Prevalence and Phenotype of Concurrent Psoriasis and Inflammatory Bowel Disease

Abstract: The prevalence of IBD in psoriasis was approximately 4 times higher than that in the general population, with the highest risk for psoriasis-PsA patients. Psoriasis-CD patients have a mild (early-onset) psoriasis but an earlier-onset and severe CD-phenotype.

“…To our knowledge, this is the first study to link a discriminative association of IBD and/or acute anterior uveitis, as opposed to psoriasis, to structural progression in axial SpA. It is well-known that the genetic predisposition and cytokine-driven pathways of AS and psoriasis, acute anterior uveitis, and IBD are closely linked (1,(17)(18)(19)(20)(21). The common physiopathology in the spectrum of SpA is illustrated by the shared success of anti-TNF therapy.…”

“…The pathophysiology of this paradoxical skin reaction may be related to a cytokine imbalance and has also been reported to occur in other biologics . Unlike the link between severe psoriasis and increased risk of psoriatic arthritis and cardiovascular disease, patients with concomitant psoriasis and inflammatory bowel disease were found to have a mild psoriasis phenotype similar to patients with psoriasis without inflammatory bowel disease . Patients with psoriatic arthritis were more likely to have an autoimmune disease than those with only cutaneous disease .…”

“…Although the precise mechanisms underlying the autoimmune co‐morbidities are not completely understood, damage to the basement membrane, melanocytes and hairs by chronic psoriatic inflammation may initiate and accelerate skin‐related autoimmune diseases. An association of Crohn's disease and collagen diseases, such as systemic lupus erythematosus, rheumatoid arthritis, Sjögren's syndrome and systemic sclerosis, has been demonstrated in psoriasis patients . Some patients with psoriasis develop collagen diseases upon treatment with biologicals …”
