические тесты могут использоваться в совокупности с показателями углеводного, белкового и липидного обменов для оценки тяжести патологического синдрома, назначения патогенетической терапии, оценки эффективности проводимого лечения. In 20 patients with neuroischemic form of DFS we studied hemostatic and biochemical blood parameters that characterize carbohydrate, protein and lipid metabolism, and biochemical marker of inflammation — C-reactive protein. Results. In patients with SDS we revealed the interrelations between hemostatic disorders and severity of acute phase response, lipid and protein metabolism. It was demonstrated that expressed hemostatic and metabolic disturbances with high and medium degree of conjugation progressed in patients with neuroischemic form of SDS. Conclusion. In conjunction with parameters of carbohydrate, protein and lipid metabolism hemostasiological tests can be used for assessment the severity of pathological syndrome, the appointment of pathogenetic therapy, the evaluation of treatment effectiveness. Введение. Аутоиммунный гепатит (АИГ) — хроническое воспалительное заболевание печени, характеризующееся агрессивным течением. Поражение ткани печени влияет на процессы коагуляции, поскольку данный орган играет центральную роль в системе гемостаза. Цель исследования: изучить нарушения системы гемостаза при развитии фиброза и цирроза печени у детей с АИГ. Материалы и методы. В исследование включено 29 пациентов в возрасте от 3 до 17 лет: 16 (55%) девочек и 13 (45%) мальчиков. Первая группа — 13 детей со степенью фиброза F 0–2 (по шкале Metavir); вторая группа — 16 пациентов со степенью фиброза F 3–4 (по шкале Metavir), у детей этой группы был диагностирован цирроз печени. Контрольную группу составили 15 детей, относившихся к I или II группам здоровьям. У всех детей оценивали состояние эндотелия, системы гемокоагуляции, эндогенных антикоагулянтов и фибринолиза. Результаты. У детей с АИГ и начальными стадиями фиброза печени нарушения системы гемостаза характеризовались повышением активности фактора фон Виллебранда (ФВ), содержания антитромбина III и фибриногена, удлинением значений протромбинового теста по Квику (ПТ) и активированного частичного тромбопластинового времени (АЧТВ). Развитие цирроза печени при АИГ сопровождалось ростом активности ФВ, снижением активности протеина С, удлинением ПТ и АЧТВ. Заключение. У детей с АИГ выявляются нарушения, затрагивающие эндотелиальное, коагуляционное и антикоагулянтное звенья системы гемостаза, которые коррелируют с тяжестью патологического процесса. Introduction. Autoimmune hepatitis (AIH) is a chronic infl ammatory liver disease that characterized by an aggressive course. Lesion of liver tissue aff ects on coagulation becouse this organ plays a central role in hemostasis. Aim: to study hemostatic disturbances in the development of fi brosis and cirrhosis in children with AIH. Materials and methods. The study included 29 patients (3–17 years old): 16 (55%) girls and 13 (45%) boys. The first group consisted of 13 children with F 0–2 degree of fi brosis (according to Metavir scale); the second group consisted of 16 patients with F 3–4 degree of fi brosis (according to Metavir scale), in this group of children liver cirrhosis was diagnosed. The control group consisted of 15 children related to I or II groups of health. The state of endothelium, hemocoagulation, endogenous anticoagulants and fi brinolysis were assessed in all children. Results. In children with AIH and early stages of liver fi brosis hemostasis disorders were characterized by increased activity of von Willebrand factor (VWF), increased levels of antithrombin III and fi brinogen, prolongation of prothrombin Quick test (PT) and activated partial thromboplastin time (APTT). The development of liver cirrhosis in AIG was accompanied by increased activity of VWF, decreased activity of protein C, prolongation of PT and APTT. Conclusion. In children with AIG disorders are detected aff ecting the endothelial, coagulation and anticoagulant hemostasis that correlated with the severity of pathological process.
