In patients who died from cardiovascular causes, mTWA, and TO values were significantly higher and TS values were significantly lower than in survivors, and the subgroup with SCD was characterized by significantly increased mTWA and TO values and decreased TS values. mTWA > 53.5 mcV at 100 b.p.m. was an independent significant predictor of SCD and increased risk of SCD by five-fold.

Patient management in clinical practice generally complies with the current guidelines; however, much importance is attached to the severity of arrhythmia (the number of VPBs per day, the presence of UVT) in addition to the presence of symptoms. In the opinion of most physicians, the initiation of treatment is justified when there are 10,000-15,000 and more per day. QOL assessment may be promising in choosing the optimal management tactics for these patients. Treatment should not be initiated immediately in patients with a high level of QOL, especially in those with arrhythmia lasting less than 12 months, by taking into account that there can be a spontaneous improvement in 38% of cases within the next month. The immediate results of ADT and RFA are comparable in patients with VA in the absence of structural heart disease. The Class IC antiarrhythmic drug ethacyzin is the most effective agent that ensures positive changes in arrhythmic syndrome in 66.7% of cases with the rate of side effects being in 17.8%.

Дилатационная кардиомиопатия (ДКМП) остается заболеванием с плохим прогнозом. Цель. Изучить актуальность улучшения стратификации риска фатальных событий, особенно внезапной сердечной смерти (ВСС), с целью своевремен-ного проведения профилактических мероприятий, поскольку ранее предло-женные неинвазивные электрофизиологические предикторы -турбулент-ность ритма сердца (ТРС) и вариабельность (ВРС), микровольтная альтерна-ция зубца Т (mTWA), мощность замедления (DC) и мощность ускорения ритма сердца (AC) при ДКМП изучены недостаточно. Материал и методы. В течение 4-х лет проводилось наблюдение за 54 паци-ентами с ДКМП и синусовым ритмом в возрасте 42 [30;58] лет (36 мужчин) и контрольной группой -54 человека без сердечно-сосудистой патологии (32 мужчины, средний возраст 47 [27;64] лет). Исходно проводили холтеровское мониторирование ЭКГ (ХМ) с оценкой ВРС, ТРС, DC, AC, mTWA и эхокардио-графию. Пациенты получали стандартную терапию хронической сердечной недостаточности (ХСН); частота имплантаций кадиовертеров-дефибриллято-ров (ИКД) составила 18,5%. Результаты. Средняя фракция выброса (ФВ) в основной группе составила 32 [22;38]%, признаки ХСН выявлены у 93% больных. Больные с ДКМП отлича-лись от контрольной группы достоверно более низкими значениями SDNN, pNN50, DC, TO, TS, более высокими значениями AC и mTWA в ранние утренние часы. В течение 4-х лет зарегистрирована 1 ВСС, от прогрессирования ХСН умерло 7 пациентов, наблюдался один адекватный шок у больного с ИКД (всего 9 фатальных событий). Пациенты с фатальными событиями по сравне-нию с выжившими имели более низкую ФВ, ВРС, DC, максимальные значения mTWA, большие конечный диастолический объем, класс ХСН, АС, mTWA, число эпизодов неустойчивой желудочковой тахикардии (нЖТ) в утренние часы. При однофакторном анализе достоверно увеличивали риск фатальных событий (в порядке убывания значимости): ФВ (отношение шансов (ОШ) 32), SDNN (ОШ 21), DC (ОШ 9), AC (OШ 7), pNN50 (ОШ 6), нЖТ (ОШ 5,2, p=0,05). При многофакторном анализе единственным независимым предиктором фаталь-ных событий явилось снижение ФВ левого желудочка менее 26% (чувстви-тельность 80%, специфичность 90%). Заключение. Больные с ДКМП, по сравнению с лицами без сердечно-сосудис той патологии, характеризуются снижением показателей ВРС и DC, повышением AC, более частым выявлением патологических значений ТРС, повышением mTWA в ранние утренние часы, причем у умерших пациентов эти отклонения более выражены. При однофакторном анализе неинвазивными электрофизиологическими предикторами, ассоциированными с плохим про-гнозом при ДКМП явились AC, SDNN, DC, pNN50, нЖТ. Однако наиболее зна-чимым и единственным независимым предиктором фатальных событий у больных с ДКМП является снижение ФВ. Использование в качестве критерия высокого риска значения ФВ менее 26% способно предсказывать увеличение риска фатальных событий в 32 раза с чувствительностью 80% и специфично-стью 90%. Aim. To assess the association between ω-3 index of erythrocytes and demographic, electrophysiological, and echocardiographic (EchoCG) predictors of sudden cardiac death (SCD) in patients with coronary heart disease (CHD) and ventricular arrhythmias (VA). Material and methods. The study included 25 patients with a verified diagnosis of CHD and VA. Gas chromatography method was used to measure the content (%) of eicosapentaenoic (EPA) and docosahexaenoic (DHA) polyunsaturated fatty acids (PUFA) in peripheral blood erythrocytes, with the calculation of a summary (EPA + DHA) ω-3 index. All participants underwent 24-hour electrocardiography (ECG) monitoring, with the assessment of maximal, minimal, and mean heart rate (HR), heart rate variability (HRV) parameters (SDNN and pNN50), heart rate turbulence (TO and TS), microvolt T wave alternans (mTWA), and the number of ventricular extrasystoles (VE) and transient and persistent ventricular tachycardia (VT) episodes. All patients also underwent EchoCG. Results. In examined patients, the values of ω-3 index of erythrocytes varied from 1,12% to 6,4% (mean 3,74%, 95% CI 2,02-4,38%). There was a weak correlation between ω-3 index or EPA levels (%) and the HRV parameter of pNN50. In addition, ω-3 index or DHA levels (%) negatively correlated with the daily VE number. The 5:00 AM value of mTWA (II lead, update factor 1/8) weakly correlated withω-3 index and DHA levels. There was a moderate positive correlation between E/A ratio and omega-3 index, or EPA and DHA levels. Conclusion. Patients with CHD and VA were characterised by low ω-3 index values and high (56%) or moderate (44%) levels of cardiovascular risk. The values of ω-3 index positively correlated with the daily VE number and negatively correlated with E/A ratio and pNN50 parameter of HRV.

