Asiatic acid, a triterpenoid compound, has been shown to have anti-inflammatory activity through the inhibition of the formation of cyclooxygenase-2 (COX-2) in vitro and in vivo. This study was conducted to determine the binding stability and the inhibitory potential of asiatic acid as an anti-inflammatory candidate. The study involved in vitro testing utilizing a colorimetric kit as well as in silico testing for the pharmacophore modeling and molecular dynamic (MD) simulation of asiatic acid against COX-2 (PDB ID: 3NT1). The MD simulations showed a stable binding of asiatic acid to COX-2 and an RMSD range of 1–1.5 Å with fluctuations at the residues of Phe41, Leu42, Ile45, Arg44, Asp367, Val550, Glu366, His246, and Gly227. The total binding energy of the asiatic acid–COX-2 complex is −7.371 kcal/mol. The anti-inflammatory activity of the asiatic acid inhibition of COX-2 was detected at IC50 values of 120.17 µM. Based on pharmacophore modeling, we discovered that carboxylate and hydroxyl are the two main functional groups that act as hydrogen bond donors and acceptors interacting with the COX-2 enzyme. From the results, it is evident that asiatic acid is a potential anti-inflammatory candidate with high inhibitory activity in relation to the COX-2 enzyme.
Depresi merupakan penyakit yang mempengaruhi perasaan, cara berpikir, dan tindakan. Depresi menyebabkan perasaan sedih dan / hilangnya minat pada suatu aktivitas. Depresi dapat diobati menggunakan antidepresan. Piper nigrum L. atau Lada Hitam merupakan tanaman yang dapat digunakan sebagai antidepresan. Pada lada hitam terkandung senyawa metabolit sekunder yang dapat sebagai antidepresan dengan mekanisme terhadap 5-HT1B. Penelitian ini bertujuan untuk mempelajari interaksi antara senyawa yang terkandung pada lada hitam yang berpotensi sebagai antidepresan terhadap protein 5-HT1B menggunakan metode molecular docking secara in silico. Pengujian dilakukan menggunakan aplikasi Autodock 4.2 untuk simulasi penambatan molekuler, divisualisasi dengan Biovia Discovery Studio 2020 Client, dan analisis hasil farmakokinetika serta toksisitas dari senyawa melalui situs pre-ADMET. Hasil penambatan diperoleh senyawa piperettin memiliki potensi paling besar menjadi kandidat obat antidepresan melalui mekanisme agonis 5-HT1B dibandingkan senyawa uji lainnya karena memiliki energi ikatan sebesar -8,84 kKal/mol dengan ikatan hidrogen pada residu asam amino THR134. Berdasarkan analisis hasil prediksi, Piperettin memiliki sifat farmakokinetika dengan nilai CaCo-2 54,6793; HIA 98,050270% ; PPB 92,408768%. Hasil pre-ADMET senyawa menunjukan adanya dugaan potensi karsinogen.Kata kunci: 5-HT1B, Antidepresan, Molecular Docking, Piperettin, Piper nigrum
Major depressive disorder (MDD) and anxiety are known as a widespread public health problem treated with sertraline, a class of Selective Serotonin Reuptake Inhibitor (SSRI) drugs as first-line therapy. However, clinical evidence showed that MDD and anxiety therapy with sertraline has not been optimal due to patient non-compliance associated with adverse drug events (ADE) occurring. A literature search on sertraline clinical study was conducted on the PubMed and Science Direct electronic database in April 2020. A total of 12 articles from 5.664 articles have been selected at the initial screening. A total of 6 articles used sertraline as a testing drug in MDD patients, 2 articles used sertraline as testing drug in anxiety patients, 2 articles with sertraline as a comparative drug, 1 article with sertraline as a positive control, and 1 article with sertraline as an adjuvant in MDD patients. Studies on these articles were carried out worldwide from 2010 to 2019. ADEs that occur due to the use of sertraline were: gastrointestinal disorders, nutritional and metabolic disorders, central nervous system disorders, sweating, sleep disorders, irritability, eye disorders, as well as disorders of the skin and subcutaneous tissues. Nausea is the most common ADE of sertraline. ADEs are affected by several factors such as, age, the sertraline combination therapy, and alcohol consumption.
Coronavirus Disease-19 (COVID-19) merupakan masalah kesehatan masyarakat serius yang telah menjadi pusat perhatian global sejak akhir 2019. COVID-19 merupakan pandemi yang menyebabkan komplikasi medis yang parah bahkan kematian di seluruh dunia pada beberapa kasus. Strategi penanggulangan yang paling efektif untuk mengatasi COVID-19 saat ini adalah pemberian vaksin COVID-19 yang efektivitasnya telah terbukti secara global. Beberapa vaksin COVID-19 di Indonesia yang telah beredar seperti Pfizer BioNTech®, Moderna, AstraZeneca, dan Coronavac telah memberikan perlindungan di masyarakat dengan merangsang respons kekebalan tubuh. Akan tetapi, pada beberapa kasus, vaksin-vaksin tersebut juga dapat menyebabkan kejadian yang tidak diinginkan setelah pemberiannya yang dikenal dengan istilah Kejadian Ikutan Pasca Imunisasi (KIPI). Artikel ini mengulas KIPI yang didapat dari artikel pada database PMC dan ScienceDirect dengan kriteria inklusi berupa artikel uji klinis pada 1 tahun terakhir dan berbahasa inggris. Ulasan ini mengeksklusi artikel review. Didapat 404 artikel tentang KIPI vaksin COVID-19 dan hanya 11 artikel yang memenuhi kriteria inklusi. KIPI akibat vaksin COVID-19 dapat terjadi secara lokal maupun sistemik. KIPI lokal yang umum terjadi di antaranya nyeri, kemerahan, dan bengkak di situs injeksi sedangkan KIPI sistemik yang umum terjadi adalah demam, nyeri otot dan sakit kepala. KIPI lain yang mungkin terjadi di antaranya diare, mual, dan muntah. selain itu, beberapa vaksin berpotensi mengakibatkan limfadenopati unilateral, bells palsy, serta trombosis vena serebral. Namun, perlu data yang lebih kuat untuk membuktikan bahwa reaksi sistemik tersebut terjadi akibat vaksin COVID-19. KIPI terjadi pada resipien dengan usia lebih muda. Frekuensi KIPI bertahan rata-rata pada 1-2 hari dengan tingkat keparahan ringan sampai sedang.
