Aim. To identify clinical and functional manifestations of hypervolemia in patients with arterial hypertension (AH). Material and methods. In total, 440 patients with Stage I-II AH were examined, including assessment of salt taste sensitivity threshold (STST), 24-hour urinary Na excretion, and NaCl excretion. In addition, 24-hour blood pressure monitoring (BPM), echocardiography (EchoCG), and psychological status assessment (SMOL, MOS SF-36) were performed. Results. In 50,5% of AH patients, daily salt intake (assessed by 24-hour NaCl excretion) was ≥16,8 g, due to adding salt to food, as well as to reduced STST. Higher salt intake was associated with more advanced clinical and functional AH manifestations. Indirect markers of hypervolemia in AH patients included higher salt intake, low effectiveness of standard antihypertensive therapy (AHT), disturbed circadian BP rhythm with inadequate nighttime BP reduction, and EchoCG signs of left ventricular volume overload. Conclusion. Complex examination of AH patients helps to identify individuals with clinical and functional manifestations of hypervolemia.
Aim. To study the functional state of arterioles in patients with type 2 diabetes mellitus and concomitant arterial hypertension (AH) and evaluatethe role of sodium in mechanisms of elevation of arterial pressure (AP) in this pathology. Materials and methods. 163 patients of whom 83 had DM2 with AH and 80 essential hypertension. They were examined by dopplerography of themicrocirclatory bed, measurement of arterial blood flow and daily sodium urinary excretion. Results. Patients with essential AH showed increased arteriolar circulation rate and enhanced reactivity of microvessels. These parameters werereduced in patients with DM2 and AH who consumed large amount of table salt. Conclusion. Mechanisms of development of essential AH and AH in DM2 are significantly different.

У здоровых людей в перифе-рической крови отмечаются единичные ауторозетки (АР), однако при патологических состояниях их коли-чество значительно увеличивается. Процесс внутри-сосудистого ауторозеткообразования (АРО) завер-шается разрушением эритроцитов, входящих в их со-став. У пациентов с сердечно-сосудистыми заболева-ниями феномен АРО изучен недостаточно. Фактически остается неясным вопрос о влиянии лекарственной те-рапии на межклеточные соотношения в перифериче-ской крови.Цель исследования -изучение изменений меж-клеточных взаимоотношений в периферической кро-ви у больных артериальной гипертонией (АГ), ише-мической болезнью сердца (ИБС) и хронической сер-дечной недостаточностью (ХСН) на фоне лекарствен-ной терапии. Изучить изменения межклеточных соотношений в периферической крови у больных артериальной гипертонией (АГ), ишемической болезнью сердца (ИБС) и хронической сердечной недостаточностью (ХСН) под влиянием лекарственной терапии. Материал и методы. Обследовано 610 больных, в том числе 250 с АГ I-III стадии, 150 -с ИБС и 210 -с ХСН I-III стадии. Все больные получали лекарственную те-рапию, кроме 50 больных АГ. В мазках капиллярной крови (окраска по Романовскому-Гимзе) подсчитывали общее число лейкоцитов, число ауторозеток (АР) и АР с лизисом эритроцитов. За АР принимали клеточную ассоциацию, состоящую из нейтрофила, моноцита или эозинофила с плотно прилегающими к их поверхности тремя и более эритроцитами. Определяли содержание эритроцитов и гемоглобина в периферической крови. Контрольную группу составили 80 здоровых лиц (40 мужчин, 40 женщин). Результаты. В контрольной группе и у больных АГ без лечения в периферической крови выявляются единичные АР. У больных ИБС, АГ и ХСН под влиянием лекарственной терапии число АР значительно повышается, нарастает число АР с лизисом эритроцитов, уменьшается содержание эритроцитов и гемоглобина крови. To evaluate neurogenic theory of essential hypertension taking into account the recently obtained findings. Design and methods. We analyzed evidence data and our own results from a group of 630 hypertensive patients. The threshold of their gustatory sensation to dietary salt (DS), urinary sodium excretion, daily blood pressure monitoring, echocardiography, cardiointervalography were studied; psychological state was assessed. The control group consisted of 350 healthy persons. Results. According to our data, inсreased DS consumption is the main factor leading to clinical and functional manifestations of arterial hypertension. According to the literature data citizens of blocked Leningrad obtained excessive amount of DS that could be the main cause of the epidemic of arterial hypertension in the blocked city. Conclusions. Decrease of DS consumption by population is fundamental preventive measure of lowering hypertension prevalence
