Abstract-Smooth muscle expresses the I␣ and the I isoforms of cGMP-dependent protein kinase I (cGKI). Inactivation of the murine cGKI gene prkg1 leads to multiple phenotypes and premature death at Ϸ6 weeks. We reconstituted mice with the cGKI␣ or -I isozyme to test which isozyme was needed to support basic smooth muscle functions. Mice were generated by gene targeting. The cGKI␣ or the -I coding sequences were placed under the control of the SM22␣ promoter to express either isoform selectively in smooth muscle cells (SM-I␣ or SM-I transgene). To generate smooth muscle-specific cGKI␣ or cGKI rescue mice, the SM-I␣ or SM-I transgenes were crossed on a cGKI Ϫ/Ϫ genetic background. The levels of cGKI␣ or -I expression were comparable to endogenous cGKI expression in wild-type aortic and intestinal smooth muscles. In cGKI␣ or -I rescue mice, expression of the isozymes was not detectable in non-smooth muscle tissues and cells. Median survival time of the I␣ and I rescue mice was 52 weeks. Both isozymes mediated the 8-bromo-cGMP-induced relaxation of precontracted jejunum and aorta muscle strips. Activation of both isozymes reduced hormone-or K ϩ -induced [Ca 2ϩ ] i levels. The cGKI␣ and cGKI rescue mice did not show a significant difference in intestinal passage time of BaSO 4 in comparison with wild-type animals. Telemetric blood pressure measurements in conscious freely moving animals did not show differences between rescues and control mice in basal blood pressure and its regulation by DETA-NO, sodium nitroprusside, carbachol, or Y-27632. These results show that cGKI in smooth muscle is essential and that either cGKI isozyme alone can rescue basic vascular and intestinal smooth muscle functions. 10 seconds (apnea) or a 40% reduction in airflow associated with decreased arterial oxygen saturation of at least 4% (hypopnea) despite persistent ventilatory efforts during sleep. In the Wisconsin Sleep Cohort Study, a population-based study of employed middle-aged adults, 24% of men and 9% of women had sleep-disordered breathing defined by an apnea hypopnea index (AHI), a summary measure of the total number of apnea and hypopnea events per hour of sleep, of five or greater. 1 An AHI of five or greater in combination with self-reported hypersomnolence is indicative of clinically symptomatic sleep apnea syndrome. 2 It is estimated that 4% of middle-aged men and 2% of middle-aged women in the general population meet minimal criteria for sleep apnea syndrome. 1 Several epidemiological studies have shown that sleep apnea syndrome is associated with cardiovascular diseases, such as hypertension, angina, arrhythmias, coronary heart disease, and congestive heart failure. [1][2][3][4][5][6][7][8][9][10][11][12][13][14][15] In recent community-based studies of sleep-disordered breathing, even mild occult sleep apnea is associated with hypertension, 16,17 excessive daytime somnolence and morning headaches, 18-20 mental and cognitive impairments, 21-24 erectile dysfunction, 25 fibromyalgia, 26 and higher rates of automobile and work-related accidents. 27,28 Many of these sequelae of Objective: To investigate the effects of sleep apnea (SA) on the quality of life (QOL). Design: A prospective study of QOL in patients with and without SA as defined by an apnea-hypopnea index (AHI) >5. Setting: University-based outpatient clinics. Patients: Primary care patients followed in a general internal medicine clinic as well as those referred to a sleep disorders clinic at the University of Wisconsin Hospital and Clinics were consecutively recruited and classified into 3 groups of subjects: (1) patients without SA (AHI<5) (n=46), (2) patients with mild SA (AHI 5-15) (n=16), and (3) patients with moderate to severe SA (AHI>15) (n=21). Interventions: NA Measurements: QOL was assessed with the Medical Outcomes Study SF-36 Health Survey. Health history and demographic data were obtained via structured interview and medical record review. All subjects underwent overnight polysomnography for diagnosis of SA. Results: After controlling for age, gender, body mass index, and number of comorbid conditions, the association between sleep apnea and QOL was significant in the domains of physical functioning and role limitation due to physical health problems (p<0.05) and was borderline in vitality (p<0.1). Patients with both mild and moderately severe SA scored significantly lower (worse) than did patients without SA in physical functioning and in role limitations