The purpose of the study was to improve the risk prediction of SCD in post-MI patients by comparing the informative value of new non-invasive SCD risk stratification factors (HRT, mTWA, DC) with the well-known factors (impaired HRV, low left ventricular ejection fraction (LVEF), ventricular arrhythmias) both alone and in combination. Holter monitoring (HM) with evaluation of the above factors and echocardiography with LVEF measurement were performed in 111 patients (84 males and 27 females) aged 64.1 10.5 years who had MI from 2 months to 36 years (mean 27 [9; 84] months) prior to admission. The follow-up period was 12 months. The endpoints included SCD and overall cardiovascular mortality. During follow-up 15 cases of SCD and 8 other cardiovascular deaths (5 repeated fatal MI and 3 lethal strokes) were registered. LVEF was the most significant predictor of overall mortality followed by DC, HRT, HRV, mTWA and QRS width. LVEF turned out to be the most significant risk factor for SCD followed by HRT, QRS width, DC, number of PVCs per day and mTWA. Noninvasive electrophysiological predictors showed the maximum SCD predictive value in patients with LVEF >40%, whereas at lower LVEF their predictive value was either decreased or completely lost. Combined risk assessment revealed that combination of HRT2 and increased mTWA caused a significantly increased risk of cardiovascular death (OR 30.7 (95% CI, 3.5-271.6), p <0.001) and especially from SCD (OR 63.3 (95% CI, 6.8-585.8), p <0.001) compared to any other combination including those with reduced LVEF. Thus, the evaluation of HRT, DC and mTWA during HM enables to define the population of post-MI patients with high risks of cardiovascular mortality and SCD. These predictors are most effective in combination, as well as in patients with LVEF >40%. The combination of HRT2 and mTWA ₁₀₀ >53 mcV is associated with a maximum increased risk of cardiovascular death and SCD.
