BackgroundIn rheumatology, comorbid infections have significant impact on patients’ morbidity, mortality and quality of life. Prevention of infections is an integral part of supervision of these patients.ObjectivesThe aim of the study is to investigate immunogenicity and safety of 23-valent polysaccharide pneumococcal vaccine (PPV-23) in patients with rheumatoid arthritis (RA) in a five-years period.MethodsThe study included 79 RA patients with ≥2 recent episodes of respiratory tract infections (bronchitis, pneumonia). 52 RA pts were treated with methotrexate (MT), 14 – with leflunomide (Lef), 13– with TNF- α inhibitors + MT. One dose (0,5 ml) of PPV-23 was administered subcutaneously without discontinuing MT/Lef or 28-30 days prior to initiation of TNF- α inhibitors. Control visits were scheduled as follows: at baseline (Visit 1), and in 1, 3, and every year after immunization. 39 out of 110 pts were followed for 24 months, 23 pts – for 36 months, 23 pts – for 48 months, and 18 - for 60 months. Standard clinical examination and lab tests were performed at each visit.Levels of serum antibodies (AB) to Pneumococcal capsular polysaccharide were measured with VaccZymeTM PCP IgG 2 panels (The Binding Site Group Ltd, Birmingham, UK). Coefficient of post-immunization response (CPR) was determined for each patient as the ratio of AB levels at Visits II, III, IV, V, VI and VII to AB level at Visit I.Results53 (67%) patients did not have any reactions to the vaccine, 26 (33%) patients indicated pain, swelling and hyperemia of the skin (diameter up to 2 cm) at the injection site of the vaccine, low-grade fever. These typical reactions following vaccination are completely regressed within days without any additional treatment. They were not associated with RA therapy and did not require changes in ist schemes.Pronounced positive immune reaction to PPV-23 was documented in all RA patients on different therapeutic regimens as significant AB incremental growth. Significantly high level of post-immunization response still persisted after 4 and 5 years of follow-up.The proportion of responders to PPV-23 after 5 years reached 73% among RA patients. (Table).No case of clinically or radiographically confirmed pneumonia was documented during the follow up period, although one case of interstitial viral pneumonia was registered in 6 month after immunization.Not a single vaccination-related RA exacerbation episode or new autoimmune phenomenon was documented during the follow-up.Table. Objective: to investigate the immunogenicity, safety, and clinical efficacy of 23-valent polysaccharide pneumococcal vaccine in patients with rheumatoid arthritis (RA) during a two-year follow-up study.Subjects and methods. The prospective open-label comparative study enrolled 110 people, of them there were 81 (73.6%) women and 29 (26.4%) men at the age of 23 to 76 years, including 79 patients with RA, as well as 31 subjects without systemic inflammatory rheumatic diseases (RD) (a control group). The group of RA patients exhibited a predominance of middle-aged women who had > 3 years’ disease duration and a moderate inflammatory activity (the mean value of DAS28, 4.32). 52 patients received methotrexate (MTX), 14 had Leflunomide (LEF), and 13 were treated with tumor necrosis factor-α (TNF-α) inhibitors + MTX.The 23-valent polysaccharide pneumococcal vaccine Pneumo-23 (Sanofi Pasteur, France) was administered in a single dose of 0.5 ml subcutaneously during continuous MTX or LEF therapy for the underlying disease or 3–4 weeks before the use of TNF-α inhibitors. Clinical examinations of the patient and conventional clinical and laboratory studies were performed during control visits (1, 3, 12, and 24 months after vaccination). Clinical effectiveness and safety were evaluated in all the patients included in the study. The serum levels of anti-pneumococcal capsular polysaccharide antibodies (Ab) were measured in 72 patients with RA and in 30 individuals in the control group during a 12-month follow-up study, including in 25 patients with RA for a 24-month follow-up study by enzyme immunoassay using commercial VaccZymeTM Anti-PCP IgG Enzyme Immunoassay kits (The Binding Site Group Ltd, Birmingham, United Kingdom). Along with this, the post-immunization response coefficient was calculated for each patient as the ratio of postvaccination Ab levels during Visits 2, 3, 4, and 5 to the baseline Ab level.Results and discussion. No clinical and radiological symptoms of pneumonia were recorded in any case during the follow-up period. The patients with RA and the control group showed a more than double significant increase of anti-pneumococcal Ab level during 3 months following vaccination. Despite the decrease in their concentration by month 12, the latter remained at the appropriate level and significantly increased at 24-month follow-up. Vaccination was well tolerated. A favorable course of the postvaccinal period was noted in all cases. There were no adverse reactions to vaccination in 72 (65%) patients; 38 (35%) patients were noted to have pain, skin swelling and hyperemia up to 2 cm in diameter at the site of injection, as well as low-grade fever. There were no episodes of a RD exacerbation or any new autoimmune disorders during the follow-up period.Conclusion. The findings were suggestive of the sufficient immunogenicity and good tolerability of 23-valent pneumococcal vaccine in patients with RD during the two-year follow-up period.
