Using this pattern recognition by direct immunofluorescence microscopy we discovered several cases of EBA which would otherwise have been erroneously diagnosed as a form of pemphigoid or linear IgA disease.
The prevalence of IBD in psoriasis was approximately 4 times higher than that in the general population, with the highest risk for psoriasis-PsA patients. Psoriasis-CD patients have a mild (early-onset) psoriasis but an earlier-onset and severe CD-phenotype.
BackgroundThe demonstration of tissue-bound immunoreactants by direct immunofluorescence microscopy (DIF) is a valuable parameter in the diagnosis of various autoimmune and immunecomplex-mediated skin diseases. For preservation of tissue-bound immunoreactants, biopsies are usually fresh-frozen in liquid nitrogen or transported in Michel's fixative. But even optimally preserved tissue specimens are no guarantee for the correct diagnosis by DIF, especially when weak to moderate IgG fluorescence of the epidermal basement membrane zone is involved. In such cases false negative results are easily obtained due to the relatively high dermal "background" fluorescence produced by polyclonal anti-human IgG fluorescein conjugates.MethodsIn the present study we have compared the use of normal saline (0.9% NaCl) with liquid nitrogen and Michel's fixative as transport medium for skin biopsies. From 25 patients with an autoimmune skin disease (pemphigus, pemphigoid, lupus erythematosus and vasculitis) four matched skin biopsies were obtained and transported in either saline for 24 and 48 hours, liquid nitrogen, or Michel's fixative for 48 hours.ResultsDirect IF microscopy showed significant reduction of background fluorescence (p < 0.01) and relatively enhanced desired specific (IgG, IgA) staining in biopsies transported in saline. A conclusive or tentative IF diagnosis was reached in 92% after 24 h saline, 83% after 48 h saline, 68% after freezing in liquid nitrogen, and 62% after 48 h Michel's medium (n = 25).ConclusionsWe conclude that transporting biopsies without freezing in normal saline for 24 hours is an adequate and attractive method for routine IF diagnosis in autoimmune and immune complex-mediated dermatoses. The superior results with saline incubation are explained by washing away of IgG background in dermis and epidermis.
Vitiligo is an acquired depigmentory disorder which affects around 1% of the general population. In approximately 50% of patients the onset of vitiligo is in childhood. Although, in adults, vitiligo vulgaris is associated with several autoimmune disorders, in children it is mainly associated with autoimmune thyroiditis. We describe two children with vitiligo who had no further symptoms, but after diagnostic work-up appeared to have an autoimmune thyroiditis. We emphasize that autoimmune thyroiditis may be present or develop in children with vitiligo, and advise to determine thyroid function more routinely at presentation, and closely monitor for thyroid dysfunction during follow-up.
InleidingVitiligo is een verworven huidaandoening, die wordt gekenmerkt door scherp begrensde gedepigmenteerde maculae. De aandoening heeft een prevalentie van 0,1-2% en bij ongeveer de helft van alle vitiligopatie¨nten ontstaan de eerste laesies op de kinderleeftijd of tijdens de adolescentie. 1 Het exacte mechanisme dat leidt tot de depigmentatie is nog onbekend. De meest plausibele theorie is dat het een multifactorie¨le aandoening is met een auto-immuunreactie tegen bestanddelen van de melanocyt. 2 Bij patie¨nten met vitiligo vulgaris zijn in de rand van vitiligolaesies inderdaad T-lymfocyten en antistoffen tegen melanocyten aangetoond. Bovendien is bij patie¨nten met vitiligo een hogere incidentie aangetoond van andere auto-immuunziekten zoals (hashimoto-) thyreoı¨ditis, pernicieuze anemie en bijnierinsufficie¨ntie. 3 Bij kinderen met vitiligo bestaat er met name een verhoogde kans op de aanwezigheid van een auto-immuunthyreoı¨ditis. 4 Aan de hand van de volgende casus laten wij zien dat men bij kinderen met vitiligo bedacht moet zijn op de aanwezigheid of het mogelijk ontstaan van thyreoı¨ditis, ondanks dat hier in de anamnese en bij het lichamelijk onderzoek aanvankelijk geen aanwijzingen voor zijn.
CasusPatie¨nt A, een 15-jarig meisje, werd door de dermatoloog naar de polikliniek kindergeneeskunde verwezen in verband met afwijkende schildklierhormoonwaarden. Zij was aanvankelijk door de dermatoloog gezien voor evaluatie van haar vitiligo op romp, extremiteiten en in het gelaat. Door de dermatoloog werd aanvullende diagnostiek verricht naar mogelijke andere auto-immuunaandoeningen. Er werd een sterk verlaagde waarde gevonden van het thyreoı¨dstimulerend hormoon (TSH) van 0,001 mE/l (referentiewaarde 0,5-4,0) en een verhoogde waarde van het vrij thyroxine (fT 4 ) van 36,8 pmol/l (11,0-20,0). Bij verder navragen bleek patie¨nte sinds negen maanden klachten van onrust, hartkloppingen en een toegenomen eetlust te hebben. Er was geen sprake van gewichtsverlies. Er bestonden geen klachten van de ogen. Haar menarche had anderhalf jaar voor het eerste poliklinische bezoek plaatsgevonden. In verband met hypermenorroe gebruikte zij microgynon. In de familie kwamen geen schildklieraandoeningen voor.
