Pulmonary embolism (PE) is a major life-threatening illness which remains one of the main causes of sudden death throughout the world. The analysis of diagnosis and treatment of 472 patients with acute pulmonary embolism for a period of 10 years was performed. High efficiency of diagnosis using multispiral computer angiopulmonography (MSCT APG) has been established, thus this method completely supersedes the traditional selective angiopulmonography. Seventeen (3.6 %) patients died due to PE recurrence, another 8 (1.7 %) patients died due to the bleeding after using fibrinolytics and anticoagulants, and 14 (2.9 %) died due to progression of organs failure. This emphasizes the need to improve measures aimed to prevent PE recurrence and identify sources of possible bleeding and refrain from aggressive fibrinolytic therapy. The use of differentiated approach to the treatment with thrombolytic therapy and anticoagulants enabled to achieve recovery in 433 (91.7 %) patients who were discharged for outpatient treatment. New oral anticoagulants were prescribed to 94 (21.7 %) patients after discharge.
In patients with COVID-19, histological examination of the pulmonary vessels shows serious disorders (local thrombosis and microangiopathy), significantly different to those in the control group composed of patients with influenza. Some studies have shown that coronavirus enters cells by binding angiotensin-converting enzyme 2 which is found mainly on the alveolar epithelium and endothelium. The increase in D-dimer levels is typical for patients with COVID-19. Although many inflammatory processes can affect D-dimer levels, an increase in D-dimer during COVID-19 is more likely to indicate thrombosis. The aim. To analyze the patients who have suffered from COVID-19 and to determine possible risk factors for the development of thrombotic complications and define the surgical or medical therapy tactics. Material and methods. The study was conducted from October 2020 to April 2021 in Ukraine (Zaporizhzhia, Uzhhorod, Kherson and Kyiv). We analyzed 121 patients aged 46.9± 15.3 years, 64 (52.8%) men and 57 (47.1%) women. All the patients had positive PCR test for COVID-19. The changes in D-dimer were analyzed. Results. Forty-one (33.9%) patients had thrombotic complications. Pulmonary embolism (PE) was diagnosed in 14 patients, deep vein thrombosis (DVT) in 17, acute stroke in 2 and peripheral artery thrombosis in 8 cases. An increase in D-dimer level was observed in most patients and was not associated with clinical manifestations of thrombosis. At a D-dimer level of 11,000-10,564 ng/ml the patients had clinical symptoms of thrombotic condition which was confirmed by computed tomography or ultrasound examination. At an increase over 725-7000 ng/ml, there were no clinical signs of thrombosis. We performed standard medical therapy in patients with PE and DVT. In case of arterial thrombosis open surgery was performed in 4 patients and direct catheter thrombolysis in 1 case. Conclusion. In our opinion, anticoagulants should be prescribed for patients with a D-dimer level of more than 700 ng/ml. Full anticoagulation is prescribed for severe forms of COVID-19 or confirmed thrombosis. We didn’t find any correlation between the development of thrombotic complications in patients with COVID-19 and comorbidities, body mass index and other factors. Medical therapy was successful in all cases of PE and DVT, anticoagulants were prescribed for 1 year. Femoral artery thrombectomy was successfully performed in 4 patients. After direct catheter thrombolysis we got peripheral pulse. In all cases of arterial thrombosis, we prescribed anticoagulants for 2 months after discharge and acetylsalicylic acid for a long time.
Т ромбоэмболия лёгочной артерии (ТЭЛА) -это частичная или полная эмболия ствола и ветвей лёгочной артерии тромбами. Чаще источником эмболии является бассейн нижней полой вены. Примерно у 50% больных тромбозом глубоких вен (ТГВ) при сканировании лёгких выявляют асимптомную ТЭЛА [1].Острая ТЭЛА является распространённым и часто смертельным заболеванием, требующим неотложной медицинской помощи. Заболеваемость продолжает оставаться высокой даже в развитых странах: 0,5 впервые выявленных случаев ТЭЛА на 1000 населения в год [2]. Уровень летальности в острой фазе варьирует в очень широких пределахот 7 до 11% [3]. Около 10% пациентов с выявленной острой ТЭЛА погибают в течение 3 месяцев после постановки диагноза [4].Первоочередная задача при лечении ТЭЛА -раннее устранение лёгочной обструкции, которое приводит к быстрому уменьшению давления и сосудистого спазма в лёгочной артерии, что сопровождается улучшением функции правого желудочка [5]. С целью разработать показания к тромболитической терапии изучены непосредственные результаты лечения 231 больного с ТЭЛА с использованием комплексной терапии, включавшей тромболизис и/или антикоагулянтную терапию, в том числе с применением новых пероральных антикоагулянтов, и лечение острой лёгочной гипертензии. Для оценки результатов лечения использовали ангиопульмонографию, эхокардиографию, мультиспиральную компьютерную томографию грудной клетки. Среди пациентов преобладали группы людей пожилого и старческого возраста с сопутствующей хронической патологией сердечно-сосудистой и дыхательной систем. Это достоверно ухудшает течение ТЭЛА. Поэтому применяли тромболизис при массивной и субмассивной ТЭЛА, а также препараты простагландина Е1 для лечения острой лёгочной гипертензии и новые пероральные антикоагулянты. Установили, что данная схема лечения приводит к значимому клиническому улучшению, восстановлению адекватного лёгочного кровотока и уменьшению лёгочной гипертензии.