due to physical-health (82 and 83 vs. 92, respectively). RESUMO:Com o aprofundamento do debate sobre questões educacionais, o professorado, a partir da década de 1980, passou a ser reconhecido como um dos principais agentes de mudança, seja da qualidade do ensino, seja da democratização da sociedade brasileira. A sua atuação organizada, entretanto, segundo Cunha (1991), poderia estar tendo um "efeito de retroanulação política", favorecendo as teses de privatização do ensino público. A pertinência desta interpretação é examinada sob a perspectiva do docente como profissional que busca alargar o seu poder e o reconhecimento de sua especialidade pelo Estado e pelo mercado, aspectos enfatizados ou criticados nos estudos sobre profissões (Barbosa, 1993;Bonelli, 1999;Freidson, 1998;Larson, 1977;Marques, 1995). ResumoIntrodução: O sono é um importante indicador de saúde e sua qualidade pode sofrer influência de diversos fatores, como alterações psicológicas e sociais; ambiente; uso de medicamentos; ingestão alcoólica e a prática regular de atividade física. Porém, ainda não está esclarecido se a atividade física teria efeito positivo sobre os distúrbios de sono mais prevalentes como insônia e apneia do sono. Objetivo: buscar publicações que falam da interação sono e atividade física, e estudos que esclareçam os efeitos da atividade física sobre a qualidade do sono e qualidade de vida. Material e Método: foi feito uma busca na literatura nas bases de dados: Biblioteca Virtual em Saúde (BVS), Scielo, Pubmed e Scopus usando os descritores Exercício; Sono; Qualidade de Vida; Apneia Obstrutiva do Sono. Resultados: foram discutidos sete artigos que abordam o assunto a atividade física como coadjuvante para a qualidade do sono mostrando que atividade física tem ação benéfica. Conclusão: a atividade física melhora a percepção subjetiva e objetiva da qualidade do sono e qualidade de vida, podendo ser utilizada como tratamento terapêutico nos tratamentos dos distúrbios do sono. AbstractIntroduction: sleep is an important indicator for health, its quality may suffer influences of several factors as psychological and social disturbances, alcohol abuse, environment, drug intake, and, positively, physical activity. However, it is not clear if physical activity would have a positive effect for the most prevalent sleep disorders as insomnia, and sleep apnea. Objective: the aim of this study was to perform a review on the literature which discussed the interaction between sleep and physical activity, regarding the repercussion on sleep quality and quality of life. Material an method: the search strategy included the databases Biblioteca Virtual em Saúde (BVS), Scielo, PubMed and Scopus, using the descriptors: Exercise; Sleep; Quality of Life; Sleep Apnea, Obstructive. Results: seven articles met the inclusion criteria, discussing physical activity as a co-adjuvant treatment, all showing improvement of sleep quality and positive effects on quality of life. Conclusion: physical activity improves subjective perception and objective evaluations of sleep quality and quality of life, and should be considered as a co-adjuvant treatment for sleep disorders.
OBJETIVO: Apontar as possíveis alterações orofaciais decorrentes do sintoma "obstrução nasal" em pacientes portadores de doenças alérgicas de vias aéreas superiores, por meio de revisão de literatura. FONTES DE DADOS: Levantamento bibliográfico utilizando bancos de dados eletrônicos, como Medline, Ovid, SciELO e Lilacs, com as palavras-chave "asthma", "rhinitis" e "mouth breathing", abrangendo os 30 últimos anos. Foram incluídos artigos de revisão, estudos observacionais e ensaios clínicos. SÍNTESE DOS DADOS: A obstrução nasal é encontrada freqüentemente em doenças alérgicas de vias aéreas, como rinite e asma. A respiração bucal decorrente da obstrução nasal pode interferir de maneira direta no desenvolvimento infantil, com alterações no crescimento do crânio e orofacial, na fala, na alimentação, na postura corporal, na qualidade do sono e no desempenho escolar. CONCLUSÕES: Devido à variedade de alterações orofaciais encontradas na criança respiradora bucal decorrente de obstrução nasal por doenças alérgicas de vias aéreas, é necessário realizar diagnóstico e tratamento precoces por uma equipe multidisciplinar, composta por médico, ortodontista e fonoaudiólogo, contemplando a visão de uma via respiratória única, que traz conseqüências ao crescimento e desenvolvimento do sistema motor oral.
