В статье представлено достаточно редкое клинические наблюдение сочетания двусторонней гигантомастии с миа стенией. Дебют миастении опасен развитием внезапной дыхательной недостаточности. Своевременная диагностика миастении при гигантомастии важна для адекватного анестезиологического пособия, ведения пациентов в раннем послеоперационном периоде. Из-за высокого риска развития токсико-анемического синдрома после оперативного лечения по поводу гигантомастии стоит рассматривать возможность двухэтапного проведения мастэктомии с последующей пластической операцией. The debut of myasthenia dangerous to sudden respiratory failure. Timely diagnosis myasthenia is important for adequate anesthetic management of patients benefit in the early postoperative period. Due to the high risk of toxic-anaemic syndrome is considered one of the stages of mastectomy.
Primary hyperparathyroidism (PHPT) is caused by parathyroid malignant neoplasm in 1% of cases. The risk of the latter is higher in patients with symptomatic PHPT. The prognosis in this group of patients depends on the extent of the process and primary surgical intervention. In these cases, the differential diagnosis between secondary foci in the bones associated with parathyroid cancer and hyperparathyroid osteodystrophy is a challenging problem. This article describes two cases of severe PHPT accompanied by hyperparathyroid osteodystrophy suspected for metastatic parathyroid cancer. Positron emission tomography in combination with computed tomography (PET/CT) with 18F-fluorodeoxyglucose (18F-FDG) and/or 18F-fluorocholine was included in the examination algorithm. In both cases, pronounced bone changes similar to parathyroid metastases were observed. Accumulation of 18F-fluorocholine was also observed only in altered parathyroid gland. Histological examination of postoperative material verified benign parathyroid tumors, and characteristic lesions of bone tissue were regarded as areas of osteodystrophy. Therefore, accumulation of 18F-fluorocholine at the areas of bone destruction does not enable differentiation between hyperparathyroid osteodystrophy and metastatic lesions; further research is required to assess sensitivity and specificity of the method with respect to topical diagnosis of altered parathyroid gland.

Распространенность ПГПТ в популяции составля-ет около 1%, увеличивается с возрастом и достигает 2% Сложности в топической диагностике первичного гиперпартиреоза в ряде случае обусловлены вариабельностью расположения околощитовидных желез. Наиболее часто верхние околощитовидные железы обнаруживаются по за-днемедиальной поверхности правой и левой долей щитовидной железы (верхняя треть доли), хотя встречаются и на уровне бифуркации общей сонной артерии, за глоткой и пищеводом, и интратиреоидно (1%). Расположение нижних околощитовидных желез более вариабельно: по боковой или задней поверхности, или ниже нижнего полюса щитовидной железы на расстоянии 0,2-1,5 см, в рогах тимуса, в переднем или заднем средостении, интратиреоид-но (1-2%). Это объясняется сложным и продолжительным характером миграции зачатка из III-IV жаберного кармана в ходе эмбриогенеза. Топическая диагностика при первичном гиперпаратиреозе опирается на данные УЗИ органов шеи, сцинтиграфии с технетрилом, мультиспиральной компьтерной томографии органов шеи и средостения. При со-четании многоузлового зоба и интратиреоидного расположения околощитовидной железы могут возникнуть допол-нительные сложности в верификации различных узлов (узлы щитовидной железы или околощитовидные железы). В настоящей статье обсуждается два клинических случая интратиреоидного расположения околощитовидных желез. Показан алгоритм топического подтверждения диагноза. Определение уровня паратгормона в пунктате из подозри-тельных образований, которые могут представлять собой интратиреоидные околощитовидные железы или узлы щи-товидной железы, поможет избежать диагностических ошибок. In this article, we discuss difficulties in parathyroid localization modalities in diagnosis of primary hyperparathyroidism. Most often, superior parathyroid glands are located on the posteromedial surface of the right and left lobes of the thyroid gland, however, they also could be found at the carotid bifurcation, behind pharynx and esophagus, as well as inside thyroid gland. Location of the inferior parathyroid glands is more variable: on the side or back surface, or below the lower pole of the thyroid gland, as well as in thymus, posterior or anterior mediastinum, or inside thyroid. Localization modalities of primary hyperparathyroidism are based on neck ultrasonography, scintigraphy with sestamibi, computed tomography of neck and mediastinum. Rare condition characterized by an intensive and excessive breast enlargement is known as gigantomastia. Gestational gigantomastia manifests during pregnancy and could represent a life-threatening condition for both the mother and the fetus, therefore the risk of spontaneous abortion complicates the choice of treatment approach. We present a clinical case of a patient requesting medical assistance with a progressive breast growth. The onset occurred on the 5th week of gestation and, subsequently, was aggravated by distal tissue necrosis. The ambiguity of treatment approach in such situation is based on, on the one hand, the insecurity of watch-and-wait approach due to the high risk of septic complications and, on the other hand, the increasing danger of fetal loss in the case of surgical treatment during the first trimester of pregnancy. After an adequate assessment of mothers health state, a decision was made towards the wait-and-see tactic until the complete formation of the placental barrier. A total bilateral mastectomy was performed at the 19th week of pregnancy, with a favorable outcome for the patient and her child. This case emphasizes the significance of an individual, interdisciplinary approach to the choice of management for patients with this pathology.
