Введение. Работа посвящена исследованию функционального состояния миокарда у детей на фоне синдрома WPW.Цель. Установить морфофункциональные признаки и причины, способствующие формированию дисфункции миокарда у детей с синдромом WPW.Материалы и методы. Данная работа выполнена среди детей, имеющих синдром WPW (108 детей), феномен WPW (48 детей), и у детей без признаков синдрома/феномена (38 детей). Всем детям проводились традиционная эхокардиография и ЭКГ. Полученные эхокардиографические показатели были стандартизованы с учетом площади поверхности тела (Pettersen M.D. et al., 2008). Выделено 4 типа синдрома WPW. В группе детей с синдромом WPW проведено эндокардиальное картирование с определением локализации дополнительных проводящих путей.Результаты. У детей, имеющих синдром WPW, установлено увеличение толщины межжелудочковой перегородки в обе фазы сердечного цикла, но более выраженное в фазу диастолы (р<0,001), утолщение задней стенки левого желудочка в фазу диастолы (р=0,003). Такие изменения размерных параметров нами выявлены у 27,5–35,8% детей с синдромом. Они носили компенсированный характер и не проявлялись клинически признаками недостаточности кровообращения. Манифестирующий тип синдрома WPW был преобладающим (43,5%) у детей с синдромом WPW и сопровождался изменениями диастолической функции. Скрытый тип синдрома был вторым по частоте (36,6%), характеризовался минимальными изменениями. Наиболее часто в изучаемой нами группе детей с синдромом WPW встречались левосторонние дополнительные атриовентрикулярные соединения (ДАВС) (52,5%; р<0,001). Для них было характерно более частое в 1,6 раза (χ2=5,58; р=0,02) наличие пролапса митрального клапана. Полученные нами результаты указывают на наличие ранних инструментальных признаков дисфункции миокарда среди детей с приступами ПТ при синдроме WPW.Выводы. Дисфункция миокарда у детей, имеющих синдром WPW, выявляется посредством ультразвукового исследования у 12,2–37,4% обследованных. Она сопровождается изменением диастолических размеров левого желудочка, отсутствием снижения его сократительной функции. Выявлена взаимосвязь между анатомическим расположением ДАВС и размерами сердечных структур. Отмечена высокая стигматизация по синдрому дисплазии соединительной ткани у детей с синдромом WPW – до 44,3% (р=0,002). Introduction. The work is devoted to the study of the functional state of the myocardium in children with WPW syndrome.Purpose. To reveal the morphological and functional signs and causes that contribute to the formation of myocardial dysfunction in children with WPW syndrome.Materials and methods. This work was performed in children with WPW syndrome (108 children), WPW phenomenon (48 children), and in children without the signs of the syndrome / phenomenon (38 children). All children underwent traditional echocardiography and ECG. The resulting echocardiographic parameters were standardized, taking into account the surface area of the body (Pettersen M.D. et al., 2008). There are 4 types of WPW syndrome. In the group of children with WPW syndrome, endocardial mapping was performed to determine the localization of additional pathways.Results. In children with WPW syndrome, the increase of the thickness of the interventricular septum in both phases of the cardiac cycle, but more pronounced in the diastole phase (p<0.001), thickening of the posterior wall of the left ventricle in the diastole phase (p=0.003) were found. Changes in diastolic function were compensated. In general, we detected the changes in diastolic function in 27.5–35.8% of children with the syndrome. They were of a compensated nature and did not show the clinical signs of circulatory insufficiency. The manifesting type of WPW syndrome was predominant (43.5%) in children with WPW syndrome, and it was accompanied by the changes in diastolic function. The latent type of the syndrome was the second in frequency (36.6%), characterized by minimal changes. The most frequently studied group of children with WPW syndrome was the left-sided additional atrio-ventricular connections (52.5%; p<0.001). They were characterized by a 1.6-fold more frequent occurrence (χ2=5.58; p=0.02) of mitral valve prolapse. Our results indicate the presence of early instrumental signs of arrhythmogenic dysfunction in children with PT attacks in WPW syndrome.Conclusions. Myocardial dysfunction in children with WPW syndrome is detected with the help of ultrasound in 12.2–37.4% of the examined patients. It is accompanied by the change in diastolic parameters of the left ventricle, without changes in its contractile function. The relationship between the anatomical location of additional atrio-ventricular connections and the size of the heart structures was revealed. High stigmatization of connective tissue dysplasia syndrome in children with WPW syndrome was noted – up to 44.3% (p=0.002).