BackgroundInterstitial lung disease (ILD) is the most common internal organ manifestation of idiopathic inflammatory myopathies (IIM) that can severely affect the course of the disease. IIM patients with ILD often show resistance to conventional treatment with glucocorticoids (GC) and cytotoxic drugs so addition of biologic agents is an interesting possibility.ObjectivesTo assess efficacy and tolerability of rituximab (RTM) in IIM patients with ILD.MethodsIIM patients fulfilling Bohan and Peter criteria and having ILD were followed up in the Nasonova Research Institute of Rheumatology from 2009 to 2015. RTM was administered in case of intolerance or inadequate response to GC and other immunosuppressive drugs. Manual muscle testing (MMT), dyspnea assessment according to NYHA, creatinkinase (CK) and anti-Jo-1 antibodies (anti-Jo-1) assay; forced vital capacity (FVC) and carbon monoxide diffusion capacity (DLCO) evaluation as well as high-resolution computed tomography (HRCT) scanning of the chest were performed at baseline, 6–12 and 18–24 months after inclusion.Results41 patients (34 females, 7 males) with median age 50 [44;56], median disease duration 24 months [7;108] were included. 28 from them had decreased MMT value and class 1–2 NYHA dyspnea was present in 19. 18 patients were positive for anti-Jo-1 and 15 had elevated CK level. HRCT showed ILD signs including ground glass opacities (GGO) in all cases. FVC<80% was revealed in 16 and DLCO<80% - in 32 cases. All patients received GC 10–90 mg/day equivalent to prednisolone, 28 were treated with immunosuppressive agents (cyclophosphamide or mycophenolate mofetil). Patients received RTM courses consisted from 2 infusions of RTM 1000 or 500 mg at days 0 and 14. 1 RTM course was provided in 13, 2 – in 10, 3 – in 11, 4 – in 2, 5 – in 2, and 8 – in 3 patients. The follow up lasted for at least 12 months in 33, and for at least 24 months – in 15 patients. Dyspnea subsided and CK level decreased to normal values in all patients within 6–12 months after the first RTM infusion. 16 patients improved FVC by >10%, and 7 demonstrated DLCO increase by >10%. Worsening of pulmonary function tests was documented in 2 cases, while in the rest they were stable. After 18–24 months of follow up FVC remained +10% better vs baseline value in 10, DLCO – in 8 patients. In the rest patients these parameters remained stable. ILD was not progressing in 27 patients based on HRCT images in 6–12 months, moreover GGO lesions count was reduced in 10 patients, but there were cases of ILD worsening documented in 6 patients. Although, HRCT monitoring in 18–24 months showed ILD improvement in 7 patients and worsening – in 2, in remaining patients ILD was stable. Comorbid infections were documented in 18 patients, commonly manifesting in 2–3 months after the first RTM infusion.ConclusionsRTM treatment allows to achieve stable disease without typical for this condition further progression of pulmonary lesions in the majority of IIM patients with ILD.Disclosure of InterestNone declared

Они способны вызывать широкий диапазон поражений -от безобидных заболеваний кожи и слизистых оболочек до ин-вазивных процессов с поражением практически любого орга-на и развитием клинической ситуации, угрожающей жизни.Нарастание числа инвазивных методов обследования и лечения, особенно с длительным пребыванием инородных предметов в кровяном русле, привело к тому, что кандиды стали одним из основных оппортунистических микроорга-низмов, вызывающих госпитальные инфекции. «Инвазив-ный кандидоз, по большому счету, -болезнь медицинского прогресса, являющаяся обратной стороной достижений в области медтехнологий в течение нескольких последних де-сятилетий» [1]. Ежегодная частота кандидемии составляет 2-11 и 6-24 случаев на 100 тыс. населения в странах Евро-пы и США соответственно [2].Среди представителей рода Candida наиболее распро-страненным остается C. albicans, который вызывает около 90% случаев поверхностного и 50-70% -инвазивного кан-дидоза. Однако за последние 20 лет отмечено существенное нарастание числа инфекций, вызванных другими видами. Так, C. tropicalis является причиной 10-20% случаев кандиде-мии и диссеминированного кандидоза. C. parapsilosis вызыва-ет 7-10% случаев инвазивного кандидоза. На долю C. glabrata приходится 8% всех случаев кандидемии у онкологических больных и 11-14% у прочих пациентов. Необходимо заме-тить, что неоправданно широкое применение антимикоти-ков для профилактики и эмпирической терапии привело к нарастанию частоты резистентных к флуконазолу штаммов C. tropicalis, C. parapsilosis и C. albicans. В России частота устой-чивых к флуконазолу штаммов среди возбудителей кандиде-мии и острого диссеминированного