Purpose of the study. Study ST2 diagnostic marker in the development and severity of heart failure, evaluation of transplant status and the risk of developing a rejection crisis, as well as the risk of death in patients with cardiovascular disease.Material and methods. There were 41 patients under observation. The cases were conventionally divided into two groups: the first group of patients with chronic heart failure (n = 28), and the control group who performed orthotopic transplantation of the heart (n = 13).Results and discussion. These results suggest that ST2 is a real marker of chronic heart failure or a good predictor of mortality in decompensated patients. Changes in ST2 levels in patients after orthotopic cardiac transplantation may be potentially useful in detecting acute cellular rejection, as well as in controlling rejection therapy. The article is devoted to the analysis of the prognostic role of the ST2 biomarker in the pre and post-transplantation period. Purpose of the study. The purpose of the work is to assess the effectiveness of using mechanical support for blood circulation in potential cardiac recipients who need urgent transplantation.Material and methods. The study included 47 patients and divided into two groups. The first group included 8 potential cardiac recipients, who had established mechanical support for the BiVAD blood flow. The second group included 39 potential cardiac recipients, who had LVAD's mechanical support for the circulatory system. The main disease that led to the development of terminal congestive heart failure was dilated cardiomyopathy. The INTERMACS scale was used to assess the patient's status. A Scale for the Classification of Patients with Progressive Heart Failure. We used the UNOS scale to assess the status of the patient who needed orthotopic cardiac transplantation. The algorithm of the Thoracic Committee of a single size distribution of organs for determining the urgent implementation of cardiac transplantation.Results and discussion. Biventricular support is necessary for patients with high central venous pressure, increased pulmonary vascular resistance or with malignant arrhythmias, resistant to drug therapy. For the purpose of the final therapy and biventricular support, the system of choice can be a fully artificial heart.Conclusions. Indications for biventricular circulation support are pronounced hemodynamic manifestations of impaired pumping function of the right and left ventricles of the transplanted heart. Systems DK (VAD) is the most effective «bridge to transplantation». It is important to choose the device for the patient, not the patient for the device. Т ромбоэмболия лёгочной артерии (ТЭЛА) -это частичная или полная эмболия ствола и ветвей лёгочной артерии тромбами. Чаще источником эмболии является бассейн нижней полой вены. Примерно у 50% больных тромбозом глубоких вен (ТГВ) при сканировании лёгких выявляют асимптомную ТЭЛА [1].Острая ТЭЛА является распространённым и часто смертельным заболеванием, требующим неотложной медицинской помощи. Заболеваемость продолжает оставаться высокой даже в развитых странах: 0,5 впервые выявленных случаев ТЭЛА на 1000 населения в год [2]. Уровень летальности в острой фазе варьирует в очень широких пределахот 7 до 11% [3]. Около 10% пациентов с выявленной острой ТЭЛА погибают в течение 3 месяцев после постановки диагноза [4].Первоочередная задача при лечении ТЭЛА -раннее устранение лёгочной обструкции, которое приводит к быстрому уменьшению давления и сосудистого спазма в лёгочной артерии, что сопровождается улучшением функции правого желудочка [5]. С целью разработать показания к тромболитической терапии изучены непосредственные результаты лечения 231 больного с ТЭЛА с использованием комплексной терапии, включавшей тромболизис и/или антикоагулянтную терапию, в том числе с применением новых пероральных антикоагулянтов, и лечение острой лёгочной гипертензии. Для оценки результатов лечения использовали ангиопульмонографию, эхокардиографию, мультиспиральную компьютерную томографию грудной клетки. Среди пациентов преобладали группы людей пожилого и старческого возраста с сопутствующей хронической патологией сердечно-сосудистой и дыхательной систем. Это достоверно ухудшает течение ТЭЛА. Поэтому применяли тромболизис при массивной и субмассивной ТЭЛА, а также препараты простагландина Е1 для лечения острой лёгочной гипертензии и новые пероральные антикоагулянты. Установили, что данная схема лечения приводит к значимому клиническому улучшению, восстановлению адекватного лёгочного кровотока и уменьшению лёгочной гипертензии.