The prevalence of OSAS in children is 0.7-3%, with peak incidence in pre-schoolers. It is characterised by partial or complete upper airway obstruction during sleep, causing intermittent hypoxia. Both anatomical (severe nasal obstruction, craniofacial anomalies, hypertrophy of the pharyngeal lymphoid tissue, laryngeal anomalies, etc.) and functional factors (neuromuscular diseases) predispose to OSAS during childhood. The main cause of OSAS in children in adenotonsillar hypertrophy. The most common clinical manifestations of OSAS are: nocturnal snoring, respiratory pauses, restless sleep and mouth breathing. Nocturnal pulse oximetry, nocturnal noise audio/videotape recording and nap polysomnography are useful tools for screening suspected cases of OSAS in children, and the gold-standard for diagnosis is overnight polysomnography in the sleep laboratory. On the contrary of SAOS adults, children usually present: less arousals associated to apnea events, more numerous apneas/hypopneas during REM sleep, and more significant oxyhemoglobin desaturation even in short apneas. The treatment of OSAS may be surgical (adenotonsillectomy, craniofacial abnormalities correction, tracheostomy) or clinical (sleep hygiene, continuous positive airway pressure--CPAP).
We proposed to compare the accuracy and effectiveness of digital breast tomosynthesis (DBT), plus digital or synthetic mammography, with digital mammography alone in women attending populationbased breast cancer screenings. We performed a systematic review and included controlled studies comparing DBT with digital mammography for breast cancer screening. Search strategies were applied to the MEDLINE, Embase, LILACS, and CENTRAL databases. With moderate quality of evidence, in 1,000 screens, DBT plus digital mammography increased the overall and invasive breast cancer rates by 3 and 2 (RR 1.36, 95% CI 1.18 to 1.58 and RR 1.51, 95% CI 1.27 to 1.79, respectively). DBT plus synthetic mammography increased both overall and invasive breast cancer rates by 2 (RR 1.38, 95% CI 1.24 to 1.54 and RR 1.37, 95% CI 1.22 to 1.55, respectively). DBT did not improve recall, false positive and false negative rates. However due to heterogeneity the quality of evidence was low. For women attending population-based breast cancer screenings, DBT increases rates of overall and invasive breast cancer. There is no evidence with high or moderate quality showing that DBT compared with digital mammography decreases recall rates, as well as false positive and false negative rates.
P ensar o currículo mínimo do curso de Psicologia e o espaço da pesquisa implica, a meu ver, tratar de uma questão mais ampla, que é a da própria formação do psicólogo que, por sua vez, insere na problemática mais geral da formação do graduado universitário.A discussão dessas questões, no meu entender, é sempre permeada por uma imagem de ensino universitário, de graduado universitário, na qual se enfatizam especialmente os níveis de competência que supostamente devem caracterizar aqueles que buscam se preparar para integrar uma determinada comunidade científico-profis¬ sional. Tais níveis de competência, no entanto, assim como o próprio lugar de uma especialidade universitária na divisão do trabalho, tendem a ser periodicamente redefinidos; pelo menos em função: a) do avanço do conhecimento em uma área do saber; b) da predominância -às vezes apenas fugaz -de uma abordagem específica; c) dos desafios impostos pela realidade concreta ao exercício profissional nos seus diferentes aspectos; d) do estágio de reflexão sobre a própria prática profissional; e) das características dos grupos internos e externos a uma especialidade universitária que circunstancialmente definem políticas de avaliação acadêmica e de exercício profissional. Situação Atual dos Cursos de PsicologiaNo que concerne à Psicologia, curso e profissão relativamente recentes no Brasil -a Lei 4.119, que as instituiu, tendo sido promulgada em agosto de 1962 e a sua regulamentação datando de janeiro de 1964predomina, como em tantos outros casos, a imagem de um profissional, liberal de preferência, que tem evidentes repercussões sobre a formação do psicólogo.De fato, o Parecer 403/62 que fundamenta a Resolução que fixa o Currículo Mínimo e a duração do curso de Psicologia apresenta o psicólogo como "...tendo uma posição (que poderá ser de relevo) no concerto das chamadas profissões liberais...", não sendo pois de estranhar que o Currículo Mínimo então definido, e que ainda hoje é vigente, privilegie nitidamente o aspecto profissionalizante da Psicologia -linha, aliás, que caracterizara as experiências de pós-graduação feitas no Rio Grande do Sul e no Rio de Janeiro na década de 50 -, bem como permita a seleção de disciplinas em função de uma única área de atuação profissional, o que explicaria a ênfase que ainda vem sendo dada à formação de psicólogos clínicos.Esta ênfase é a explicitação da forma que a implantação da divisão do trabalho intelectual neste campo tomou no nosso país.