кандидоза (ОДК) соста-вляет 15-30%, а в отдельных стационарах -50% [3,4] От других оппортунистических микозов кандидоз отли-чается в первую очередь тем, что является экзогенной ин-фекцией. Способность кандид колонизировать слизистые оболочки у большинства людей приводит к тому, что источ-ник инфекции локализуется не во внешней среде, а в орга-низме самого больного. При содействии разнообразных фа-кторов риска (табл. 1) происходит поражение макроорга-низма (инвазия) кандидами в первую очередь через повреж-денный эпителий кишечника. В качестве экзогенных ис-точников кандидоза выступают, главным образом, веноз-ные катетеры, инфицирование которых происходит с кожи больного или через руки медицинского персонала. В табл. 2 представлены основные формы кандидоза в соответствии с рубрикацией МКБ-10. Общепринятая клас-сификация рассматриваемой патологии отсутствует. BackgroundIn current rheumatology practice concurrent infections produce significant negative impact on patients’ morbidity, mortality and quality of life. Based on WHO estimations the annual incidence of influenza in adult population amounts to 5-10% worldwide. Influenza can lead to hospitalization (3 to 5 million cases per year) and even death (250-500 thousand cases per year). Flu and its complications rates are higher in patients with rheumatic diseases (RD) as compared to general population. Therefore, prevention of influenza should be viewed as integral part of RD population management.ObjectivesTo study the safety, efficacy and immunogenicity of inactivated split-virus influenza vaccine in patients with rheumatoid arthritis (RA) and ankylosing spondylitis (AS).Methods126 subjects (90 females and 36 males, aged 22 - 82 y) with recent acute respiratory viral infections (ARVI) and flu episodes in medical records were enrolled, including 52 RA patients, 34 AS patients and 40 healthy volunteers as the control group. 39 RA pts received methotrexate (MTX), 12 - TNFα inhibitors +MTX, 8 – Leflunomide, 2 – abatacept, 2 – sulfasalazine, 1 – tofacitinib + MTX. 19 AS patients were treated with nonsteroidal anti-inflammatory drugs (NSAIDs), 15 –with TNFα inhibitors. The RD duration ranged from 2 months to 46 years. All participants were injected subcutaneously with one dose (0.5 ml) of the trivalent inactivated influenza vaccine containing the actual influenza virus strains, with ongoing therapy. The control visits were scheduled at baseline, and in 1, 3 and 6 months after vaccination (Visits 0, 1, 2 and 3, respectively). Standard clinical and laboratory tests were performed during each visit. Immunogeniity of vaccine was measured with ELISA test kits.ResultsVaccine tolerability was good in 103 participants (77,4%). Post-vaccination pain, swelling and redness of the skin up to 2 cm in diameter were registered in 20 cases (15%), low-grade fever, myalgia and malaise were documented in 10 cases (7,5%). There was no casual relationship between these reactions and principal therapy, therefore, no modifications of therapeutic regimens were required, and complete resolution occurred within 24 hours without additional interventions. No RD exacerbations or new autoimmune disorders were observed during the FUP. At baseline mean pts’ DAS28 and BASDAI scores were 3.56 and 3.85, improving up to 1.99 and 3,09, respectively, 6mo post-vaccination. For the entire FUP there were no cases of influenza or influenza-like illness registered.The proportion of respondents to the vaccine is 70% in the group of patients with RD and 75% in the main group. The level of humoral immune response was not significantly different in the group of patients with RD and in the control. It is noteworthy that there were no significant differences in the level of post-vaccination response after a month (p = 0.6) or after 3 or 6 months of observation. В современной ревматологии несомненного внимания заслуживает проблема коморбидных инфекций, формирование которых обусловлено как самим ревматическим заболеванием (РЗ), так и необходимостью применения препаратов с иммуносупрессивным действием. Многочисленные проблемы, обусловленные инфекциями в ревматологии и иных отрас-лях клинической медицины, нельзя решить только путем использования большого количества антиинфекционных пре-паратов, которые доступны на сегодняшний день. Следовательно, в ближайшей перспективе важная роль будет отведена созданию, совершенствованию и быстрому (по возможности) внедрению в клиническую практику вакцин различной направленности. В данном обзоре рассмотрены вопросы, касающиеся применения вакцин против гриппа и пневмококко-вой инфекции у больных РЗ, а также сформулированы перспективы дальнейшего внедрения вакцинопрофилактики в ревматологии.