Цель. Оценить функциональное состояние проводящей системы сердца и выявить причины, способствующие изменению его состояния, у детей с синдромом WPW.Материалы и методы. В исследовании участвовали дети, имеющие синдром WPW (108 детей), феномен WPW (48 детей), и дети без признаков синдрома/феномена (38 детей). Всем детям выполнено холтеровское мониторирование ЭКГ, были проанализированы показатели суточной вариабельности ритма сердца. В группе детей с синдромом WPW проведено чреспищеводное электрофизиологическое исследование и эндокардиальное картирование с определением локализации дополнительных проводящих атриовентрикулярных соединений (ДАВС). Функциональное состояние проводящей системы определяли на основании оценки функционирования синусового узла и атриовентрикулярного соединения, ДАВС. Результаты. У 43% детей с синдромом WPW выявлено усиление парасимпатических влияний (р=0,010). Вегетативные расстройства отмечались вне приступов пароксизмальной тахикардии почти у каждого пятого ребенка (21,3%). При манифестирующем типе отмечена избыточная активация парасимпатического отдела ВНС преимущественно в ночное время. По сравнению с феноменом при синдроме WPW отмечается некоторое замедление проведения (р=0,03) через узел на фоне улучшения проведения через ДАВС (р=0,04). В то же время проводимость по ДС при феномене значительно хуже (р<0,010), чем через АВ узел. Анатомическое расположение ДС оказывало влияние на длительность эффективного рефрактерного периода, который оказался максимально удлинен при септальном расположении (р<0,001) и был минимален при левосторонней локализации (р<0,001).Выводы. Клиническое течение синдрома WPW определяется вегетативным балансом. Наиболее вегетативно нестабильным типом синдрома WPW является интермиттирующий. Различия электрофизиологического состояния проводящей системы сердца у детей с синдромом WPW обусловлены укорочением рефрактерного периода во всей проводящей системе и замедлением проведения через АВ узел. Более выраженные отличия в состоянии проводящей системы сердца выявлены при сравнении манифестного и скрытого типов синдрома. Purpose. This work was conducted among children with WPW syndrome (108 children), the WPW phenomenon (48 children), and in children without signs of the syndrome / phenomenon (38 children). All children underwent Holter ECG monitoring; the daily heart rate variability was analyzed. In the group of children with WPW syndrome, the transesophageal electrophysiological study and endocardial mapping were performed to determine the localization of additional conductive atrioventricular compounds (DIC). Assessment of the functional state of the conducting system was carried out on the base of the assessment of the functioning of the sinus node and atrioventricular connection, DIC.Results. Among 43% of children with WPW syndrome, the increase of parasympathetic influences was revealed (p=0.010). Autonomic disorders were noted outside of PT attacks in almost every fifth child (21.3%). In the manifesting type, the excessive activation of the parasympathetic division of the ANS was observed mainly at night. Compared with the phenomenon in WPW syndrome, there is a slight slowdown in the conduction (p=0.03) through the node on the background of the improvement of conduction through the DIC (p=0.04). At the same time, the conductivity along the DS with the phenomenon is much worse (p<0.010) than through the AV node. The anatomical location of the DS had the effect on the duration of the effective refractory period, which turned out to be maximally elongated at the septal location (p<0.001) and was minimal at the left-side localization (p<0.001).Conclusions. The clinical course of WPW syndrome is determined by the autonomic balance. The most vegetatively unstable type of WPW syndrome is intermittent one. The differences in the electrophysiological state of the cardiac conduction system in children with WPW syndrome are caused by the shortening of the refractory period in the entire conduction system and the slowing down of conduction through the AV node. More pronounced differences in the state of the cardiac conduction system were revealed when comparing the manifest and latent types of the syndrome.
Objective: to determine the prognostic factors determining the severity of Wolff-Parkinson-White syndrome (WPW) in children. Methods: a retrospective analysis of the course of the disease was performed in 108 children with WPW syndrome aged 13.6 (12-16) years. Depending on the number of attacks of paroxysmal tachycardia (PT), all children were divided into 2 groups: group A consisted of 47 children (43.5%) with a history of no more than three attacks of PT and they were rare; group B included 61 children (56.5%) with frequent (more than once a month) attacks of PT, more than four in the history. We studied: the family history, the clinical picture of the disease, the nature of paroxysmal tachycardia, the effectiveness of treatment, and the results of instrumental studies. Using the method of mathematical modeling, the informative value of each feature was determined, and the diagnostic value was determined. Results: age and gender differences in the development of WPW syndrome in children were established. The presence of a family history of ECG signs of ventricular preexcitation is a reliable sign (p=0.02), contributing to more frequent PT attacks. More frequent development of PT attacks was noted in patients with mitral valve prolapse (p=0.03) and additional chords in the left ventricular cavity (p=0.001). In the group with frequent PT attacks, signs of sinus node dysfunction were detected more often (<0.001). According to the echocardiographic study, children with frequent PT attacks were more likely to have disorders in the form of thickening of the interventricular septum in the diastole (p=0.010) and signs of diastolic dysfunction of the right ventricle (p = 0.010). Conclusion: The factors of prognostic value include: male gender, the presence of the expressed symptoms of the attack of PT and PT frequency more than once per month, low vagal efficiency on relieving the attack, a history of frequent attacks, a family history of ECG characteristics of premature ventricular excitation, the presence of signs of sinus node dysfunction during 24 hours without the attack and the signs of arrhythmogenic myocardial dysfunction by echocardiography.
