On example of Lactobacillus and Bifidobacterium strains, approaches in creation of human multistrain probiotic metabolite mixtures for different goals were proposed. Human probiotic lectin systems (LS) (mucosal, others) reveal functions needed for organism. Advanced features of such systems include capability to recognize synthetic polymeric polyvalent glycoconjugates (GC)-imitators of natural ones (modified polysaccharides, glycoantigens of medical significance). Probiotic lectin systems function as imitators of multipro-/sym-/synbiotics in their resulting actions. They serve as carriers of the biotope glycoconjugate décor including glycoprebiotics, glycometabiotics, glycodrugs, and agents supporting décor organization and resistance. Probiotic lectin systems represent new perspective system agents to improve the health of mucosal microbiocenoses (MB) organized as communicative bodies to be corrected according to the principle "there is bodythere are diseases." They act as metabolomebiotics according to the principle "the network in the network." They deepen biotope resistance allowing quicker return to balance. They support prophylactic and therapeutic procedures directed to prolong aging and improve quality of life. Multistrain metabolite constructions can predict perspective cellular formulas of multipro-/synbiotics for prophylaxis, supporting and accompanying therapy. Approaches developed are universal. They are useful in the study of any Gram-positive and eukaryotic (yeasts and yeast-like fungi) monoand mixed cultures. The methodological principles proposed and described are of value for extended fields of clinical microbiology and medical bio-/nanotechnology.

Methods A retrospective audit was performed over 6 months (Jan 2011-June 2011) period. Management of all breastfed infants admitted with weight loss (>7% of birth weight) with or without other clinical symptoms of dehydration were reviewed. Information was collected from infant clinical notes, discharge summaries, hospital pathology reporting system and maternal notes. Results 1908 infants were born during this period, 33 infants (1.73%) required readmissions. No risk factor was identified in 3 infants, 30 infants had one or more risk factors, commonest being caesarean section delivery (61%) followed by infants born to 'first time mother's (33%). 72% of the infants developed jaundice. Majority of the infants(59%) were managed with breast feeding support only whereas 15% needed intravenous fluid support. Most infants (69.6%) were discharged within 48 hrs of admission. Half of the infants developing hypernatremia had comparatively mild weight loss (≤10% of birth weight). Conclusions Weight loss and Hypernatremia continues to be a problem in neonates particularly in 'at risk' infants. Most of these infants responded well to enteral feeding only. Degree of hypernatremia was not always proportion to the degree of weight loss.

Cervical cancer is an important problem in women’s health and a worldwide oncological disease. In 2018, the WHO registered 569,847 new cases in the world, and 3.4% were in the Russian Federation. We describe here a case of invasive cervical cancer stage IB2 associated with human papilloma virus in a woman who was treated by multicourse photodynamic therapy (PDT). A 38-year-old woman presented with abdominal pain and genital tract spotting in October 2015. Colposcopy revealed a neoplasm in cauliflower form. PAP smear result was cancer in situ (Tis). The biopsy result from the cervical canal and neoplasm was invasive squamous cell carcinoma. The patient underwent full preoperative examination (blood test, biochemical blood test, coagulation test, urinalysis, X-ray of chest organs, ECG, ultrasound investigation of pelvic organs, and PAP smear). Magnetic resonance imaging investigation showed a heterogeneous tumor, uneven contours, and intensity accumulating contrast. The patient was not pregnant, and a fertility-preserving treatment method was used. Three PDT sessions allowed to avoid vaginal radical trachelectomy. Pregnancy occurred 3 years and 8 months after the first PDT session. The patient had testing after treatment 4 times (3rd, 12th, 24th, and 60th months). She had a pregnancy without complications and had operative delivery by Cesarean section in April 2020. There was a 5-year remission period without episodes of relapse. The patient has an 8-month-old baby.

Immunisation with outer membranes of Francisella tularensis induced an efficient protection in guinea pigs against challenge with the virulent strains 503 or 144/713 (type B biovar holarctica), both clinical isolates, and prevented the development of typical signs of infection in hamadryads (baboons), challenged with the virulent strain Schu (type A, biovar tularensis) of F. tularensis. Immunisation with a lipopolysaccharide protein complex isolated from the outer membranes afforded protection in CBA mice against challenge with strain 503. Another LPS-protein complex obtained by the simple mixture of LPS preparations from strain 503 and a 17-kDa membrane protein from the avirulent R-variant of the vaccine strain 15 also demonstrated protective properties against experimental tularemia in mice.