ЛечениеНесмотря на обширный арсенал антимикробных средств, разработанных и внедренных в клиническую практику в течение последних 50 лет, и интенсивное раз-витие кардиохирургии, курация больного с инфекцион-ным эндокардитом (ИЭ) по-прежнему остается далеко не легкой задачей.Антибактериальная терапия. Основополагающим принципом лечения ИЭ является ранняя, массивная и дли-тельная (не менее 4-6 нед) антибактериальная терапия с учетом чувствительности выделенного возбудителя к анти-биотикам. Необходимо применять антибиотики, оказываю-щие бактерицидное действие.При наличии у больного остро протекающего ИЭ (вы-раженные явления интоксикации, быстро прогрессирую-щая клапанная регургитация, нарастание застойной недос-таточности кровообращения) антибактериальную терапию назначают эмпирически сразу же после 3-кратного (с ин-тервалом 0,5-1 ч) взятия крови из вены для исследования на гемокультуру. В случаях подострого ИЭ лечение может быть отсрочено до 48-72 ч с целью получения данных, не-обходимых для этиотропной антибактериальной терапии.Основные схемы антибактериальной терапии ИЭ в за-висимости от наиболее часто выявляемых инфектогенов представлены в табл. 1.Лечение ИЭ, вызванного пенициллиночувствительны-ми штаммами зеленящих и других стрептококков, проводят бензилпенициллином или цефалоспоринами III поколе-ния, в частности, цефтриаксоном, фармакокинетические особенности которого позволяют успешно применять его 1 раз в сутки. При указанных схемах монотерапии бактерио-логическое излечение достигается у 98% больных.Имеются данные о высокой эффективности 2-недель-ного комбинированного лечения пенициллином и гентами-цином либо цефтриаксоном и нетилмицином [4]. Однако применение указанных курсов лечения оправдано только у больных моложе 65 лет с ИЭ, вызванным высокочувстви-тельными к пенициллину стрептококковыми штаммами (Str. viridans или Str. bovis), протекающим без осложнений в виде внутрисердечных абсцессов, экстракардиальных оча-гов инфекции и тромбоэмболических феноменов, при со-хранной функции почек и непораженной VIII паре череп-ных нервов (nn. vestibulocochlearis) при условии четкой поло-жительной динамики после 7 дней терапии.При ИЭ, вызванном стрептококками, умеренно чув-ствительными к пенициллину, также показана комбини-рованная терапия пенициллином в более высоких суточ-ных дозах (до 30 млн ЕД), а при непереносимости послед-него -цефалоспоринами I (цефазолин) или III (цефтриа-ксон) поколения в сочетании с 2-недельным курсом ген-тамицина. Следует учесть, что при длительном примене-нии аминогликозидов (в частности, гентамицина) в высо-ких дозах необходимо мониторирование концентрации этих препаратов в сыворотке крови с целью предупрежде-ния их ото-и нефротоксического эффектов. Однако это требование не всегда выполнимо, учитывая недостаточ-ную техническую оснащенность отечественных стациона-ров общего профиля.
